Background: vocational guidance promotes a correct choice of studies according to the needs of the social and labor context. Methods: a cross-sectional study was conducted from February to March 2013, with the aim of characterizing the behavior of vocational guidance in the decision to opt for medical studies. The universe consisted of the 107 first and second year medical students, who study at the "José Ramón León Acosta", "Marta Abreu", "Santa Clara" and "XX Aniversario" Teaching Polyclinics. A documentary analysis was conducted; it included the regulatory rulings for this work, and records of interviews that were conducted at the beginning of the school year. Interviews with key informants were conducted, and a questionnaire was given to students. A database was created with the SPSS software, and the information was processed with its help. Results: it is found a predominance of females, who were also the ones that showed more determination in choosing their studies. In the study, 57.94 % of respondents reported having relatives who work in the health sector, and 71.03 % of them sought medical studies as the first option. Respondents believe that a proper coordination must be achieved between the different levels of education. Conclusions: some elements associated with the choice of medical studies are highlighted; and it is emphasized the need to prepare students to make their self-assessment during the choice of their studies, and in their vocational training.
