Medication errors can occur at any of the stages of use of medicines, there are possibilities to prevent them with the participation and effort of all those involved to avoid them. In Cuba, in 2018 the reformulation of heparin was carried out, as part of the continuous quality improvement. Being a new product in the national market, the present work is justified. To evaluate the behavior of Heparin sodium 5000 IU/mL, through the detection of adverse effects related to medication errors, through spontaneous notifications, from June 2018 to December 2021, in Cuba. An observational, descriptive and cross-sectional case series study was carried out using the method of evaluating spontaneous notifications. They make up the universe, all the notifications of this product, which appear in the national database. Variables in studies: temporal sequence, pathological history, medical prescription, route of administration, number of doses used, personnel reporting the notification, level of care where these appeared, and region of the country where they were manifested. 31 adverse reactions were reported, corresponding to patients with some reaction after receiving treatment. The temporal sequence and the pathological history facilitated the analysis of the degree of association of the appearance of adverse reactions with the administration of the drug. The reasons for prescription, routes of administration and doses used, complied with the indications of this product that appear in the summary of the characteristics of the same. The highest number of notifications were reported by physicians. Santa Clara reported the highest number of 10 cases (32,25%). At the time under study, no adverse effects related to medication errors were detected, which suggests that the use of Heparin sodium 5000 IU/mL does not constitute a public health problem.
