Introduction: the influenza is one of the most infectious diseases of respiratory transmission having an acute presentation of greatest incidence all over the world; its prevention is mainly supported on vaccination. Objective: to determine the impact of anti-influenza vaccination in a residential home for the elderly in Pinar del Rio province all along 2004-2001. Material and method: a descriptive, longitudinal and retrospective study which included two moments, one from the years 2000-2003 when old people did not get vaccinations and the other from 2004 to 2011 when they were vaccinated. The target group and the sample included all the elderly of the residential home, who were divided into those vaccinated and non-vaccinated. Data were collected from the registers of the hospital movements, chronological series and vaccination records, general data of patients, secondary adverse reactions, incidence of acute respiratory infections and complications associated before and after vaccination were as well collected. Measures to sum-up the quantitative data (absolute frequencies and percentages) were calculated. Results: acute respiratory infections and secondary complications decreased after the anti-influenza vaccination and the local adverse effects prevailed over the systemic ones. Conclusion: findings showed a positive effectiveness after the anti-influenza immunization from the year 2004.
