Introduction: traumatic cataracts are a challenge for the surgeon of the anterior segment due to the large number of factors to be taken into account in its approach. The backgrounds to the type of trauma are vital, which together with a thorough physical examination determines the appropriate surgical approach and surgical strategy. Case report: a male patient, 26 years old, who suffered blunt trauma with a blunt object in the left eyeball, which then began to occur a progressive visual loss. The ophthalmologic review found: nasal pterygium sector and opacities level lens (cataract rosette) with iris-phacodonesis without the presence of vitreous in the anterior chamber. The affected patient's pupil was dilated, confirming zonular rupture of just over 90 degrees. At the bottom of indirect eye, a normal posterior pole without trauma injuries perceived product. After this assessment was diagnosed with traumatic cataract complicated. Conclusions: the solution of this disease was surgical.
