Presentación de dos pacientes con hemicorea vascular

 

Report of Two Patients with Vascular Hemichorea

RESUMEN

El término corea procede del griego choreia, que significa danza. Conocida desde la Antigüedad, en la Edad Media esta enfermedad se extendió como una epidemia por toda Europa por los caminos que recorrían los peregrinos. El término se aplica a los amplios movimientos arrítmicos de tipo espasmódico, rápido y potente, que aparecen como rasgo dominante en la enfermedad de Huntington, corea Sydenham y en la variedad de esa enfermedad asociada con el embarazo (corea gravidarum), está relacionada al uso de neurolépticos y con otras situaciones. Puede estar limitada a un lado del cuerpo (hemicorea), es poco frecuente en la enfermedad cerebrovascular aguda, su aparición está relacionada con la afectación contralateral de los ganglios basales, sobre todo del estriado. Se describieron dos casos clínicos: una paciente femenina de 72 años de edad y un hombre de 67 años. En ambos casos los signos y síntomas aparecieron de forma súbita. En los estudios complementarios se constataron lesiones en ganglios basales, núcleo caudado en un caso (mujer) y tálamo en el  otro caso  (hombre), en ambos enfermos las lesiones fueron isquémicas, contralaterales al hemicuerpo afectado. Las lesiones encontradas tienen gran variabilidad en su localización, este hecho refuerza el concepto de red funcional global de los ganglios basales que puede ser interrumpida en varios puntos y originar una misma clínica.

ABSTRACT

Chorea derives from the Greek word meaning dance; it refers to involuntary arrhythmic movements of a forcible, rapid, jerky type. Typical choreic movements are the dominant features of Huntington disease, Sydenham chorea and of the variety of the disease associated with pregnancy (chorea gravidarum), it is asociated with Neuroleptics and other situations. Chorea may be limited to one side of the body (hemichorea). Hemichorea is uncommon in acute cerebrovascular disease. Its appearance is in relation to the contralateral involvement of the basal ganglia, particularly the striate ganglion.Two clinical cases were described: a 72- year- old female patient and a man of 67 years. In both patients the clinical features had an abrupt onset. Complementary studies showed lesions in the basal ganglion were found. The site was caudate nucleus in one case (woman) and thalamus in the other case (man). Both lesions were ischemic type, contralateral to the side of the affected body. The lesions were found in different parts of the extrapiramidal system, this fact supports the functional global network of the basal ganglio which may be interrupted at several points and lead to the same clinical findings.
