With the neuromonitoring and intensive management of the several cranioencephalic trauma a group of different pathophysiological events, which require dissimilar and specifical measures for each patient have been identified making more rational to treatment of them. This has brough about radical changes in the old therapeutic patterns established during years, such as management of hydric reposition trying to keep euvolemia, the substitution of rutine hyperventilation by optimum ventilation and changes in the politics for the use of osmotic agents, barbiturate therapy and descompressive craniotomy in cases of refractory intracranial hypertension to conventional therapeutic measures. In this work, we make an update review of the main therapeutic measures, specific for the management of endocranial hypertension, associated to the critical neurological state of traumatic, which we divide into two phases: actions of first line and those of the second line for the treatment of refractory endocranial hypertension.
