La hidrocefalia obstructiva por anomalías del acueducto de Silvio o lesiones del cuarto ventrículo no son frecuentes en el adulto, a pesar de ser uno de los sitios más vulnerables del cuerpo humano por su diminuto tamaño de 11 milímetros de largo y 0,9 de diámetro. Hasta hoy no existe una definición internacionalmente aceptada del término hidrocefalia, pero sí es conocida como la condición que presenta un aumento en el volumen del líquido cefalorraquídeo (LCR) circulante. El tratamiento habitual es la colocación de un sistema de derivación de LCR. Se presenta un paciente masculino de 55 años de edad sin antecedentes de enfermedades crónicas, refiere comenzar hace meses con alteración en la marcha e incontinencia de esfínter, mencionando la familia que presenta deterioro cognitivo progresivo; acompañan la sintomatología vómitos en proyectil frecuentes. Se solicita Resonancia Magnética donde se aprecia la dilatación ventricular por el acumulo de LCR con estrechamiento del acueducto de Silvio elcual se muestra en un video en 3D de la imagen cerebral del paciente), debido a ello recibió tratamiento quirúrgico. Es de gran importancia el diagnóstico temprano de esta patología para mejorar el área cognitiva del paciente lo cual conlleva a mejor calidad de vida.

Obstructive hydrocephalus due to anomalies of the aqueduct of Silvius or lesions of the fourth ventricle are not frequent in adults, despite being one of the most vulnerable sites in the human body due to its tiny size of 11 millimeters long and 0.9 millimeters in diameter. To this day there is no internationally accepted definition of the term hydrocephalus, but it is known as the condition that presents an increase in the volume of circulating cerebrospinal fluid (CSF). The usual treatment is the placement of a CSF bypass system. We present a 55-year-old male patient with no history of chronic diseases, who reports starting a few months ago with gait disturbance and sphincter incontinence, mentioning the family that presents progressive cognitive impairment; The symptoms are accompanied by frequent projectile vomiting. Magnetic Resonance is requested where ventricular dilation is appreciated by the accumulation of CSF with narrowing of the Silvio aqueduct (which is shown in a 3D video of the patient's brain image), reasons for which he received surgical treatment. Being of great importance the early diagnosis of this pathology to improve the cognitive area of the patient which leads to better quality of life.
