Introduction: anterior pain of the knee constitutes an important cause in the consultation of Orthopedics and Traumatology specialty. The incidence of other illnesses related with this symptom is every time greater, example of it is chondromalacia patellae, tendinitis patellar, patellofemoral osteoarthritis among other, and the diagnosis of these diseases is due to the knowledge gained, the introduction of techniques and advanced imaging equipments. Development: a bibliographical review on anterior pain of the knee was performed, with special emphasis in the theories involved in its physiopathology; among those are the patello-femoral bad-alignment, equilibrium of the tissular homeostasis and increase of the intraosseous pressure, besides offering the clinical picture of the disease briefly. Finally the therapeutic approach is exposed that is based fundamentally on the conservative treatment, also are mentioned the modalities of the surgical treatment.
