Introduction: Obese patients are classified as cases of high-risk pregnancy, with a high cesarean delivery rate and difficult anesthetic management during surgery, due to the anatomical conditions to be faced by both surgeons and anesthesiologists. Objective: Determine which anesthetic technique (general anesthesia or central neuraxial blocks) has a greater impact on the Apgar score of babies born to morbidly obese mothers by cesarean section. Methods: A prospective longitudinal correlational study was conducted of a universe of obstetric patients with a diagnosis of morbid obesity who underwent anesthetic procedures for cesarean delivery at Dr. Agostinho Neto hospital in the province of Guantánamo from December 2009 to December 2013. The chi-square test was used to determine the relationship between anesthetic technique and fetal wellbeing according to the Apgar score of the newborns. Results: Hypertension predominated among women undergoing cesarean delivery under general anesthesia, whereas this technique raised the risk of depression at birth. This group showed the greatest alteration in the Apgar score. Peridural anesthesia caused the lowest depression incidence. Hypotension predominated in the group of mothers whose children had moderate depression. A correlation was found between anesthetic technique and fetal wellbeing according to the Apgar score. Conclusions: There is a relationship between anesthetic technique and fetal wellbeing.
