<body><![CDATA[<p><font size="2" face="Verdana">Universidade Federal da Para&#237;ba, Brasil.</font></p>     <p>&nbsp;</p>     <p>&nbsp;</p> <hr>     <p><font size="2" face="Verdana"><b>RESUMO</b> </font></p>     <p><font size="2" face="Verdana"><b>Objetivo:</b> analisar a densidade dos mast&#243;citos    em Queilite Act&#237;nica segundo as caracter&#237;sticas histol&#243;gicas    da les&#227;o comparando com a mucosa normal.     <br>   <b>M&#233;todo:</b> estudo descritivo onde    a amostra foi composta por dois grupos, sendo o primeiro com 33 esp&#233;cimes    de casos de queilite act&#237;nica diagnosticados clinicamente, e o segundo    composto por 9 esp&#233;cimes de mucosa oral normal, registrados no Servi&#231;o    de Cirurgia de Cabe&#231;a e Pesco&#231;o do Hospital Dr. Napole&#227;o Laureano,    Jo&#227;o Pessoa, PB. Os blocos parafinados da amostra foram cortados e corados    em Hematoxilina e Eosina para avaliar o grau de displasia epitelial e infiltrado    inflamat&#243;rio; e por azul de toluidina para quantificar os mast&#243;citos.    A contagem dos mast&#243;citos foi realizada com o auxilio de um ret&#237;culo    quadrado em 8 campos por caso. A leitura final foi expressa com o valor m&#233;dio    de mast&#243;citos por caso em c&#233;lulas/&#956;m&#178;. Os dados foram analisados    pelo SPSS (<i>Statistical Package for Social Sciences</i>) vers&#227;o 15.0.    <br>   <b>Resultados:</b> em 57,6 % dos casos    houve algum grau de displasia epitelial, sendo 39,4 % leve, 15,2 % moderada    e 3 % severa. Em 21,2 % havia carcinoma de c&#233;lulas escamosas. A presen&#231;a    de infiltrado inflamat&#243;rio e elastose solar foi observada em 84,9 % e 81,8    % dos casos, respectivamente. Os mast&#243;citos foram identificados em 87,8    % da amostra. A densidade dos mast&#243;citos nos casos de queilite act&#237;nica    foi de 17,4&#177;10,4 c&#233;lulas/&#956;m&#178; e no tecido normal 1,78&#177;1,64    c&#233;lulas/&#956;m&#178; (p&lt;0,001). <body><![CDATA[<p> <font size="2" face="Verdana"><b>Objective:</b> analyze the density of mast    cells in Actinic cheilitis according to the histological characteristics of    the lesion compared with the normal mucosa.    <br>   <b>Methods: </b>descriptive study where the sample consisted of two groups,    the first with 33 specimens of cases of actinic cheilitis diagnosed clinically,    and the second consists of 9 specimens of normal oral mucosa, registered in    the Service of Head and Neck Hospital Dr. Napoleon Laureano, Jo&#227;o Pessoa,    PB. The paraffin blocks of the sample were cut and stained with hematoxylin    and eosin to assess the degree of dysplasia and inflammatory infiltrate, and    toluidine blue to quantify mast cells. The count of mast cells was performed    with the aid of a square grid in the case of field 8. The final reading was    expressed with an average value of mast cells by case / &#956;m&#178;. Data    were analyzed by SPSS (Statistical Package for Social Sciences) version 15.0.    <br>   <b>Results:</b> in 57.6 % of cases there was some degree of dysplasia, and 39.4    % mild, 15.2 % moderate and 3 % severe. 21.2 % had squamous cell carcinoma.    The presence of inflammatory infiltrate and solar elastosis was observed in    84.9 % and 81.8 %, respectively. Mast cells were identified in 87.8 % of the    sample. The density of mast cells in cases of Actinic Cheilitis was 17.4 &#177;    10.4 cells / &#956;m&#178; and 1.78 &#177; 1.64 normal tissue cells / &#956;m&#178;    (p &lt;0.001). There was a statistically significant correlation between mast    cell density with the processes of dysplasia (p = 0.004) and inflammatory cell    infiltration (p = 0.000).     <br>   <b>Conclusion:</b> the increase in mast cell density in actinic cheilitis lesions    and its correlation with the processes of inflammation and dysplasia suggest    involvement of these cells in disease progression for Squamous Cell Carcinoma    lip. </font></p>     <p> <font size="2" face="Verdana"><b>Keywords:</b> Mast cells, actinic cheilitis,    squamous cell carcinoma, carcinogenesis. </font></p> <hr>     <p><font face="Verdana" size="2"><b>RESUMEN</b></font></p>     <p><font face="Verdana" size="2"><b>Objetivo</b>: analizar la densidad de los    mastocitos en la queilitis act&iacute;nica seg&uacute;n las caracter&iacute;sticas    histol&oacute;gicas de la lesi&oacute;n en comparaci&oacute;n con la mucosa    normal.    <br>   <b>M&eacute;todos</b>: estudio descriptivo donde la muestra consisti&oacute;    en dos grupos, el primero con 33 muestras de casos de queilitis act&iacute;nica    diagnosticados cl&iacute;nicamente, y el segundo se compone de 9 muestras de    mucosa oral normal. <body><![CDATA[<p><font face="Verdana" size="2"><b>Palabras clave</b> :Mastocitos, queilitis    act&#237;nica, carcinoma de c&#233;lulas escamosas, carcinog&eacute;nesis. </font></p> <hr>     <p>&nbsp;</p>    <p>&nbsp;</p>     <p> <font size="2" face="Verdana"><b><font size="3">INTRODU&#199;&#195;O</font></b>    </font></p>     <p><font size="2" face="Verdana"> A queilite act&#237;nica (QA) &#233; uma les&#227;o    potencialmente maligna que pode transformar-se em carcinoma de c&#233;lulas    escamosas (CCE) de l&#225;bio. Afeta principalmente o l&#225;bio inferior de    pessoas de pele clara, na faixa et&#225;ria de 50 a 60 anos de idade, que se    exp&#245;em excessivamente &#224; radia&#231;&#227;o solar.<sup>1-5</sup> </font></p>     <p><font size="2" face="Verdana"> Histologicamente a QA revela com freq&#252;&#234;ncia    v&#225;rias altera&#231;&#245;es epiteliais, sendo comum a ocorr&#234;ncia de    displasia leve &#224; severa <sup>6</sup> e Carcinoma <i>in situ </i><sup>7</sup>;    e, no tecido conjuntivo, freq&#252;entemente se observa elastose solar e infiltrado    inflamat&#243;rio de grau variado.<sup>3-4,8</sup> </font></p>     <p><font size="2" face="Verdana"> Alguns autores afirmam que o infiltrado inflamat&#243;rio    tumor-associado contribui para a carcinog&#234;nese escamosa.<sup>9-10</sup>    Outros acreditam que os mecanismos que resultam na maliginiza&#231;&#227;o da    queilite act&#237;nica possam ter a participa&#231;&#227;o de componentes do    processo inflamat&#243;rio, a exemplo dos mast&#243;citos, que s&#227;o c&#233;lulas    que proliferam ap&#243;s indu&#231;&#227;o pela radia&#231;&#227;o ultravioleta    (UV) e que liberam potentes mediadores que atuam na inflama&#231;&#227;o, angiog&#234;nese    e degrada&#231;&#227;o da matriz intercelular. <sup>2,11-14 </sup>Os mast&#243;citos    est&#227;o presentes em maior quantidade em les&#245;es provocadas pela radia&#231;&#227;o    UV, como a queilite act&#237;nica e o carcinoma de c&#233;lulas escamosas de    l&#225;bio. Alguns estudos associam a maior quantidade de mast&#243;citos ao    desenvolvimento da queilite act&#237;nica.<sup>2,11</sup> </font></p>     <p><font size="2" face="Verdana"> Neste contexto, o presente estudo teve como    objetivo analisar a densidade dos mast&#243;citos em Queilite Act&#237;nica    segundo as caracter&#237;sticas histol&#243;gicas da les&#227;o comparando com    a mucosa normal. </font></p>     <p>&nbsp;</p>     <p> <font size="2" face="Verdana"><b><font size="3">M&#201;TODO</font></b> </font></p>     ]]></body>
<body><![CDATA[<p><font size="2" face="Verdana">O estudo foi aprovado pelo Comit&#234; de &#201;tica    em Pesquisa do Hospital Lauro Wanderley. CEP/HULW/UFPB sob protocolo n&#250;mero    448/10. </font></p>     <p><font size="2" face="Verdana"> Foi um estudo descritivo onde um grupo foi composto    por 33 blocos parafinados de casos com diagn&#243;stico cl&#237;nico de queilite    act&#237;nica em indiv&#237;duos de ambos os sexos, registrados no Servi&#231;o    de Cirurgia de Cabe&#231;a e Pesco&#231;o do Hospital Dr. Napole&#227;o Laureano,    Jo&#227;o Pessoa, PB, obtidos no per&#237;odo entre os anos de 2000 a 2008.    O segundo grupo foi composto por 9 blocos parafinados de esp&#233;cimes da mucosa    oral normal. Para serem inclu&#237;dos na amostra, os blocos parafinados deveriam    ter condi&#231;&#245;es para novo corte e colora&#231;&#227;o para an&#225;lise    histol&#243;gica. </font></p>     <p><font size="2" face="Verdana"> Cada bloco selecionado foi recortado em sec&#231;&#245;es    de 4 &#181;m de espessura para a obten&#231;&#227;o de duas l&#226;minas, uma    por colora&#231;&#227;o por t&#233;cnica de rotina de Hematoxilina e Eosina(HE)    e uma outra l&#226;mina para ser colada pela t&#233;cnica de Azul de Toluidina    (AT), para identifica&#231;&#227;o de mast&#243;citos.<sup>11</sup> </font></p>     <p><font size="2" face="Verdana"> A avalia&#231;&#227;o histol&#243;gica foi realizada    por dois examinadores independentes, previamente calibrados, em um microsc&#243;pio    &#243;ptico, onde foi realizada an&#225;lise microsc&#243;pica das caracter&#237;sticas    histol&#243;gicas epiteliais de acordo com os crit&#233;rios estabelecidos pela    Organiza&#231;&#227;o Mundial da Sa&#250;de (OMS). A displasia epitelial foi    classificada de acordo com OMS em displasia leve, displasia moderada e displasia    severa <sup>6</sup>. A presen&#231;a de elastose solar e de infiltrado inflamat&#243;rio    tamb&#233;m foi registrada, sendo o infiltrado classificado como leve, moderado    ou intenso e na sele&#231;&#227;o das &#225;reas para an&#225;lise foram desconsideradas    regi&#245;es de ulcera&#231;&#227;o.<sup>15</sup> </font></p>     <p><font size="2" face="Verdana"> Para contagem dos mast&#243;citos, foi utilizado    um microsc&#243;pio &#243;ptico Motic BA 300&#174;, Canad&#225;, com magnitude    400X, acoplado ao computador com programa Motic Images Plus 2.0. A leitura foi    realizada em 8 campos por l&#226;mina, usando um ret&#237;culo quadrado (20x20),    onde cada lado do campo tinha 12&#956;m, somando uma &#225;rea de 144 m&#178;    por campo; e a soma total dos oitos campos igual a 1052 m&#178;. A m&#233;dia    de mast&#243;citos foi obtida pela leitura dos 08 campos e expressa em c&#233;lulas/m&#178;.    </font></p>     <p><font size="2" face="Verdana"> Os dados foram tabulados e realizados testes    estat&#237;sticos em programa estat&#237;stico SPSS (<i>Statistical Package    for Social Sciences</i>) para Windows vers&#227;o 15.0. Os procedimentos de    infer&#234;ncia estat&#237;stica foram realizados por testes U de <i>Mann-Whitney,    Kuskall-Wallis,</i> Qui-Quadrado e o coeficiente de Spearman, sendo consideradas    estatisticamente significantes diferen&#231;as quando p&lt;0,05. </font></p>     <p>&nbsp;</p>     <p> <font size="2" face="Verdana"><b><font size="3">RESULTADOS</font></b> </font></p>     <p><font size="2" face="Verdana">Dentre os 33 esp&#233;cimes do grupo com Q.A    foi observado que 78,8 % apresentam altera&#231;&#227;o epitelial variando de    displasia leve a CCE. Em 57,6 %(19) houve algum grau de displasia, sendo 39,4    % (13) leve, 15,2 % (5) moderada e 3 % (1) severa; e em 21,2 % (7) dos casos    ocorreu CCE. A presen&#231;a de infiltrado inflamat&#243;rio foi observada em    84,9 % da amostra, variando de 39,4 % (13) leve, 15,2 % (5) moderado e 30,3    % (10) intenso. Elastose solar foi observada em 81,8 % dos casos. </font></p>     <p><font size="2" face="Verdana"> Os mast&#243;citos foram identificados em 87,8    % (29) dos casos (<a href="img/revistas/est/v51n4/f0103414.jpg">Figuras 1A</a>, <a href="img/revistas/est/v51n4/f0103414.jpg">1B</a>,    <a href="img/revistas/est/v51n4/f0103414.jpg">2A</a> e <a href="img/revistas/est/v51n4/f0103414.jpg">2B</a>) e estavam localizados    pr&#243;ximos a &#225;rea de epit&#233;lio displ&#225;sico (30,3 %), na l&#226;mina    pr&#243;pria (24,2 %), permeando a elastose solar (15,5 %), na proximidade de    vasos (9 %), na regi&#227;o peritumoral (18,8 %) e submucosa (6 %). Em 15,2    %(5) dos casos, os mast&#243;citos estavam ativos, em est&#225;gio de degranula&#231;&#227;o,    especialmente quando pr&#243;ximos &#224; &#225;rea displ&#225;sica, regi&#227;o    tumoral e de vasos. </font></p>     ]]></body>
<body><![CDATA[<p><font size="2" face="Verdana"> A m&#233;dia de mast&#243;citos nos esp&#233;cimes    de QA foi de 17,4&#177;10,4 c&#233;lulas/m&#178; com varia&#231;&#227;o de 5,9    a 33 c&#233;lulas/m&#178;, enquanto que nos esp&#233;cimes de mucosa normal    foi de 1,78&#177;1,64 c&#233;lulas/m&#178;, com varia&#231;&#227;o de 0,52 a    3,04 c&#233;lulas/m&#178;. Houve diferen&#231;a estatisticamente significante    (U de Mann-Whitney, p&lt;0,001) quando comparada a concentra&#231;&#227;o de    mast&#243;citos entre os dois grupos. A m&#233;dia de mast&#243;citos foi de    27,57&#177;5,94 c&#233;lulas/m&#178; nos casos de CCE, sendo significativamente    maior (Kruskall-Wallis, p=0,001), quando comprados tecido normal e graus de    displasia epitelial (<a href="img/revistas/est/v51n4/f0303414.gif">Fig. 3</a>). </font></p>     <p><font size="2" face="Verdana"> Houve uma correla&#231;&#227;o estatisticamente    significante (coeficiente de Spearman, p=0,004) entre a densidade dos mast&#243;citos    e a presen&#231;a de displasia epitelial. Sendo observado um aumento progressivo    significativo (Kruskall-Wallis, p=0,001) na quantidade de mast&#243;citos, quando    havia aumento da severidade dos graus de displasia. </font></p>     <p><font size="2" face="Verdana"> O infiltrado inflamat&#243;rio apresentou uma    correla&#231;&#227;o estatisticamente significante com a densidade dos mast&#243;citos    (Coeficiente de Spearman, p=0,000) e a quantidade de mast&#243;citos aumentou    em fun&#231;&#227;o dos graus de inflama&#231;&#227;o (Kruskall-Wallis, p=0,003)    (<a href="img/revistas/est/v51n4/t0103414.gif">Tabela</a>). </font></p>     <p><font size="2" face="Verdana">Tamb&#233;m foi observada correla&#231;&#227;o    estatisticamente significante entre displasia epitelial e infiltrado inflamat&#243;rio    (Coeficiente de Spearman, p=0,002). No entanto, ao verificar se os graus de    displasia estavam associados aos graus de inflama&#231;&#227;o, n&#227;o foi    observada associa&#231;&#227;o significativa (Qui-Quadrado, p=0,398). </font></p>     <p><font size="2" face="Verdana"> Ao analisar a densidade dos mast&#243;citos    com a presen&#231;a/aus&#234;ncia de elastose solar foi observada que n&#227;o    houve diferen&#231;a estatisticamente significante (U de Mann-Whitney, p=0,05).    </font></p>     <p>&nbsp;</p>     <p> <font size="2" face="Verdana"><b><font size="3">DISCUSS&#195;O </font></b>    </font></p>     <p><font size="2" face="Verdana"> Os mast&#243;citos s&#227;o c&#233;lulas inflamat&#243;rias    que t&#234;m sido reconhecidas como importantes efetoras dos efeitos delet&#233;rios    da radia&#231;&#227;o solar sobre a pele <sup>16-17</sup>. Recentes estudos    mostram que essas c&#233;lulas est&#227;o significativamente aumentadas em les&#245;es    de l&#225;bio induzidas pela radia&#231;&#227;o como a queilite act&#237;nica    <sup>2,18-19</sup> e carcinoma de c&#233;lulas escamosas de l&#225;bio<sup>11,15,20-25</sup>.    No presente estudo foi observado aumento do n&#250;mero dos mast&#243;citos    em les&#245;es de Q.A quando comparada ao tecido normal intrabucal, corroborando    com os resultados de outros estudos.<sup>11,15,18</sup> </font></p>     <p><font size="2" face="Verdana"> Por outro lado, no presente estudo, foi identificada    uma associa&#231;&#227;o significante entre densidade de mast&#243;citos e os    diferentes graus de displasia, sendo esses dados semelhantes aos de outras pesquisas.<sup>11,26</sup>    Nestes estudos, os autores observaram estreita correla&#231;&#227;o entre a    ativa&#231;&#227;o dos mast&#243;citos e as diferentes fases de hiperceratose,    displasia, carcinoma in situ e carcinoma oral invasivo. Para estes autores durante    o processo de carcinog&#234;nese h&#225; uma infiltra&#231;&#227;o seq&#252;encial    e degranula&#231;&#227;o de c&#233;lulas mast&#243;citos em CCE. Vale destacar    que as altera&#231;&#245;es no citoesqueleto epitelial em les&#245;es potencialmente    malignas podem anteceder as altera&#231;&#245;es estruturais, como provavelmente    observado por Santos, Sousa, Nunes, Sotto, Ara&#250;jo (2003)<sup>27</sup> que    identificaram altera&#231;&#245;es no padr&#227;o de diferencia&#231;&#227;o    de queratina em QA. Supostamente estas altera&#231;&#245;es dependem ent&#227;o    no est&#225;gio em que se encontra o processo de transforma&#231;&#227;o, bem    como da continuidade da a&#231;&#227;o do agente etiol&#243;gico sobre o tecido.    </font></p>     <p><font size="2" face="Verdana"> A rela&#231;&#227;o entre a densidade de mast&#243;citos    e os diferentes graus de displasia epitelial n&#227;o s&#227;o achados frequentemente    observados divergindo dos nossos resultados.<sup>15,18-19</sup> </font></p>     ]]></body>
<body><![CDATA[<p><font size="2" face="Verdana"> A localiza&#231;&#227;o dos mast&#243;citos    esteve relacionada a regi&#227;o peritumoral,ao redor de vasos, pr&#243;ximos    a &#225;rea de inflama&#231;&#227;o, elastose solar e displasia epitelial. Estes    achados foram semelhantes aos resultados de outros autores<sup>2,11,18-20,22-23</sup>    refor&#231;ando a hip&#243;tese de que os mast&#243;citos poderiam participar    do processo de transforma&#231;&#227;o maligna da queilite act&#237;nica. </font></p>     <p><font size="2" face="Verdana"> Assim como os resultados de Rojas, Spencer,    Mart&#237;nez, Maurelia, Rudolph (2005)<sup>20</sup>, Rojas (2009)<sup>28</sup>,    Grimbaldeston, Finlay-Jones, Hart (2006)<sup>29</sup> a densidade dos mast&#243;citos    esteve correlacionada com a presen&#231;a local da inflama&#231;&#227;o. H&#225;    ind&#237;cios que a resposta inflamat&#243;ria, naquela regi&#227;o tecidual,    poder&#225; estar contribuindo com a progress&#227;o do processo benigno para    maligno.<sup>14</sup> </font></p>     <p><font size="2" face="Verdana"> A migra&#231;&#227;o dos mast&#243;citos para    o s&#237;tio tumoral e a sua ativa&#231;&#227;o pode ser a condi&#231;&#227;o    necess&#225;ria para seu efeito promotor sobre o tumor. Visto que a falha neste    processo pode ser a respons&#225;vel pela diminui&#231;&#227;o dessas c&#233;lulas    em les&#245;es com potencial de maligniza&#231;&#227;o e em les&#245;es malignas    da cavidade oral.<sup>30</sup> </font></p>     <p><font size="2" face="Verdana"> A presen&#231;a consistente de mast&#243;citos    nas les&#245;es de QA de nossa amostra podem ter sido mediada pelo mecanisno    explicado por Huang y otros(2008).<sup>31</sup> Neste processo, em presen&#231;a    de inflama&#231;&#227;o, a libera&#231;&#227;o do mediador qu&#237;mico SCF    (Fator tumoral derivado de c&#233;lulas tronco) &#233; respons&#225;vel pela    ativa&#231;&#227;o do receptor c-kit em mast&#243;citos resultando em diferencia&#231;&#227;o,    migra&#231;&#227;o, matura&#231;&#227;o e sobreviv&#234;ncia destas c&#233;lulas    no microambiente tumoral. </font></p>     <p><font size="2" face="Verdana"> O aumento da densidade dos mast&#243;citos nas    les&#245;es de QA, em rela&#231;&#227;o &#224; mucosa normal, e sua correla&#231;&#227;o    com o processo de displasia e de inflama&#231;&#227;o sugerem sua participa&#231;&#227;o    na progress&#227;o da doen&#231;a para CCE de l&#225;bio. </font></p>     <p>&nbsp; </p>     <p><font size="2" face="Verdana"><b><font size="3">REFER&#202;NCIAS BIBLIOGR&#193;FICAS</font></b>    </font></p>     <!-- ref --><p><font size="2" face="Verdana"> 1 .Souza Lucena EE, Costa DC, Silveira EJ, Lima    KC.
