La incidencia de esta afección es alta en pacientes que practican deportes de contacto y de no ser tratados de forma adecuada, los resultados son desfavorables. Objetivo: profundizar en los factores necesarios para el tratamiento adecuado de enfermos con esta lesión y evitar las complicaciones. Método: se realizó una revisión bibliográfica de un total de 300 artículos publicados en Pubmed, Hinari y Medline mediante el localizador de información Endnote, de ellos se utilizaron 52 citas seleccionadas para realizar la revisión, 48 de ellas de los últimos cinco años donde se incluyeron seis libros. Desarrollo: se discuten los aspectos relacionados con el diagnóstico clínico e imaginológico. Se aborda la clasificación de esta lesión en cuanto a varios aspectos como: tiempo, lesión de ligamentos u ósea, aislada o combinada y parcial o total. Se mencionan los parámetros para obtener resultados satisfactorios al considerar factores como la edad del enfermo, lesiones asociadas y tiempo de la lesión. Se relacionan los requisitos para la selección del implante y se mostró las ventajas y desventajas de cada tipo de injerto. Las complicaciones de esta cirugía están relacionadas con la técnica quirúrgica y médicas. Conclusiones: las lesiones del ligamento son entidades traumáticas que afectan por lo general a pacientes jóvenes. Para lograr un resultado satisfactorio en el tratamiento es necesario el conocimiento de su anatomía, clasificación, tipo de injerto a emplear según sus ventajas y desventajas.]]></p></abstract>

The incidence of this complaint is high in patients who practice contact sports and if it is not treated properly results can be unfavorable. Objective: to study in depth the necessary factors for the proper treatment of patients who suffer from this injury and how to avoid complications. Method: a bibliographic review of a total of 300 articles published in Pubmed, Hinari and Medline was conducted by means of the information localizer EndNote; 52 quotations of them were selected for making the review, among them 48 were from the last five years and six books were included. Development: aspects related to the imaging and clinical diagnosis, are discussed. The classification of this injury regarding some aspects like: time, ligament or osseous injury, isolated or combined, partial or total, is also discussed. The parameters to obtain satisfactory results when taking into account factors like age of the patient, associated injuries and time of the injury are mentioned. The requirements for the selection of the implant are listed and the advantages and disadvantages of each type of graft are shown. The complications of this surgery are related to the medical and surgical techniques. Conclusions: ligament injuries are traumatic complaints that generally affect young patients. To know about its anatomy, classification and type of graft to use according to its advantages and disadvantages is necessary to achieve a satisfactory result in the treatment.]]></p></abstract>

Esta incidencia es m&aacute;s elevada en pacientes que practican  alg&uacute;n deporte de contacto como: f&uacute;tbol, deportes de combate y baloncesto. <sup>5,6</sup></font></p>     ]]></body>
<body><![CDATA[<p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Las mujeres que practican alg&uacute;n tipo de actividad  deportiva tienen entre dos a ocho veces m&aacute;s posibilidades de presentar ruptura  del LCA que loshombres posiblemente relacionado con las alteraciones del &aacute;ngulo  Q, la morfolog&iacute;a de la articulaci&oacute;n, dimensiones p&eacute;lvicas, el tipo de  entrenamiento, estado hormonal, disminuci&oacute;n de la actividad protectora de los  estabilizadores din&aacute;micos de la rodilla (cu&aacute;driceps y m&uacute;sculos de la corva),  disminuci&oacute;n en las fuerzas de cizallamiento anterior por contractura muscular;  el LCA es m&aacute;s peque&ntilde;o en la mujer, al igual que la escotadura intercond&iacute;lea. <sup>7-9</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Cuando se presenta una ruptura del LCA se pueden  encontrar dos tipos de pacientes: los que toleran y los que no toleran la  ruptura del LCA. Los tolerantes son aquellos pacientes que realizan de forma  asintom&aacute;tica todas las actividades, que incluyen las deportivas que realizaban  antes de la ruptura del ligamento. Por otro lado, los no tolerantes son  aquellos que presentan s&iacute;ntomas relacionados con la inestabilidad articular y  por lo tanto necesitan de reconstrucci&oacute;n, para regresar a sus actividades de la  vida diaria. <sup>10-12</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Seg&uacute;n Cugat BR, et al, <sup>13</sup> alrededor de  un 10 a 15 % de los enfermos presentan fallo de la cirug&iacute;a de reconstrucci&oacute;n  del LCA, de all&iacute; la importancia de conocer todos los factores involucrados en  esta complicaci&oacute;n para evitarla.</font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Debido a la importancia del tema, la siguiente  investigaci&oacute;n se propone profundizar en todos aquellos factores necesarios para  el tratamiento adecuado de enfermos con lesi&oacute;n del LCA y prevenir el fallo de  la cirug&iacute;a.</font></p>     <p align="justify">&nbsp;</p>     <p><font size="3" face="Verdana, Arial, Helvetica, sans-serif"><b>M&Eacute;TODOS</b></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Se realiz&oacute; una revisi&oacute;n bibliogr&aacute;fica de un total  de 300 art&iacute;culos publicados en Pubmed, Hinari y Medline mediante el localizador  de informaci&oacute;n Endnote, de ellos se utilizaron 50 citas seleccionadas para  realizar la revisi&oacute;n, 44 de ellas de los &uacute;ltimos cinco a&ntilde;os donde se incluyeron  seis libros.</font></p>     <p align="justify">&nbsp;</p>     <p><font size="3" face="Verdana, Arial, Helvetica, sans-serif"><b>DESARROLLO</b></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Los pacientes con una lesi&oacute;n del LCA acuden a los  servicios de urgencia con dolor e impotencia funcional total de la rodilla  afectada. En el interrogatorio es importante definir el mecanismo de  producci&oacute;n, tanto en las actividades deportivas como laborales. <sup>14-16</sup>  A la inspecci&oacute;n se observ&oacute; aumento de volumen marcado de la articulaci&oacute;n,  donde, desde los primeros momentos se pudiera observar equimosis y hematomas.  La palpaci&oacute;n muestra aumento de la temperatura local caracter&iacute;stico de la  hemartrosis traum&aacute;tica. Las maniobras de estabilidad para cruzado anterior son  positivas. Es importante que pueden encontrarse otras lesiones asociadas como:  ruptura del ligamento cruzado posterior, colateral medial y lateral adem&aacute;s de  los meniscos. <sup>17-19</sup></font></p>     ]]></body>
<body><![CDATA[<p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">La IRM (Imagen de Resonancia Magn&eacute;tica) desempe&ntilde;a  un papel importante en el diagn&oacute;stico de estos enfermos y es el complemento  ideal de la exploraci&oacute;n f&iacute;sica. <sup>20,21</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Las lesiones del LCA se clasifican de acuerdo a  varios aspectos: <sup>22,23</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">-. Tiempo: se consideran lesiones agudas aquellas  por debajo de las cuatro semanas y las cr&oacute;nicas son aquellas que presenta  cuatro semanas o m&aacute;s. <sup>24</sup>    <br>   -. Lesi&oacute;n de ligamentos u &oacute;sea: son aquellas que  ocurren solo en la zona del ligamento y las otras son causadas por avulsi&oacute;n.  Esta clasificaci&oacute;n est&aacute; en desuso, ya que era aplicada en pacientes con  lesiones de avulsi&oacute;n &oacute;sea, a los que se les realizaba reparaci&oacute;n primaria, pero  los resultados no fueron satisfactorios. <sup>22</sup>    <br>   -. Lesi&oacute;n aislada o combinada: las lesiones del LCA  est&aacute;n por lo general asociadas a lesiones de menisco, ligamentos colaterales y  da&ntilde;os del cart&iacute;lago articular. <sup>22,23</sup>    <br>   -. Lesi&oacute;n parcial o total: se refiere al da&ntilde;o en el  espesor del LCA. <sup>23</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Los par&aacute;metros m&aacute;s importantes para lograr un  resultados satisfactorio en pacientes con lesi&oacute;n del LCA son: selecci&oacute;n del  enfermo, obtener un rango de movimiento y fuerza muscular adecuados en el  preoperatorio, correcta colocaci&oacute;n de los t&uacute;neles, selecci&oacute;n del injerto,  fijaci&oacute;n r&iacute;gida del injerto y permitir el movimiento precoz de la articulaci&oacute;n.  <sup>25-27</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Una gran cantidad de estudios sobre el tema han  revelado que no todos los pacientes con lesiones del LCA presentan fallo  articular o inestabilidad. El objetivo fundamental del tratamiento es evitar  que se prolonguen o presenten s&iacute;ntomas relacionados con la inestabilidad. <sup>28-30</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Es por ello que la indicaci&oacute;n de la cirug&iacute;a en  estos enfermos debe ser bien valorada especialmente en pacientes asintom&aacute;ticos,  con pocos s&iacute;ntomas y que requieren de bajas demandas f&iacute;sicas de la  articulaci&oacute;n. Para lo cual se debe tener en cuenta los siguientes elementos. <sup>31-33</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">-. Edad: es poco probable que los pacientes j&oacute;venes  modifiquen su estilo de vida, por lo tanto, en estos enfermos la cirug&iacute;a  reconstructiva es la mejor opci&oacute;n. La edad por encima de 45 a&ntilde;os no debe  constituir una contraindicaci&oacute;n quir&uacute;rgica siempre y cuando sean pacientes  activos y sin signos importantes de gonartrosis. <sup>31</sup></font></p>     ]]></body>
<body><![CDATA[<p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">    <br>   -. Lesi&oacute;n asociada: la aparici&oacute;n de una lesi&oacute;n meniscal  en un paciente que hab&iacute;a tolerado la ruptura del LCA, debe conducir a la  reparaci&oacute;n quir&uacute;rgica especialmente si es factible la reparaci&oacute;n del menisco,  ya que la extirpaci&oacute;n del cuerno posterior va a aumentar la inestabilidad de la  rodilla y favorece la aparici&oacute;n de episodios de fallo articular y por lo tanto,  de fen&oacute;menos degenerativos.<sup>31</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">    <br>   -. Tiempo de la lesi&oacute;n: aunque no existe un  consenso general, la mayor&iacute;a de los estudios justifican diferir la cirug&iacute;a m&aacute;s  all&aacute; de tres semanas, ya que disminuye la incidencia de rigidez articular. Pero  lo m&aacute;s importante no es el tiempo, sino que la inflamaci&oacute;n articular  desaparezca y que la articulaci&oacute;n recupere su rango de movimiento completo.  Seg&uacute;n Strobel MJ, et al, 34 los pacientes para ser operados deben  reunir los siguientes requisitos: la articulaci&oacute;n no debe estar irritada a&uacute;n  por el da&ntilde;o agudo del LCA, el rango de movimiento debe ser de 10 a 110 grados,  ausencia de dolor y en caso de existir aumento de la temperatura al palpar la  articulaci&oacute;n, la cirug&iacute;a debe ser diferida. <sup>32</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Una vez decidida la intervenci&oacute;n quir&uacute;rgica se  procede a la selecci&oacute;n de la t&eacute;cnica m&aacute;s adecuada para el enfermo. Existen tres  m&eacute;todos de reconstrucci&oacute;n aut&oacute;logos, los intrarticulares, extrarticulares y en  ocasiones una combinaci&oacute;n de los dos. Las t&eacute;cnicas intrarticulares act&uacute;an sobre  la tibia e intentan simular el LCA, mientras que las extrarticulares lo hacen a  cierta distancia del LCA. <sup>35-37</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Para seleccionar el injerto adecuado se deben tener  presente las siguientes condiciones (<a href="#cuadro1">cuadro 1</a>). <sup>22</sup></font></p>     <p align="center"><img src="/img/revistas/amc/v19n1/c01140115.jpg" alt="cuadro 1" width="418" height="200" longdesc="img n1 2014/c01140115.jpg"><a name="cuadro1"></a></p>     
<p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Actualmente el tercio medio central del tend&oacute;n  rotuliano aut&oacute;logo es el injerto m&aacute;s utilizado para la reparaci&oacute;n del LCA y es  el patr&oacute;n con el que debe compararse los dem&aacute;s injertos. Este injerto se conoce  m&aacute;s popularmente por las siglas HTH: Hueso-Tend&oacute;n-Hueso. <sup>38,39</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">El LCA est&aacute; conformado por dos fasc&iacute;culos el  anteromedial (AM) y el posterolateral (PL). La t&eacute;cnica de reconstrucci&oacute;n  monofascicular no reproduce de forma exacta la totalidad de las fibras del LCA.  La localizaci&oacute;n tibial intrarticular corresponde al punto de inserci&oacute;n original  del fasc&iacute;culo PL con el objetivo de evitar el temido e indeseado choque o roce  con la escotadura intercond&iacute;lea (que debe ser comprobado con la realizaci&oacute;n de  flexo-extensi&oacute;n tras la colocaci&oacute;n del injerto). El punto de inserci&oacute;n femoral  se localiza a nivel del fasc&iacute;culo AM con la finalidad de buscar la orientaci&oacute;n  m&aacute;s posterior y evitar as&iacute; mismo el citado choque intercond&iacute;leo y obtener una  posici&oacute;n anat&oacute;mica de la plastia, que proporcione menor posibilidad de elongaci&oacute;n.  <sup>40-42</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Aunque la t&eacute;cnica de doble fasc&iacute;culo logra una  reconstrucci&oacute;n m&aacute;s anat&oacute;mica, la misma no ha mostrado hasta la actualidad  superioridad sobre la reconstrucci&oacute;n monofascicular. Los autores que defienden  la t&eacute;cnica de doble fasc&iacute;culo basan sus argumentos en la alta incidencia de  pacientes con osteoartritis posoperatoria cuando se realiza la t&eacute;cnica  monofascicular. <sup>43-45</sup> Morse KR, et al, <sup>46</sup> indica la  t&eacute;cnica de doble fasc&iacute;culo en enfermos con fallo por la modalidad  monofascicular y la contraindica en caso de da&ntilde;o severo del cart&iacute;lago articular  de tipo degenerativo.</font></p>     ]]></body>
<body><![CDATA[<p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Seg&uacute;n    Murawski CD, et al, <sup>23</sup> existen ventajas y desventajas en cada tipo    de injerto empleado para la reconstrucci&oacute;n del LCA (<a href="/img/revistas/amc/v19n1/c02140115.jpg">cuadro    2</a>).</font></p>     
<p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Para    definir el fallo de la cirug&iacute;a Johnson DL y Fu H, <sup>47</sup> establecieron    el criterio que deber&iacute;a considerarse fallo cl&iacute;nico toda rodilla    intervenida por ruptura del LCA que presente inestabilidad recurrente o que,    a&uacute;n al estar estable, presente dolor y rigidez con un rango de movimiento    entre 10 a 120 grados.</font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Las complicaciones pueden ser divididas en aquellas  relacionadas directamente con el procedimiento quir&uacute;rgico y las m&eacute;dicas. <sup>48-50</sup>  Dentro de las quir&uacute;rgicas las m&aacute;s encontradas despu&eacute;s de una cirug&iacute;a del LCA  son: infecci&oacute;n, rigidez articular, fallo del injerto y osteoartritis. La  infecci&oacute;n aunque es rara es muy desbastante y causa da&ntilde;o severo del cart&iacute;lago  articular e incrementa la rigidez de la articulaci&oacute;n, una vez identificada se  debe proceder al lavado articular, desbridamiento y aplicaci&oacute;n de  antimicrobianos de alto espectro. La rigidez articular puede estar presente del  4 al 35 % de los enfermos y sus causas son multifactoriales.</font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Los factores m&aacute;s asociados a la rigidez articular  son: reconstrucci&oacute;n antes de un mes del trauma, sexo masculino y la reparaci&oacute;n  combinada del ligamento colateral medial. La osteoartritis es la complicaci&oacute;n  que m&aacute;s se reporta en la literatura y su incidencia puede ser de 50 al 70 % de  los pacientes operados. <sup>51-52</sup> Por otra parte, las complicaciones  m&eacute;dicas m&aacute;s frecuentes son la trombosis venosa profunda y el tromboembolismo  pulmonar. <sup>48</sup></font></p>     <p align="justify">&nbsp;</p>     <p align="justify"><font size="3" face="Verdana, Arial, Helvetica, sans-serif"><b>CONCLUSIONES</b></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Las lesiones del LCA son entidades traum&aacute;ticas que  afectan, por lo general, a pacientes j&oacute;venes. Para lograr un resultado  satisfactorio en el tratamiento es necesario el conocimiento de su anatom&iacute;a,  clasificaci&oacute;n y el tipo de injerto a emplear seg&uacute;n sus ventajas y desventajas.
