This paper is aimed at reflecting on the non-communicable diseases and their long-term follow-up, as well as on the learning problem. The premises, challenges and practical consequences of this follow-up model are discussed. If a rapid diagnosis and a proper treatment may be enough in an acute health situation, it is very different with the chronic patients, where the diagnosis leads to certain frustration and the treatment relies mostly on their ability and motivation that requires some learning and, therefore, new challenges to care management. To prevent these diseases the health diagnosis of the community and the interdisciplinary and intersectorial character of the programs of promotion and health education are very important. Besides knowing how to do and being clinically updated, health providers need certain skills to be efficient in patient education. It is necessary to develop life styles and healthy environments and to attain the indissoluble unity of care management and education. It is stated that the long-term follow-up requires a different care paradigm that makes health providers to surpass the physiological vision of the damaged organ and to focus on the sick individual and help him to live without an unnecessary handicap and to play an active role in the development of his health status.
