The Charcot neuroarthropatic is a strange illness, of progressive character. It is associated with serious outlying neuropathic. It is characterized by inflammation, luxation to articulate and bony destruction with later deformation of the foot. It is important to suspect it above all diabetic patient with swelling and blush in ankle or foot, with absence of vibratory sensibility and default reflective. At the moment the most frequent cause is the diabetes mellitus but equally it can associate to infectious illnesses as it is the case of the osteomyelitis.  In this work they are described a case of arthropatic secondary neuropathic to an osteomyelitis in a diabetic patient diagnosed in the rheumatology center and it becomes special mention to the element that should guide to the diagnosis of this entity in their initial phases.
