Objetivo: profundizar en los elementos necesarios para el tratamiento quirúrgico de pacientes con fractura de diáfisis humeral. Métodos: se realizó una revisión bibliográfica de un total de 400 artículos publicados en Pubmed, Hinari y Medline mediante el localizador de información Endnote, de ellos se utilizaron 50 citas seleccionadas para realizar la revisión, 47 de ellas de los últimos cinco años donde se incluyeron tres libros. Desarrollo: se discuten los aspectos relacionados con el diagnóstico clínico e imaginológico. Se abordan las indicaciones quirúrgicas según los criterios del McKee MD basados en propias de la fractura, traumas asociados y propios del enfermo. En relación a los métodos de osteosíntesis se plasman sus indicaciones en cada caso, así como sus ventajas y desventajas. En relación a las complicaciones se describen las más importantes y el accionar médico, se propone un algoritmo para pacientes con lesión del nervio radial. Conclusiones: el tratamiento quirúrgico de pacientes con fracturas de la diáfisis humeral está basado en indicaciones quirúrgicas muy específicas. El método de osteosíntesis a emplear depende de las ventajas de cada uno y de las condiciones del paciente y el cirujano. Los resultados por lo general son favorables con un mínimo de complicaciones.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[Background: generally speaking, fractures of the humeral diaphysis are treated in a conservative way, but there is a little percentage of patients that need surgical treatment based on very specific indications. Objective: to study in depth the necessary elements for the surgical treatment of patients with fracture of the humeral diaphysis. Methods: a bibliographic review of 400 articles published in Pubmed, Hinari and Medline by means of the reference management software Endnote, was made. Fifty references were selected for making the review, 47 of them were from the last five years and three books were included. Development: aspects related to the clinical and imaging diagnosis are discussed. Surgical indications according to Mckee criteria based on the indications of the fracture, on associated traumas and on own traumas of the patient, are tackled. Regarding the osteosynthesis methods, its indications step by step, as well as its advantages and disadvantages, are presented. Concerning the complications, the most important ones are described; the medical procedure is also described. An algorithm for patients with lesion of the radial nerve is proposed. Conclusions: the surgical treatment of patients with humeral diaphysis is based on very specific surgical indications. The osteosynthesis method to use depends on the advantages of each of them and on the conditions of the patient and the surgeon. In general, the results are favourable with a minimum of complications.]]></p></abstract>

La articulaci&oacute;n proximal (hombro) y la distal (codo) tienen  elementos muy espec&iacute;ficos. El hombro es la articulaci&oacute;n de mayor rango de  movimiento del organismo y su inmovilizaci&oacute;n prolongada reduce de manera  significativa la capacidad funcional. Por otra parte el codo aunque es m&aacute;s  estable en relaci&oacute;n al hombro, cuando se inmoviliza por tiempo prolongado sufre  limitaciones en su funci&oacute;n. <sup>1-3</sup> </font></p>     ]]></body>
<body><![CDATA[<p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Se definen como fracturas de la di&aacute;fisis humeral (FDH) aquellas que  ocurren distales al cuello quir&uacute;rgico del humero y proximal a la regi&oacute;n  supracond&iacute;lea. Para definir la zona diafisiaria puede ser empleado tambi&eacute;n el  m&eacute;todo del cuadrado descrito por Urs Heim, citado por M&uuml;ller ME, et al. <sup>4</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Seg&uacute;n McKee MD y Larson S <sup>5</sup> las FDH son frecuentes, con una  incidencia aproximada de 70 000 al a&ntilde;o en los Estados Unidos de Am&eacute;rica. Esta enfermedad  traum&aacute;tica, seg&uacute;n estos propios autores, representan del 3 % al 5 % de todas  las fracturas. </font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">El sexo masculino es el m&aacute;s afectado por traumas de alta energ&iacute;a en  pacientes j&oacute;venes y el femenino por los de baja cuant&iacute;a, en especial por encima  de los 51 a&ntilde;os de edad. <sup>6-7</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">El 5 % de estas fracturas son abiertas, el 63 % presenta un trazo de  fractura simple y el 90 % de los pacientes puede ser tratado de forma  conservadora. <sup>8</sup> Sin embargo el resto de los enfermos necesita de  tratamiento quir&uacute;rgico. </font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Debido a  la importancia del tema, la siguiente investigaci&oacute;n se propone profundizar en  los elementos necesarios para el tratamiento quir&uacute;rgico de enfermos con FDH. </font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif"><b>&nbsp;</b></font></p>     <p align="justify"><font size="3" face="Verdana, Arial, Helvetica, sans-serif"><b>M&Eacute;TODOS</b></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Se realiz&oacute; una revisi&oacute;n bibliogr&aacute;fica de un  total de 400 art&iacute;culos publicados en Pubmed, Hinari y Medline mediante el localizador  de informaci&oacute;n Endnote, de ellos se utilizaron 50 citas seleccionadas para  realizar la revisi&oacute;n, 47 de ellas de los &uacute;ltimos cinco a&ntilde;os donde se incluyeron tres libros. </font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif"><b>&nbsp;</b></font></p>     <p align="justify"><font size="3" face="Verdana, Arial, Helvetica, sans-serif"><b>DESARROLLO</b></font></p>     ]]></body>
<body><![CDATA[<p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Las fracturas de la di&aacute;fisis humeral se pueden presentar de forma  aislada o combinada a otras lesiones traum&aacute;ticas.<sup>9-11</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Aunque generalmente se tratan de manera conservadora, las indicaciones  quir&uacute;rgicas se imponen en ciertas situaciones, donde se conjugan factores  generales y locales del enfermo. <sup>12-14</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Los tratamientos quir&uacute;rgicos est&aacute; en estrecha relaci&oacute;n con el tipo de  implante a utilizar as&iacute; como la localizaci&oacute;n anat&oacute;mica de la fractura. <sup>15-17</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">El diagn&oacute;stico positivo de esta enfermedad se basa en los antecedentes,  cuadro cl&iacute;nico y examen radiogr&aacute;fico. <sup>18-21</sup> </font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">El antecedente m&aacute;s relevante en estos enfermos consiste en la presencia  de un trauma de alta o baja energ&iacute;a, despu&eacute;s del cual el paciente es llevado al  cuerpo de guardia con la actitud t&iacute;pica de sujetar el brazo enfermo con el  sano. <sup>22,23</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">En el examen f&iacute;sico se detecta acortamiento, angulaci&oacute;n y rotaci&oacute;n de la  extremidad. Puede haber o no presencia de heridas que comuniquen con el foco de  fractura. Mediante la palpaci&oacute;n se comprueba crepitaci&oacute;n &oacute;sea y movilidad  anormal a nivel del foco de fractura. <sup>24-26</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">En la exploraci&oacute;n f&iacute;sica, uno de los elementos m&aacute;s importantes es el  examen neurol&oacute;gico, en especial del nervio radial. <sup>27</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Se debe examinar a los pacientes en busca de otras lesiones asociadas de  la propia extremidad o en otras zonas del organismo. <sup>28</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">La exploraci&oacute;n radiogr&aacute;fica mediante vistas en proyecciones anteroposterior  y lateral son determinantes para el diagn&oacute;stico de la fractura y establecer su  configuraci&oacute;n geom&eacute;trica. <sup>30-32</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Los ex&aacute;menes de tomograf&iacute;a axial computarizada e imagen de resonancia magn&eacute;tica  no est&aacute;n justificados como procedimientos de de rutina para pacientes con esta enfermedad  traum&aacute;tica. <sup>30-33</sup></font></p>     ]]></body>
<body><![CDATA[<p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Las fracturas de la di&aacute;fisis humeral son por lo general tratadas de  manera conservadora, pero este aspecto no se desarrolla en la presente  investigaci&oacute;n, ya que se trata sobre tratamiento de tipo quir&uacute;rgico.</font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Las indicaciones quir&uacute;rgicas seg&uacute;n McKee MD, et al, <sup>5</sup> son: </font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">I. Propias de la fractura    <br>   -Incapacidad para mantener la reducci&oacute;n cerrada (acortamiento mayor a  tres cent&iacute;metros, rotaci&oacute;n mayor a 30 grados, angulaci&oacute;n mayor a 20 grados).    <br>   -Fracturas segmentarias.    <br>   -Fracturas patol&oacute;gicas.    <br>   -Fracturas con extensi&oacute;n intrarticular hacia el hombro o codo.     <br>   II.  Indicaciones por traumas asociados    <br>   -Fracturas abiertas.    <br>   -Lesi&oacute;n vascular.    ]]></body>
<body><![CDATA[<br>   -Da&ntilde;o del plexo braquial.    <br>   -Fractura de antebrazo ipsilateral.    <br>   -Fracturas del hombro y codo ipsilaterales, fracturas bilaterales.    <br>   -Fracturas de la extremidad inferior que requieren del apoyo de carga de  peso a trav&eacute;s del miembro superior.     <br>   -Quemaduras.    <br>   -Las causadas por armas de fuego de alta velocidad.    <br>   III. Indicaciones propias del enfermo:     <br>   -Pacientes politraumatizados.     <br>   -Trauma craneal asociado con Glasgow &le; 8.     <br>   -Trauma tor&aacute;cico.     ]]></body>
<body><![CDATA[<br>   -Pobre tolerancia del paciente al tratamiento conservador, obesidad y  mamas voluminosas.     <br>   Una vez que se decide el tratamiento quir&uacute;rgico, es importante  seleccionar el tipo de implante a utilizar. Los medios de fijaci&oacute;n empleados  son: placas y tornillos del sistema AO de 4,5 mil&iacute;metros,  placas mini invasivas conocidas en la literatura anglosajona como: <u>minimal</u> <u>invasive</u> <u>plate</u> <u>osteosynthesis</u> (mipo), intramedulares  flexibles o r&iacute;gidos y la fijaci&oacute;n externa. <sup>34-36</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Cada uno de estos sistemas de fijaci&oacute;n tiene ventajas y desventajas, que  se relacionan a continuaci&oacute;n (<a href="#cuadro">cuadro</a>). 5, 37</font></p>     <p align="center"><img src="/img/revistas/amc/v19n2/c00120215.jpg" alt="cuadro" width="587" height="400" longdesc="../img/c00120215.jpg"><a name="cuadro"></a></p>     
<p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Las l&aacute;minas AO tienen como ventaja que es el m&aacute;s empleado, presentan por  lo general buena consolidaci&oacute;n de la fractura, tiene bajo &iacute;ndice de  complicaciones y permite un retorno r&aacute;pido de la funci&oacute;n. Como desventajas esta  modalidad tiene la necesidad de ser m&aacute;s extenso con consecuencias cosm&eacute;ticas,  existe la probabilidad de lesi&oacute;n iatrog&eacute;nica del nervio radial, posible infecci&oacute;n  de la herida y p&eacute;rdida de la fijaci&oacute;n en especial en enfermos con hueso  osteopor&oacute;tico. <sup>38-40</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Por su parte la fijaci&oacute;n intramedular, tanto flexible como r&iacute;gida tiene  las siguientes ventajas: son realizadas a trav&eacute;s de peque&ntilde;as incisiones, no  produce ruptura del hematoma de fractura, menor disecci&oacute;n de partes blandas y  permite altos &iacute;ndices de consolidaci&oacute;n aunque no comparables con los de las  placas AO. Sin embargo con el uso de este sistema de fijaci&oacute;n puede presentarse  dolor posoperatorio del hombro, lesi&oacute;n del manguito rotador y rigidez del  hombro. <sup>41-43</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">La indicaci&oacute;n de la fijaci&oacute;n intramedular es indicada en pacientes con  fracturas de tipo segmentarias, en enfermos obesos o con quemaduras, tambi&eacute;n en  los pacientes politraumatizados. Este m&eacute;todo est&aacute; contraindicado en pacientes  con enfermedad pre-existente del hombro, canal medular estrecho menor de nueve  mil&iacute;metros y lesi&oacute;n asociada del nervio radial. <sup>5,10,40</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">La fijaci&oacute;n externa es empleada en pacientes politraumatizados, en  fracturas abiertas y en lesiones vasculares. <sup>5,41</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Las complicaciones m&aacute;s frecuentes en pacientes con fractura de la  di&aacute;fisis humeral son par&aacute;lisis del nervio radial, lesi&oacute;n del plexo braquial,  retardo de la consolidaci&oacute;n, seudoartrosis e infecci&oacute;n.<sup>5,42</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Las par&aacute;lisis del nervio radial pueden encontrarse desde un 3 % a un 34  % y existen factores relacionados con su presencia como: fracturas abiertas,  pacientes politraumatizados, da&ntilde;o vascular y fracturas ipsilaterales. La  mayor&iacute;a de estas lesiones son del tipo de la neuropraxia.  <sup>43</sup></font></p>     ]]></body>
<body><![CDATA[<p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">En caso de par&aacute;lisis del nervio radial, existen indicaciones para su  exploraci&oacute;n quir&uacute;rgica seg&uacute;n Carrol EA et al 44 como: fracturas  abiertas, traumas por alta velocidad por armas de fuego o da&ntilde;o penetrante,  lesi&oacute;n vascular, da&ntilde;o neurol&oacute;gico despu&eacute;s de la reducci&oacute;n cerrada y fracturas  del tercio distal como las descritas por Holstein Lewis citado por Liu GY, et al.  <sup>45</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Para el tratamiento de pacientes con lesi&oacute;n del nervio radial y fractura  de di&aacute;fisis humeral, se recomienda el siguiente algoritmo que sirve de gu&iacute;a a  todos los m&eacute;dicos que traten este tipo de afecci&oacute;n traum&aacute;tica (<a href="#figura">figura</a>). </font></p>     <p align="center"><img src="/img/revistas/amc/v19n2/f00120215.jpg" alt="figura" width="469" height="545" longdesc="../img/f00120215.jpg"><a name="figura"></a></p>     
<p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">La  seudoartrosis despu&eacute;s del tratamiento quir&uacute;rgico responde por lo general a  errores de la t&eacute;cnica como selecci&oacute;n inadecuada de la placa AO, distracci&oacute;n del  foco de fractura, inadecuada colocaci&oacute;n de tornillos y fallo mec&aacute;nico en  pacientes con hueso osteopor&oacute;tico. El tratamiento de esta complicaci&oacute;n se basa  en dos pilares, el primero lograr la estabilidad mec&aacute;nica y el otro estimular  la consolidaci&oacute;n &oacute;sea; por ejemplo el uso de placas AO con injerto de cresta  il&iacute;aca ha demostrado ser efectivo en la mayor&iacute;a de los pacientes. <sup>46,47</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">En  ocasiones la seudoartrosis se presenta en pacientes con hueso osteopor&oacute;tico,  por lo que en este caso se pueden aplicar varias opciones como: placas m&aacute;s  largas con mayor cantidad de tornillos, uso de cemento &oacute;seo, empleo de placas  anguladas, injerto &oacute;seo ant&oacute;logo, por ejemplo de peron&eacute;. <sup>47,49</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Seg&uacute;n McKee  MD, et al, <sup>5</sup> en pacientes con presencia de seudoartrosis se recomienda  primero investigar si existe o no infecci&oacute;n; antes de comenzar el tratamiento,  los acortamientos menores de tres cent&iacute;metros son bien tolerados por los  enfermos; la correcci&oacute;n de la deformidad y el tratamiento con placas es el  m&eacute;todo de elecci&oacute;n; agregar injerto &oacute;seo o sustitutos en caso de seudoartrosis  atr&oacute;fica y emplear las t&eacute;cnicas descritas con anterioridad en pacientes con  hueso osteopor&oacute;tico. </font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">La infecci&oacute;n como complicaci&oacute;n de las fracturas de la di&aacute;fisis humeral  es infrecuente debido a la excelente vascularidad de la zona y de cobertura  muscular, incluso en pacientes que presentan fracturas abiertas y son tratados  mediante la colocaci&oacute;n de placas AO de forma inmediata. Esta complicaci&oacute;n por  lo general se relaciona con la presencia de diabetes mellitus y otras  enfermedades que afectan el sistema inmunol&oacute;gico del organismo. Una vez que la  infecci&oacute;n est&aacute; presente de manera local se realizan lavados y desbridamientos  de la herida, con toma seriada de cultivos con el objetivo de identificar el  germen causal y es necesaria la administraci&oacute;n de antimicrobianos. En relaci&oacute;n  al implante quir&uacute;rgico, si est&aacute; fijo se deja en su posici&oacute;n y de otra manera es  retirado seguido de la colocaci&oacute;n de una fijaci&oacute;n externa. Para el tratamiento  de esta complicaci&oacute;n se pueden aplicar adem&aacute;s antimicrobianos locales, injertos  &oacute;seos o sustitutos. <sup>48,50</sup></font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif"><b>&nbsp;</b></font></p>     <p><font size="3" face="Verdana, Arial, Helvetica, sans-serif"><b>CONCLUSIONES</b></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">El  tratamiento quir&uacute;rgico de pacientes con FDH est&aacute; basado en indicaciones  quir&uacute;rgicas muy espec&iacute;ficas. El m&eacute;todo de osteos&iacute;ntesis a emplear depende de  las ventajas de cada uno y de las condiciones del enfermo y el cirujano. Los  resultados por lo general son favorables con un m&iacute;nimo de complicaciones. </font></p>     ]]></body>
