Introduction: The non-cardiogenic pulmonary edema secondary to a negative pressure is a infrequent complication occurring after the high airway obstruction with a higher presentation rate in children, in a infrequent way in adults and no recounted for the patient presenting with morbid obesity in reviewed bibliography. Objective: To describe the perioperative behavior and the course of a morbid obese patient with pulmonary edema due to negative pressure. Case presentation: Its etiopathogenesis is varied, but is specially emphasized the negative intrathoracic pressure provoked by the spontaneous forced inspiration in a patient with a closed glottis (laryangospasm) resulting in fluid transudation of pulmonary capillaries to alveolointerstitial accompanied by hemorrhage or not. The resultant pulmonary edema appears in a spectacular way few minutes after airway obstruction or in a different way after some hours. This clinical picture potentially severe, habitually respond well to treatment con oxygen therapy, mechanical ventilation with positive pressure and diuretics. Conclusions: It is essential the suspicion diagnosis to adapt the treatment. Our experience with a clinical case of a morbid obese patient presenting with pulmonary acute edema due to negative pressure is presented.
