The presurgery discussion is a graduate session intended to assess, select, and describe the most suitable neurosurgical procedure. Most residents, however, feel insecure when having to face PD by themselves. To describe PD, propose a guide for its practice, and point out some dos and don’ts. When faced with PD, the resident must not lose sight of a) the clinical and imaging profile of the disease, b) the patient’s status, c) the conditions of the surgery ward, and d) the ethical standards involving both the patient and his/her family. The procedure being chosen will aim to modify the natural course of the disease. There are three types of presurgery discussions individual, collective, and scholarly. Each one serves a purpose of its own. A patient status may require more than one discussion session. The PD considers the patient’s position, the kind of anaesthesia, and the procedure itself, which in turn spans three stages, boarding, essential procedure, and synthesis. As the PD lays the gnoseologic foundations for surgery, residents are to select the most suitable procedure, which becomes a tool capable of modifying the course of the condition. PD is of relevance for resident training and performance when considering the complexity of surgical therapy and the challenges posed by a much-demanded surgical unit.
