A retrospective, descriptive and cross-sectional study was conducted in 94 patients with community-acquired pneumonias -- selected by systematic probability sampling -- admitted to "Dr. Joaquín Castillo Duany" Teaching Clinical Surgical Hospital of Santiago de Cuba during 2011 to assess the clinical applicability of the Fine index. In the series categories 3 and 4 (high risk) accounted for the highest percentages in terms of hospital stay longer than 10 days (50.0%), the possibility of transfers to the Intensive Care Unit (28.0%) and hospital mortality (13.0%); similarly, the most frequent risk factors and clinical changes were age of 70 years and over, history of stroke, and heart disease or chronic obstructive pulmonary disease; altered mental state and heart rate greater than 125 beats per minute during admission. The Fine index was not useful to evaluate antimicrobial use (single o combined) or to define who should meet the treatment on an outpatient basis or in the health institution; however, it is useful in the clinical management of patients in the categories of high risk, but does not replace clinical assessment in medical decision making
