A patient with Wegener's classical vasculitis is presented with affection of both lungs paranasal sinuses, kidney, nervous system and ulceronecrotic lesions of superior and inferior limbs' fingers. The importance of cytoplasmic antineutrophilic antibodies (CANN) is stressed in the diagnosis of this disease; as well as the answer to ciclophosphamide in a relevant way in our patient. Literature on the concept of Wegener's disease in a limited fashion is reviewed and the differential diagnosis with lethal granuloma of middle line is discussed. Treatment is updated and the need for the creation of an early treatment plan in all cases is analyzed.
