Se hace una evaluación cuantitativa en cada paciente según las alteraciones que conllevaron a la disfunción de órganos y se obtiene de esta forma una puntuación o puntaje individual; se aplica a la puntuación de todos los pacientes la medida de tendencia central o mediana que corresponde a 11 puntos. Resultados: el 85 % de los pacientes fallecidos obtuvo puntuación mayor de 11 (p < 0,01), la mortalidad fue mayor entre los enfermos con 5 ó más órganos afectados (p < 0,01) y en este grupo el 90 % se encontraba por encima de la mediana de los puntos (p < 0,01). La mortalidad alcanza el 92,8 % en los pacientes que evolucionan hasta 3 días en fallo y se encuentran con más de 11 puntos (p < 0,01).]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[A prospective study of 39 patients admitted at the Intensive Care Unit of the "Angel Arturo Aballí" Pediatric Teaching Hospital with the diagnosis of multiple organ dysfunction syndrome due to sepsis, was conducted during 16 months. A quantitative evaluation of every patients is made according to the alterations leading to organ dysfunction and thus tan individual scoring is obtained. The measure of central or mean trend corresponding to 11 points is applied to all patients&acute;scoring. Results: 85 % of the dead patients obtained a scoring over 11 (p<0.01), mortality was higher among patients with 5 or more organs effects (p<0.01), and this group 90 % were above the mean score (p<0.01). Mortality amounts to 92.8 % in those patients with failure that are able to evolve up to 3 days and have more than 11 points (p<0.01).]]></p></abstract>

La  mortalidad alcanza el 92,8 % en los pacientes que evolucionan hasta 3 d&iacute;as  en fallo y se encuentran con m&aacute;s de 11 puntos (p &lt; 0,01).        <P>Descriptores DeCS: INSUFICIENCIA DE MULTIPLOS ORGANOS/mortalidad; INSUFICIENCIA  DE MULTIPLOS ORGANOS/etiolog&iacute;a; UNIDADES DE CUIDADO INTENSIVO PEDIATRICO;  SEPSIS/complicaciones.        <P>La respuesta inflamatoria sist&eacute;mica del hu&eacute;sped frente  a una infecci&oacute;n (descrita como un estado de sepsis) representa un  complejo proceso fisiopatol&oacute;gico con la liberaci&oacute;n de m&uacute;ltiples  mediadores proinflamatorios capaces de no ser controlados, de provocar  una inadecuada perfusi&oacute;n h&iacute;stica y llevar al paciente a un  s&iacute;ndrome de disfunci&oacute;n m&uacute;ltiple de &oacute;rganos  (SDMO)<I>.</I><SUP>1-3</SUP>        <P>La sepsis es una causa importante de morbilidad y mortalidad en pacientes  ingresados en unidades de cuidados intensivos, por ella fallecen entre  el 10 al 20 % de las personas que la padecen e incluso se describen valores  mayores<I> </I>cuando evoluciona hasta un shock s&eacute;ptico o una disfunci&oacute;n  m&uacute;ltiple de &oacute;rganos (DMO).<SUP>4-8</SUP>        <P>Actualmente se realizan m&uacute;ltiples investigaciones que eval&uacute;an  el grado de disfunci&oacute;n en los diferentes &oacute;rganos o sistemas,  as&iacute; como muchos autores encaminan sus trabajos en definir las mejores  variables que representan el fallo en cada &oacute;rgano. Estos estudios  sirven como &iacute;ndice pron&oacute;stico de mortalidad y representan  un arma valiosa en el manejo de los pacientes con esta afecci&oacute;n.<SUP>9-12</SUP>        <P>Teniendo en cuenta estos conocimientos nos vimos motivados a realizar  un trabajo en nuestra Unidad de Cuidados Intensivos, donde sean evaluados  en forma cuantitativa las alteraciones capaces de definir el s&iacute;ndrome  encontrado en nuestros pacientes.  <H4>  M&Eacute;TODOS</H4>  Se realiza un estudio prospectivo en 39 pacientes ingresados en la Unidad  de Cuidados Intensivos del Hospital Pedi&aacute;trico Docente "&Aacute;ngel  Arturo Aball&iacute;" en un per&iacute;odo de 18 meses, con el diagn&oacute;stico  de DMO por sepsis.        <P>Se establecen como criterios para el diagn&oacute;stico de la DMO la  presencia de m&aacute;s de 2 fracasos simult&aacute;neos de &oacute;rganos  o sistemas, en un enfermo con respuesta inflamatoria sist&eacute;mica por  infecci&oacute;n o sepsis. Se evaluaron los siguientes sistemas: respiratorio,  cardiovascular, digestivo, renal, hemolinfopoy&eacute;tico, nervioso y  metab&oacute;lico.        <P>Las variables designadas que definen la disfunci&oacute;n en cada &oacute;rgano  fueron tomadas de la experiencia de trabajos anteriores<SUP>13,14</SUP>  y se muestran en el anexo.        <P>A todos los casos evaluados se les aplic&oacute; un formulario que recoge  el puntaje diario obtenido por cada paciente, seg&uacute;n las alteraciones  presentadas por ellos (anexo).        <P>Para determinar el puntaje se consideraron en cada ni&ntilde;o las alteraciones  que conllevaron a la disfunci&oacute;n de los sistemas u &oacute;rganos,  y se les dio una evaluaci&oacute;n entre 1 y 3 puntos en dependencia de  la gravedad de &eacute;stas. Se sumaron los puntos alcanzados diariamente  por nuestros pacientes, y se escogi&oacute; el d&iacute;a que mayor puntuaci&oacute;n  acumul&oacute; cada uno para agruparlos. A dichas puntuaciones se les aplic&oacute;  la medida de tendencia central o mediada; &eacute;sta correspondi&oacute;  con el n&uacute;mero 11, lo que nos posibilit&oacute; tomar como referencia  ese n&uacute;mero para realizar la comparaci&oacute;n entre las diferentes  variables empleadas, dentro de ellas: puntaje, tiempo de evoluci&oacute;n  cl&iacute;nica (en d&iacute;as), n&uacute;mero de &oacute;rganos en fallo,  y mortalidad.        ]]></body>
<body><![CDATA[<P>En el an&aacute;lisis de los resultados se aplic&oacute; la prueba de  chicuadrado con correcci&oacute;n de continuidad y se acept&oacute; un  error del 5 %, con el objetivo de determinar la existencia de posible asociaci&oacute;n  entre variables.  <H4>  RESULTADOS</H4>  En la tabla 1 podemos ver la relaci&oacute;n entre el puntaje medio que  obtuvieron los pacientes y la mortalidad. De un total de 21 pacientes que  se encuentran por encima de 11 puntos el 85 % fallece, mientras que por  debajo de dicha puntuaci&oacute;n s&oacute;lo fallece el 17 % (p&lt; 0,01).  <DIR>      <CENTER>TABLA 1<I>. <body><![CDATA[<p>&nbsp;</p> </CENTER>    <H4>  DISCUSI&Oacute;N</H4>  La severidad de las alteraciones (expresadas por el puntaje) y la mortalidad  son proporcionales, por lo que pensamos que una determinaci&oacute;n de  la calidad de las alteraciones en cada &oacute;rgano o sistema evaluada  cuantitativamente es &uacute;til desde el punto de vista cl&iacute;nico,  para orientarnos en el paciente de mayor riesgo hacia una evoluci&oacute;n  fatal.        <P>Otros estudios realizados sobre evaluaci&oacute;n cuantitativa en pacientes  con<I> </I>DMO<I> </I>demuestran que existe una fuerte correlaci&oacute;n  entre el incremento de la funci&oacute;n de &oacute;rgano da&ntilde;ada  y la probabilidad de muerte.<SUP>9-12</SUP> Por ejemplo en la investigaci&oacute;n  llevada a cabo por Marshall,<SUP>10</SUP> se observa que la mortalidad  asciende en modo linear con el aumento de los valores num&eacute;ricos  que reciben sus pacientes; con una puntuaci&oacute;n entre 9 y 12 fallece  el 25 % y cuando en el score se alcanza entre 17 y 20 puntos la mortalidad  asciende al 75 %.        <P>En nuestro estudio el n&uacute;mero de &oacute;rganos en fallo y mortalidad  tienen una relaci&oacute;n directamente proporcional, en la medida que  se ven afectados m&aacute;s &oacute;rganos la posibilidad de muerte aumenta.  Estos resultados coinciden con lo se&ntilde;alado por otros autores.<SUP>15-17</SUP>        <P>El da&ntilde;o que ocurre en el endotelio vascular junto a otras alteraciones  fisiopatol&oacute;gicas descritas en la sepsis, hacen que los diferentes  sistemas se involucren cada vez m&aacute;s, para aumentar el estado cr&iacute;tico  de los pacientes y con ello su riesgo de morir; esta situaci&oacute;n no  s&oacute;lo depende del n&uacute;mero de sistemas en fallo sino tambi&eacute;n  del grado de disfunci&oacute;n en ellos.<SUP>10,12,18</SUP>        <P>En nuestro trabajo se observa que no existe significaci&oacute;n estad&iacute;stica  cuando se relacionan los d&iacute;as que evoluciona el paciente en fallo  con la mortalidad, por lo que pensamos que &eacute;sta no depende exclusivamente  de la persistencia del s&iacute;ndrome. A quantitative evaluation of every patients is made according  to the alterations leading to organ dysfunction and thus tan individual  scoring is obtained. The measure of central or mean trend corresponding  to 11 points is applied to all patients&acute;scoring. Results: 85 % of  the dead patients obtained a scoring over 11 (p&lt;0.01), mortality was  higher among patients with 5 or more organs effects (p&lt;0.01), and this  group 90 % were above the mean score (p&lt;0.01).
