The indices used in the clinical laboratory provide evidence of the organs and systems function in different conditions. They also allow evaluating the effect of drugs, adverse reactions that constitute important aspects of clinical trials and the Pharmacovigilance strategy. Glomerular filtration is the best index to assess kidney function. Serum creatinine is the general glomerular filtration marker, despite being subject to various sources of variability. Cystatin C is a low molecular weight protein more sensitive than creatinine, particularly for the initial identification of small decreases in renal function. The objective of this work is a bibliographic review using as search criteria: Cystatin C as a biomarker of kidney function, types and classification of kidney damage, drugs that cause kidney damage or failure, differential diagnosis and biomarkers used to determine kidney damage. A total of 1430 articles were found. After filtering, 185 were determined to be useful and of sufficient quality to be used. Updated definitions, general aspects related to Cystatin C, its quantification in various conditions and also its modification during the use of different medications were evaluated and discussed. The use of cystatin C in certain groups of patients such as the elderly, children, diabetics or individuals infected with the human immunodeficiency virus seems to provide more information than creatinine. Biomarkers used in clinical laboratory tests for diagnosis, prognosis and monitoring of patients with renal damage constitute a topic of current research.
