Las anomalías del árbol vascular son comunes. Los aneurismas aislados de la arteria aorta son de observación bastante frecuentes. Las estadísticas asociadas con esta patología son de gran preocupación y se prevé que su incidencia se incremente en los próximos años, debido al aumento en la expectativa de vida de la población. El calibre de la aorta abdominal no excede de 3 cm de diámetro y se va afilando progresivamente a medida que discurre distalmente. El riesgo de ruptura depende del tamaño del aneurisma. Se reportó un caso femenino, de 61 años de edad, que asistió a consulta por presentar dolor, acompañado de hipotensión una anemia aguda, fatigas con sudoraciones profusas, pulsaciones en epigastrio. Se le realizó un ultrasonido de abdomen, observándose ligera dilatación de la aorta abdominal de 3,7 cm que se extendía hasta las ramas iliacas. Se le indicó tomografía axial computarizada de abdomen, donde se halló afectación de la aorta abdominal.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[The aneurism of the abdominal aorta has been recognized as an important health problem in the last decade. The anomalies of the vascular tree are common. The isolated aneurisms of the aorta artery are observed quite frequently. The statistics associated to this disease provoke great preoccupation and it is foreseen that its incidence increases in the subsequent years, due to the increase of the population&#8217;s life expectancy. The caliber of the abdominal aorta does not exceed 3 cm of diameter and it gets thinner progressively at the time it wanders distally. The rupture risk depends on the aneurism size. We reported the case of a woman aged 61 years, who assisted the consultation for having pain, accompanied by hypotension, an acute anemia, fatigue with profuse perspiration and pulsations in the epigastrium. An abdominal ultrasound was made, finding a slight distension of the abdominal aorta of 3,7 cm, extended up to the iliac branches. We indicated an abdominal computed axial tomography discovering the damage of the abdominal aorta.]]></p></abstract>

Las estad&iacute;sticas asociadas con esta patolog&iacute;a  son de gran preocupaci&oacute;n y se prev&eacute; que su incidencia se incremente  en los pr&oacute;ximos a&ntilde;os, debido al aumento en la expectativa de vida  de la poblaci&oacute;n. <body><![CDATA[<p>&nbsp;</p>    <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">El  aneurisma a&oacute;rtico son dilataciones circunscritas de la arteria. La tomograf&iacute;a  axial computarizada (TAC) proporciona una valoraci&oacute;n m&aacute;s global  de los aneurismas abdominales que la ecograf&iacute;a.<sup>(1-3)</sup> Se pueden  definir los diferentes tipos de aneurismas en aneurisma verdadero y pseudoaneurisma.  El verdadero, afecta las tres capas de la pared arterial; producen dilataciones  fusiformes, saculares o esf&eacute;ricas de la aorta; el inflamatorio est&aacute;  rodeado de un tejido inflamatorio; la aorta est&aacute; solo m&iacute;nimamente  dilatada, aunque presenta una pared engrosada; en el disecante la disecci&oacute;n  de la media con sangre a trav&eacute;s de una fisura en la intima origina un segmento  arterial dilatado con dos luces (demostrando el flap de la intima separando las  luces verdaderas y la falsa, la disecci&oacute;n puede extenderse hasta el origen  de la arteria renal.<sup>(4-7)</sup> El pseudoaneurisma o aneurisma falso, por  su parte, es una perforaci&oacute;n de la arteria; se observa la dilataci&oacute;n  de la aorta que est&aacute; tapizado por un trombo mural cuando se administra  contraste se observa un realce de la luz, esto puede ser causado por traumatismo,  cirug&iacute;a, cateterizaciones o destrucci&oacute;n de la pared arterial por  infecci&oacute;n mic&oacute;tica.<sup>(8-10)</sup></font></p>    <p align="justify">&nbsp;</p>    <p align="justify"><font size="3" face="Verdana, Arial, Helvetica, sans-serif"><strong>PRESENTACI&Oacute;N  DEL CASO</strong></font></p>    <p align="justify">&nbsp;</p>    <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">  Paciente femenina de 61 a&ntilde;os de edad, con antecedentes familiares de dos  hermanos fallecidos por rotura de aneurisma disecarte de la aorta abdominal. Asisti&oacute;  a consulta por presentar dolor puls&aacute;til en epigastrio, acompa&ntilde;ado  de hipotensi&oacute;n, fatigas con sudoraciones profusas.</font></p>    <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif"><strong>Ex&aacute;menes  complementarios</strong></font></p>    <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">  Hemoglobina: 9 gramos por litro.    <br> Rayos x de trax: &Aacute;rea cardiaca de  tama&ntilde;o normal, bot&oacute;n a&oacute;rtico prominente. No alteraciones  pleuro-pulmonares.    <br> Ultrasonido de abdomen: se observa que la aorta abdominal  descendente med&iacute;a 3,7 cm, con gran eretismo de la misma, ateromatosis difusa.</font></p>    ]]></body>
<body><![CDATA[<p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">  TAC de abdomen: se utiliz&oacute; la TAC simple y la contrastada, es la segunda  &uacute;til en el diagn&oacute;stico de la disecci&oacute;n a&oacute;rtica con  buena definici&oacute;n de las im&aacute;genes, con contraste endovenoso, donde  se confirm&oacute; la presencia de una ligera dilataci&oacute;n de la aorta descendente,  que mide 37,21 mm, con aneurisma disecante en toda la extensi&oacute;n de la aorta,  con calcificaci&oacute;n de la pared (<a href="#figura1">fig. 1</a>), aneurisma  inflamatorio con peque&ntilde;o trombo en la bifurcaci&oacute;n de la aorta, la  disecci&oacute;n se extiende hasta el origen de la arteria renal izquierda (<a href="#figura2">fig.  2</a>), adem&aacute;s de la dilataci&oacute;n ligera de ambas il&iacute;acas,  m&aacute;s marcado de la il&iacute;aca derecha, que mide 22,75 mm y la il&iacute;aca  izquierda mide 18,84 mm, con calcificaciones de sus paredes (<a href="#figura3">fig.  3</a>).</font></p>    <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">  A los 6 meses se le realiza un chequeo evolutivo a la paciente, debido al empeoramiento  de los s&iacute;ntomas cl&iacute;nicos.    <br> Rayos x de t&oacute;rax: se observ&oacute;  una gran aneurisma disecante de la aorta tor&aacute;xica, que produce ensanchamiento  del mediastino superior y medio de densidad vascular, comparando con estudio anterior.</font></p>    <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">A  los pocos d&iacute;as la paciente fallece.</font></p>    <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">  Anatom&iacute;a Patol&oacute;gica informa: gran aneurisma disecante roto en la  aorta tor&aacute;xica. <body><![CDATA[<p align="left"><font size="3" face="Verdana, Arial, Helvetica, sans-serif"><strong>DISCUSI&Oacute;N</strong></font></p>    <p align="left">&nbsp;</p>    <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">La  ectasia o los aneurismas de la aorta son el resultado de la enfermedad ateroscler&oacute;tica  que conduce a trombosis mural.<sup>(10,11)</sup> Cuando el di&aacute;metro de  la aorta abdominal es mayor de 3 a 4 cm, se considera dilataci&oacute;n aneurism&aacute;tica.<sup>(12)</sup>  El peligro de rotura de un aneurisma es mayor si la luz es exc&eacute;ntrica o  si la forma de la secci&oacute;n transversal del vaso es muy irregular. Una dilataci&oacute;n  por encima de 6 cm de di&aacute;metro posee un alto riesgo de rotura.<sup>(13,14)</sup>  Los aneurismas pueden localizarse en cualquiera de las arterias del organismo,  son m&aacute;s frecuentes en la aorta, troncos cervicales, arteria mesent&eacute;rica  superior, arterias renales y de las extremidades. Los aneurismas cong&eacute;nitos  se localizan con m&aacute;s frecuencia en las arterias cerebrales, aparecen como  defecto, ausencia o escaso desarrollo cong&eacute;nito de la musculatura de la  pared.<sup>(1,5,8,11)</sup></font></p>    <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">En  la TAC se observa una dilataci&oacute;n de la aorta fusiforme y conc&eacute;ntrica  en los fen&oacute;menos degenerativos y focales, sacular y exc&eacute;ntrica en  los inflamatorios. Se debe evaluar su di&aacute;metro y extensi&oacute;n, su relaci&oacute;n  con otras ramas que nacen de la aorta, los trombos intramurales (su extensi&oacute;n  y localizaci&oacute;n) y los cambios inflamatorios que lo pueden acompa&ntilde;ar  con rarefacci&oacute;n de la grasa.
