Syphilis is a chronic and infectious disease as old as mankind. Its secondary stage is characterized mainly by skin lesions and mucous , but in some cases it can affect important organs as the eyes and the liver, where it can cause syphilitic hepatitis. A case of a male patient with cutaneous and mucous manifestations of jaundice is presented, whose onset was concomitant with skin lesions suggestive of a secondary syphilitic lesion. The patient was treated for this cause and the clinical symptoms did not improve, so he was performed an endoscopic retrograde cholangiopancreatography which reported the presence of a pancreatic tumor process, which invaded entirety this organ and infiltrated bile ducts. The case is presented as suspicion of possible syphilitic hepatitis, a rare condition in our country, with a pancreas tumor and to highlight the importance of the differential diagnosis of syphilitic hepatitis.
