Resumo

Introduction:

the stroke has classically been characterized by a neurological deficit attributed to an acute focal lesion of the central nervous system by a vascular cause, including cerebral infarction, intracerebral hemorrhage and subarachnoid hemorrhage.

Objective:

to identify the tomographic variables that influence the prognosis of short-term death in ischemic stroke.

Method:

a single cohort analytical study was carried out that included 185 patients admitted for acute cerebral infarction of ischemic etiology confirmed by CT, at the "Carlos Manuel de Céspedes" Hospital in Bayamo, from September 2016 to September 2018. The variables used in the study were age, topography, presence of cerebral edema, mass effect, coexistence of old lesions, hemorrhagic transformation of the infarct and loss of differentiation between gray and white matter. The independent predictor variables were obtained as a result of the multivariate strategy that was based on the adjustment of a binary logistic regression model.

Results:

brain edema, mass effect and greater diameter greater than 5.5cm showed direct influence on prognosis.

Conclusions:

in patients with acute cerebral infarction of ischemic etiology, brain edema, mass effect and greater diameter greater than 5.5cm have a direct influence on the prognosis of death, significantly increasing the risk of dying before 30 days.
