Introduction:Polyserositis, the result of simultaneous inflammation of several serous membranes, is associated with various diseases, including multisystem inflammatory syndrome in pediatrics. This rare but serious syndrome can develop after a SARS-CoV-2 infection, even in mild or asymptomatic forms of COVID-19.

Objective:

provide a complete and educational description of this clinical case, emphasizing the clinical relevance and comprehensive approach in the management of this complex condition.

Case presentation:

a case of suspected multisystem inflammatory syndrome in an 8-year-old boy with no previous personal pathological history is presented. The described clinical picture of insidious abdominal pain of several days of evolution and fever, together with the physical examination performed due to suspicion of signs of inflammation with altered values of laboratory tests, leads to indicating an exploratory laparoscopy, where serous inflammatory fluid was found in the abdominal cavity and signs of appendiceal inflammation (classified as phase II appendicitis). In addition, signs of dehydration and neurological compromise were observed. The literature review supports the suspicion of polyserositis associated with multisystem inflammatory syndrome.

Conclusions:

the clinical method and complementary examinations in the present case contributed to confirming the diagnosis of polyserositis associated with multisystem inflammatory syndrome that could be related to COVID-19.
