Introduction: biomarkers such as C-reactive protein (CRP) have an important role in the diagnosis of acute coronary syndromes. Objective: behavior of quantitative CRP as a biomarker of acute myocardial infarction with S-T segment elevation. Method: analysis of series of cases to specify the behavior of the CRP in the diagnosis of acute myocardial infarction with S-T segment elevation. The study group consisted of 71 patients evaluated prospectively and subsequently admitted to the Emergency Service of Vladimir I. Lenin Hospital from December 2009 to January 2011.  CPK Mb, SGOT and serial electrocardiograms were determined. Chi-Squared test was applied with a significance level of 95%. Results: the mean age of patients was 63.7, with a mean deviation of 12 years approximately with a predominance of men (62%). The CRP increased above 6 mg/l, it was significantly associated with ST elevation and the values ​​of CPK Mb and SGOT. The CRP showed a sensitivity of 89.5%, specificity of 71.4%, positive predictive value 92.7% and negative predictive value of 62.5% for the diagnosis of acute myocardial infarction. Conclusions: the conventional quantitative CRP is an advantageous procedure, simple and economical for the diagnosis of acute coronary syndrome in the form of myocardial infarction with ST elevation.
