A patient with spontaneous rupture of the esophages, diagnosed at a late phase is presented. Possible etiologies, importance of the early diagnosis, its differential diagnosis and complications as well as the need of combining an antibiotic therapy and a carefull nutrition are commented. Concluding, excellence therapeutic is the surgical intervention.
