Resumen

Con el objetivo de identificar las publicaciones realizadas por autores cubanos para informar sobre los resultados de ensayos clínicos realizados en el país, tanto en revistas nacionales como extranjeras, así como de ubicar la posición de Cuba con respecto al resto de los países iberoamericanos y de la región, se exploró la base de datos PubMed/Medline, disponible en Internet. Adicionalmente, se examinaron las bases de datos del Web of Science. Se hallaron 142 referencias a informes de ensayos clínicos publicados por autores cubanos, con la participación o sin ella de autores de otros países, en Medline. De ellas, 116 se encontraron en publicaciones extranjeras y 26 en nacionales. Cuba se ubicó en el sexto lugar entre los 20 países analizados del área, según la publicación de informes sobre ensayos clínicos, donde el primer autor labora en una organización perteneciente a dicho país. Iberoamérica, como región, generó un número de trabajos similar a la que muestra Finlandia; sin embargo, más de la mitad de esta producción se concentró sólo en dos países: Brasil y México. Es imprescindible una visión, una conciencia, una infraestructura, una organización y una voluntad política de los países de la región para estimular la actividad clínico-investigativa de los países en Iberoamérica.

Abstract

Pub/Medline database, available in the Internet, was explored aimed at identifying the publication of clinical trial result reports by Cuban authors in national and foreing journals as well as the position of Cuba with regard to the rest of the Iberoamerican countries. The Web of Science was also examined. In Medline, 142 references of clinical trial reports published by Cuban authors were found: 116 in foreing publications and 26 in national journals. Cuba is situated in the 6th place among the 20 countries analyzed according to the production reports where the first author correspond to that country. According the last indicator, Iberoamerica produced a work number similar to those produced by Denmark. Nevertheless, most of the half of this production is located in two countries: Brasil and Mexico. A vision, a conciousness, a frame and also a political willingness of the countries to encourage the clinical research activity of Iberoamerican countries is deeply needed

Los ensayos clínicos, un tipo de estudio planificado con anterioridad, para establecer la seguridad, eficacia y dosificación de uno o más drogas profilácticas, terapéuticas o diagnósticas, así como de equipos o técnicas seleccionadas de acuerdo con criterios predeterminados de eligibilidad y evidencias de sus efectos favorables o desfavorables, constituyen la piedra angular de la investigación clínica actual y el soporte esencial de los metanálisis y las revisiones sistemáticas, los estudios integradores y sintetizadores, que conforman, a su vez, las bases donde se sustenta la práctica de la MBE.

La realización de ensayos clínicos, y la consecuente publicación de sus resultados, constituye un indicador confiable para medir el nivel alcanzado por la investigación clínica en un país.

Cuba, un país con un interés especial en el área de los ensayos clínicos, derivado de su pujante industria médico- farmacéutica y de la búsqueda constante de una mejor atención clínica de su población, se enfrenta ahora a la tarea de insertarse en la fuerte corriente de desarrollo que genera la MBE a escala internacional. Y no son pocos los trabajos que se han realizado en un corto periodo de tiempo.1-3

Métodos

Con el objetivo de identificar las publicaciones realizadas con la participación de autores cubanos para informar sobre los resultados de ensayos clínicos realizados en el país, tanto en revistas nacionales como extranjeras, se exploró la base de datos PubMed/Medline, disponible en Internet.

Para ello, se empleó la estrategia de búsqueda siguiente:

Y así sucesivamente, con cada uno de los 20 países integrantes del territorio iberoamericano.
Adicionalmente, y con el objetivo de ampliar la búsqueda de informes de ensayos clínicos publicados por autores cubanos, se realizó una exploración de las bases de datos del Web of Science.
Para este propósito, se utilizó la siguiente estrategia de búsqueda:

Y con el propósito de ubicar la posición que ocupa Cuba entre los países iberoamericanos y de la región con respecto a algunos países desarrollados seleccionados de acuerdo con su producción de informes sobre ensayos clínicos, se realizó una búsqueda por países en la base de datos Medline.

La medición se realizó de acuerdo con el número de informes sobre ensayos clínicos cuyo primer autor se afilia a una institución ubicada en el país estudiado.

Las búsquedas se realizaron entre los días 25 y 27 de agosto del año 2003.

Resultados y discusión

Cuba aparece relacionada en 3 064 registros en Medline (27 de agosto del 2003). No obstante, aparece en 2 192 registros en el campo de afiliación. En Medline, el campo afiliación (AD) registra, como se conoce, la organización donde labora el primer autor del trabajo.

El total de ensayos clínicos publicados con la participación de autores cubanos en revistas extranjeras, identificados en Medline -on line y off line-, asciende a 142. De ellos 26, (18,30) se publicaron en las tres revistas cubanas de medicina procesadas por Medline y 116 (81,69) en revistas extranjeras (tabla 1) (anexos 1 y 2).

El primer informe sobre un ensayo clínico realizado en Cuba, publicado en una revista extranjera y registrado en Medline, corresponde al año 1973. Se trata de un artículo donde se informan los resultados preliminares de un estudio realizado en el país con el cromiglicato disódico. Los primeros ensayos registrados en una revista cubana procesada por esta base de datos se ubican en el año 1974 y corresponden a dos estudios publicados en la Revista Cubana de Estomatología. El primer ensayo clínico aleatorio se publicó en 1948 en la British Medical Journal.

Y es comprensible que Cuba durante las dos décadas que siguieron a la publicación del primer ensayo clínico en el mundo no incursionara en la realización de esta clase de estudios. En 1959, la Revolución se encontró con un país prácticamente sin médicos y sin una infraestructura de salud que posibilitara la atención de la población. La primera prioridad fue entonces crear un sistema de salud nacional que permitiera satisfacer las necesidades de salud de una creciente población, realmente una tarea muy difícil y costosa.

Sin embargo, no es hasta la década de los años 90, cuando la actividad investigativa en el área de los ensayos adquiere un lugar prominente dentro de la investigación en salud en el país. La creación del Polo del Oeste y de otros polos de investigación en el país, unido a las necesidades de la industria médico-farmacéutica cubana impulsaron significativamente la generalización del ensayo clínico como vía para obtener pruebas sólidas sobre la eficacia de los productos nacionales, un requisito indispensable para su comercialización y consumo.

El balance de informes publicados en revistas nacionales y extranjeras, procesadas por Medline, así como las debilidades actuales de las revistas cubanas de medicina aconsejan la edición de estudios clínicos relevantes en revistas visibles internacionalmente, con el objetivo de obtener una mayor difusión de los estudios realizados en el país.

Cuba en Iberoamérica

Cuba, según el indicador utilizado para establecer la posición que ocupa entre los países iberoamericanos, donde se mide su liderazgo en la actividad de investigación en el área de los ensayos clínicos, se ubicó en el sexto lugar detrás de Brasil, México, Argentina, Chile y Venezuela, con 140 registros (tabla 2).

Es oportuno aclarar que esta cifra no se corresponde con el total de referencias a ensayos clínicos,lo que se anexa a esta exposición, debido a las diferencias que introduce la revisión manual de los registros.

Y en este punto es oportuno indicar que Cuba, según estos resultados, se halla en una posición un tanto rezagada con respecto a sus aspiraciones. La acción de factores objetivos y subjetivos, como la carencia de una alta disciplina y profesionalidad editorial entre las revistas cubanas y la carencia de una adecuada cultura de publicación, ha impedido que muchos estudios clínicos de calidad ingresaran oportunamente a las bases de datos internacionales para obtener la visibilidad necesaria.2

En general, se aprecian considerables diferencias en la producción de informes de ensayos clínicos entre los países de la región. Así, por ejemplo, Brasil y México produjeron más trabajos que el resto de los países. Muy pocos países, Brasil, México, Argentina, Chile, Venezuela, Cuba y Colombia han generado 100 o más trabajos, cuyo primer autor pertenece a una institución del país.

Iberoamérica en el mundo

Con el fin de comparar la actividad investigativa en materia de ensayos clínicos de los países iberoamericanos y de la región en general, con respecto al mismo indicador, medido en algunos países desarrollados, se realizaron búsquedas similares a las efectuadas para establecer la participación de los países del centro y el sur de América.

La exploración según regiones de interés contempladas en el MESH, mediante una estrategia que incluyó sólo el nombre del área y el tipo de publicación, permitió establecer la existencia de una cierta inclusión de América del Sur (859), América Latina (267) y América Central (156) en las investigaciones clínicas, en particular como objeto de estudio de ensayos realizados simultáneamente en varios países.

Es imprescindible no sólo incrementar la actividad clínico-investigativa de la región como un todo, sino fortalecer la actividad en cada país en particular. Sin embargo, dicha tarea es sumamente ardua y, sin una conciencia nacional sobre su importancia, una infraestructura, una organización y un empeño colectivo, es prácticamente imposible, debido a los recursos y a la coordinación requerida. Para muchos puede significar partir de cero, y ello exige mucha visión, para lograr ver más allá de los agobiantes problemas que enfrentan a diario las autoridades y los pueblos de Iberoamérica.

Antes de concluir el presente trabajo, deseamos tratar un último aspecto: las limitaciones de las búsquedas automatizadas para identificar ensayos clínicos. No son pocas las bases de datos, y no es precisamente el caso de Medline, en que es imposible prácticamente recuperar automáticamente estudios con las características exigidas para los ensayos clínicos.

Sin embargo, aún en Medline, una búsqueda de esta clase de estudios requiere de una revisión manual para eliminar algunos registros que realmente no corresponden con el tipo de publicación buscado. Los resultados de los ensayos clínicos no aparecen sólo en forma de artículos originales, sino también de cartas, editoriales, revisiones tradicionales y tutoriales, entre otros, entre los que es necesario seleccionar aquellos que verdaderamente contienen resultados originales de ensayos clínicos. En ciertas ocasiones, no se encuentra disponible el resumen del trabajo, imprescindible para determinar si se cumplen con los requisitos exigidos para este tipo de informes.

Así, por ejemplo, entre los 141 ensayos clínicos publicados por autores cubanos, procesados en PubMed/Medline, sólo 66 fueron ensayos controlados aleatorios y una cifra menor, 17, ensayos controlados (27 de agosto del 2003).

Y es loable, el esfuerzo realizado por la Biblioteca Nacional de Medicina de los Estados Unidos para facilitar las búsquedas de esta clase en sus bases de datos. Pero, aún con los esfuerzos realizados, la presencia de uno o varios revisores manuales, es imprescindible para aportarle una mayor exactitud a los resultados.

Ambas vías para la búsqueda, la manual y la automatizada, son complementarias y cada una tiene su propia función. ]]> A los efectos de este estudio, se analizaron los ensayos clínicos en su totalidad, sin consideración o distinción a su carácter controlado o aleatorio como exige la Colaboración Cochrane para su registro en sus bases de datos.

Con vistas a la profundización de los resultados del presente estudio, es aconsejable la realización de un análisis informétrico sobre el flujo iberoamericano de informes de ensayos clínicos en el futuro.

13- TI - All-trans retinoic acid in acute promyelocytic leukemia: preliminary results in Cuba. ]]> AB - Seven patients with acute promyelocytic leukemia (APL) were treated with all-trans retinoic acid (ATRA). Five (71.4%) achieved complete remission (CR). Most side effects were transient and well tolerated. Hyperleukocytosis was the major adverse effect. These observations confirm the efficacy of ATRA for inducing CR in APL.
