Introducción:

a pesar del tratamiento antirretroviral y su amplia cobertura actual, el VIH/SIDA representa un problema de salud en aumento, y con él, las alteraciones de la composición corporal.

Objetivo:

evaluar la influencia de factores propios del individuo, del VIH y el tratamiento antirretroviral, sobre los cambios longitudinales de la adiposidad corporal, en personas con VIH/SIDA.

Métodos:

estudio longitudinal-retrospectivo con datos de 159 personas VIH/SIDA de Pinar del Río, Cuba, (72,3 % hombres) con al menos tres determinaciones: índice de masa corporal, pliegue cutáneo tricipital, circunferencia de la cintura y cadera, en un período de 1,3 años. Se estimaron modelos longitudinales con dos niveles, mediante regresión multinivel para medidas repetidas.

Resultados:

solo la circunferencia de la cintura e índice cintura/cadera mostraron cambios longitudinales significativos (p<0,001). El sexo, el tiempo con terapia y el índice cintura/cadera inicial, fueron las variables predictoras que aportaron de manera significativa (p<0,1) al incremento del índice cintura/cadera en los modelos de regresión. El sexo masculino aportó un incremento de 4,2 % al índice cintura/cadera. Por cada año con antirretrovirales el índice cintura/cadera aumentó en 0,63 % y el índice cintura/cadera que tenían los individuos al finalizar el estudio representó el 78,1 % del índice cintura/cadera inicial más un valor constante de 20 %.

Conclusiones:

los indicadores de adiposidad central fueron los de mayor utilidad para estimar los cambios longitudinales de la adiposidad corporal en individuos VIH/SIDA, el ICC inicial fue el mejor predictor de dicho indicador al finalizar el período de seguimiento.

Introduction:

despite antiretroviral treatment and its current wide coverage, HIV/AIDS continues as an increasing health problem, and with it, alterations in body composition.

Objective:

to assess the influence of individual factors, HIV and ART, on the longitudinal changes in body fat in people with HIV/AIDS.

Methods:

longitudinal-retrospective study that included the data of 159 HIV/AIDS people from Pinar del Río, Cuba, (72,3 % men) with at least three determinations of body mass index, tricipital skin-fold thickness, waist and hip circumference, over a period of 1,3 years. Longitudinal models with two levels were estimated using multilevel regression for repeated measurements.

Results:

only waist circumference and waist-to-hip ratio showed significant longitudinal changes (p<0,001). Sex, time with antiretroviral treatment and the initial waist-to-hip ratio were the predictor variables that contributed significantly (p<0,1) to the increase of the waist-to-hip ratio in the regression models. Male sex contributed with an increase of 4,2 % to the waist-to-hip ratio. For each year with antiretroviral treatment the WHR increased by 0,6 % and the waist-to-hip ratio that the individuals had at the end of the study represented 78,1 % of the initial WHR plus a constant value of 20 %.

Conclusions:

the indicators of the central abdominal fat were the most useful to estimate the longitudinal changes of body fat index in HIV/AIDS individuals, the initial WHR was the best predictor of this indicator at the end of the follow-up period.
