Background: the hemodynamic monitoring with Swan-Ganz catheter is a method of diagnosis in the field of Cardiology and reached maturity providing to the anesthetist an invaluable aid in the control of the patient in the perioperative period. Objective: to demonstrate the benefits of its use in the integral analysis of patients with precarious hemodynamic states during their first 72 hours of evolution. Method: a descriptive prospective study was conducted in the intensive care unit at the University Hospital Manuel Ascunce Domenech from January 1989 to January, 2011 (22 years). The universe of study was constituted by 480 patients between 15 and 70 years, with precarious hemodynamic state, cardiogenic origin 280 patients and non-cardiogenic 192. Results: it required changes of the catheter in the diagnostic and treatment 67 patients and only changes in diagnosis in 36; changes in the treatment 51 patients, with no changes 107. With regard to the influence on treatment, kept equal in 50 patients, remained equal but was increased or decreased the dose in 200; continued equal treatment and added another in 110 and changed completely in 121 patients. Several complications were detected, including arrhythmias in 50 patients, dislocation of the catheter in 22. It showed no significant changes in mortality with the use of this catheter, which is not a purpose of analysis. Conclusions: the rational, well justified use of the Swan-Ganz catheter was useful in the management of critical patients and in the correct interpretation of data provided; it must carried out a rigorous training to doctors and nurses in the technique to place it.
