Background: müllerian mixed malignant tumors are very infrequent neoplasms. The commonest site of occurrence is in the female genital tract; inside him the body of uterus, uterine neck, ovary, and tuba uterina appear. Case presentation: a great difficult diagnostic case both clinical and imaging is presented, with histological confirmation of giant müllerian mixed malignant tumor of cervix and body of uterus in a postmenopausal woman who was presented with a large abdominal mass and a radiotherapy history of eight years ago for a uterine neck carcinoma. To the initial medical examination and computerized axial tomography appeared to be an ovarian tumor, during the surgical act appear like giant and deformed irregular uterus tumor, v/s endometrial stromal sarcoma, however, the pathological examination revealed a müllerian mixed malignant tumor of cervix and body of uterus. The malignant epithelial component was a squamous carcinoma with a homologous sarcomatous component. The patient was surgically treated. Conclusion: Accurate diagnosis of cervix MMMT is important for the patients appropriate treatment.
