Objetivo: profundizar en los elementos esenciales y características de pacientes que sufren de fracturas segmentarias de la diáfisis tibial. Métodos: se realizó una revisión bibliográfica de un total de 400 artículos publicados en Pubmed, Hinari y Medline, mediante el localizador de información Endnote, de ellos se utilizaron 51 citas seleccionadas para realizar la revisión, 47 de ellas de los últimos cinco años, además se incluyeron cuatro libros. Desarrollo: se describen los mecanismos de producción más frecuentes, así como las principales diferencias entre las fracturas segmentarias y no segmentarias. En relación a la clasificación, se describió la propuesta por el grupo de Asociación para la Osteosíntesis (AO) en especial la del tipo C y la aplicación de la clasificación de Winquist R y Hansen S. Se hace referencia a los traumas asociados y las complicaciones reportadas en la literatura. Los tipos de tratamientos son por lo general quirúrgicos y pueden ser mediante enclavijado intramedular, uso de fijadores externos y placas subcutáneas. Conclusiones: las fracturas segmentarias de la diáfisis tibial son lesiones graves, que necesitan de acciones inmediatas, para prevenir o aminorar las complicaciones inmediatas y tardías características de esta lesión.]]></p></abstract>

Objective: to deepen in the essential elements and characteristics of patients who suffer from segmental tibial shaft fractures. Methods: a bibliographic review of 400 articles published in Pubmed, Hinari and Medline was made by means of the reference management software Endnote. Fifty-one references were selected for making the review, 47 of them were from the last five years and four books were included. Development: the most frequent mechanisms of production are described, as well as the main differences between segmental and non-segmental fractures. Regarding the classification, the proposal described by the Osteosynthesis Association, especially type C was described, as well as the application of the classification by Winquist R and Hansen S. The traumas associated and the complications reported in the literature are mentioned. The types of treatment are generally surgical and can be conducted by means of intramedullar nailing, external fixators and subcutaneous plates. Conclusions: segmental tibial shaft fractures are serious lesions that need to be treated immediately to prevent or reduce the instant complications and the belated characteristics of this lesion.]]></p></abstract>

<body><![CDATA[<p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Las FSDT tienen las  siguientes caracter&iacute;sticas: son causadas por trauma de alta energ&iacute;a, necesitan  de mayor tiempo de consolidaci&oacute;n, se asocian a un mayor n&uacute;mero de  complicaciones inmediatas y tard&iacute;as, presentan afecci&oacute;n extensa de partes  blandas, se asocian a otras lesiones traum&aacute;ticas, una gran cantidad de casos  son abiertas y el foco proximal por lo general consolida m&aacute;s r&aacute;pido que el  distal. <sup>4-6</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">En la evaluaci&oacute;n  cl&iacute;nica de estos enfermos, es importante el examen local donde se detecta  deformidad, heridas que comunican con uno o los dos focos de fractura y  afecci&oacute;n marcada del manto cut&aacute;neo. Es imprescindible, adem&aacute;s, el examen  neurovascular, con vistas a identificar estas lesiones de forma temprana y  actuar para evitar complicaciones que justifiquen la amputaci&oacute;n de la  extremidad o la muerte del enfermos. <sup>7-9</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">El tratamiento de  los pacientes que presentan FSDT no es simple, ya que se conjugan una serie de  factores que no solo dependen de la fractura en s&iacute;, sino disponibilidad de  medios de fijaci&oacute;n y entrenamiento del cirujano. Los m&eacute;todos de tratamiento  pueden ser de los conservadores a los quir&uacute;rgicos, estos &uacute;ltimos son los  preferidos para realizar el control de da&ntilde;os de estos enfermos. <sup>10-12</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Debido a la  importancia del tema, los autores del presente trabajo se proponen profundizar  en el tema, con vistas a que sirva de ayuda a los profesionales involucrados en  el tratamiento de esta lesi&oacute;n traum&aacute;tica.</font></p>     <p align="justify">&nbsp;</p>     <p align="justify"><font size="3" face="Verdana, Arial, Helvetica, sans-serif"><b>M&Eacute;TODOS</b></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Se realiz&oacute; una revisi&oacute;n bibliogr&aacute;fica de un  total de 400 art&iacute;culos publicados en Pubmed, Hinari y Medline mediante el  localizador de informaci&oacute;n Endnote, de ellos se utilizaron 51 citas seleccionadas  para realizar la revisi&oacute;n, 47 de ellas de los &uacute;ltimos cinco a&ntilde;os donde se  incluyeron cuatro libros.</font></p>     <p align="justify">&nbsp;</p>     <p align="justify"><font size="3" face="Verdana, Arial, Helvetica, sans-serif"><b>DESARROLLO</b></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Seg&uacute;n Court-Brown CM et  al, <sup>13</sup> los mecanismos de producci&oacute;n m&aacute;s frecuente en pacientes con  FSDT son los accidentes automovil&iacute;sticos y las ca&iacute;das con un 35 % y 10,9 %  respectivamente. En un estudio comparativo entre fractura no segmentaria y  segmentaria de la di&aacute;fisis tibial realizado por Terra M et al, <sup>3</sup> se  evidencias las caracter&iacute;sticas de las FSDT en relaci&oacute;n a retardo en la  consolidaci&oacute;n &oacute;sea, donde 21 de 29 enfermos presentaron esta complicaci&oacute;n. Con  relaci&oacute;n a la seudoartrosis est&aacute; se detect&oacute; en cuatro pacientes de 29. El  tiempo de consolidaci&oacute;n fue mayor en los pacientes con FSDT y el foco distal  necesit&oacute; de un mayor tiempo, que promedi&oacute; las 32 semanas. La re-operaci&oacute;n no  planificada fue mayor en el grupo de pacientes con FSDT (<a href="#tabla1">tabla 1</a>).</font></p>     ]]></body>

<p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Hasta la actualidad  no existe una clasificaci&oacute;n en espec&iacute;fico para las FSDT, sin embargo el grupo  AO (<i>Association</i> <i>for</i> <i>Osteosynthesis</i>), <sup>14-16</sup>  propone una divisi&oacute;n en grupos, dentro de ellos, las del grupo C que abarcan de  forma muy precisa este tipo de fractura, las que se presentan seg&uacute;n Court-Brown  CM, et al, <sup>13</sup> con mayor incidencia en el tercio proximal de la  di&aacute;fisis tibial en un 16,6 %.</font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">La aplicaci&oacute;n de esta  clasificaci&oacute;n, no solo permite catalogar el tipo de fractura, sino que adem&aacute;s  sirve de pron&oacute;stico de la lesi&oacute;n, al avanzar en su puntuaci&oacute;n. <sup>17-19</sup>  Por otra parte, existe una clasificaci&oacute;n que puede ser empleada en este tipo de  lesiones traum&aacute;ticas, que es la propuesta por Winquist R y Hansen S, <sup>20</sup>  en espec&iacute;fico los tipos tres y cuatro. Aunque esta fue descrita para fracturas  de la di&aacute;fisis femoral, puede ser empleada en caso de FSDT.</font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Dentro de las  lesiones asociadas, las m&aacute;s frecuentes son: traumas m&uacute;sculo-esquel&eacute;ticos en un  50 %, trauma de t&oacute;rax 30 %, trauma abdominal y cr&aacute;neo-facial 20 %. <sup>21-23</sup>  En relaci&oacute;n a los traumas m&uacute;sculo-esquel&eacute;ticos, estos pueden ocurrir en la  misma tibia afectada, ya que el paciente puede presentar una fractura  segmentaria de la tibia y presentar una fractura del tobillo, adem&aacute;s de poder  presentar fractura de la di&aacute;fisis femoral del mismo lado, conocido en la  literatura como rodilla flotante. <sup>24-26</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Seg&uacute;n Teraa M, et  al, <sup>3</sup> los riesgos relativos de los pacientes con FSDT son varios y  dentro de ellos los que mostraron significaci&oacute;n estad&iacute;sticas se encontraron: problemas  &oacute;seos (RR 2,4/ p=0,003), retardo de la consolidaci&oacute;n (RR 2/ p=0,036), infecci&oacute;n  profunda (RR 22/ p=0,031), re-operaci&oacute;n (RR 2/ p=0,036) y retardos en la  cicatrizaci&oacute;n (RR 1,7/ p=0,015). Las estad&iacute;sticas de este autor confirman que  los pacientes con FSDT presentan complicaciones graves, que interfieren en el  pron&oacute;stico (<a href="#tabla2">tabla 2</a>).</font></p>     <p align="center"><img src="/img/revistas/amc/v19n4/t02110415.jpg" alt="tabla 2" width="578" height="284" longdesc="../v19n4/img/t02110415.jpg"><a name="tabla2"></a></p>     
<p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">En caso que se  presente seudoartrosis, debe ser tratada de forma convencional como en las  fracturas de tibia no segmentarias, en ocasiones cuando existe rigidez del  tobillo se puede realizar una artrodesis de esta articulaci&oacute;n para mejorar la  irrigaci&oacute;n sangu&iacute;nea del segmento m&aacute;s distal de la tibia y de esta manera  ayudar a la consolidaci&oacute;n &oacute;sea. <sup>27-29</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">El tratamiento de  pacientes con FSDT puede ser desde el conservador al quir&uacute;rgico. El tratamiento  conservador se asocia a muy malos resultados por la naturaleza de la lesi&oacute;n, de  all&iacute; que las modalidades quir&uacute;rgicas son las m&aacute;s indicadas en estos enfermos. <sup>30-32</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">El tratamiento  quir&uacute;rgico puede ser mediante el uso de fijaci&oacute;n externa, de forma transitoria  o definitiva, placas subcut&aacute;neas y la fijaci&oacute;n intramedular. <sup>33-35</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">La fijaci&oacute;n  intramedular mediante clavos autocerrojados es el tratamiento quir&uacute;rgico de  elecci&oacute;n por la mayor&iacute;a de los autores consultados para la realizaci&oacute;n de este  trabajo, las ventajas de este m&eacute;todo radican en la mayor conservaci&oacute;n posible  de las partes blandas adosadas en el fragmento intermedio de la fractura, pero  mediante esta modalidad se pueden detectar rotaciones y angulaciones del  fragmento intermedio, que dificultan la consolidaci&oacute;n &oacute;sea. Para la correcta colocaci&oacute;n  del clavo intramedular, en ocasiones es necesaria la aplicaci&oacute;n de un  distractor. <sup>36-38</sup></font></p>     ]]></body>
<body><![CDATA[<p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">La fijaci&oacute;n con  placas subcut&aacute;neas es un m&eacute;todo muy &uacute;til, sin embargo no existen hasta la  actualidad estudios a largo plazo, que justifiquen esta variante de  tratamiento. <sup>39-41</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">La fijaci&oacute;n externa  es usada tanto de forma temporal como definitiva. <sup>42-44</sup> Cuando se  emplea la fijaci&oacute;n externa de forma transitoria, es necesario tener en cuenta las  siguiente precauciones: mantener los pines lo m&aacute;s alejados posible de los focos  de fracturas, pues esto provoca p&eacute;rdida de la estabilidad y se deben aplicar  dobles barras y usar fijadores de varios planos, segundo: emplear barras  radiol&uacute;cidas con vistas a observar la alineaci&oacute;n del foco de fractura, y  tercero: colocar al menos de dos a tres pines proximales y distales a los focos  de fractura, para colocar lo antes posible las barras en los pacientes  politraumatizados. <sup>45, 46 </sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">La conversi&oacute;n de la  fijaci&oacute;n externa a la intramedular debe ser antes de los 28 d&iacute;as, <sup>47</sup>  ya que la posibilidad de infecci&oacute;n en el trayecto de los pines se incrementa en  un 83 % pasado este tiempo, pero el tiempo ideal es de siete a diez d&iacute;as. <sup>48-50</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Giotakis N, et al, <sup>51</sup>  aboga por el tratamiento quir&uacute;rgico mediante la aplicaci&oacute;n de fijaci&oacute;n externa  circular, ya que permite el cuidado de las heridas y la correcci&oacute;n de las  deformidades, as&iacute; como de la consolidaci&oacute;n &oacute;sea.</font></p>     <p align="justify">&nbsp;</p>     <p align="justify"><font size="3" face="Verdana, Arial, Helvetica, sans-serif"><b>CONCLUSIONES</b></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">La FSDT  es una entidad grave provocada por traumas de gran energ&iacute;a, de all&iacute; que se  asocia a gran cantidad de complicaciones locales, generales, inmediatas y  tard&iacute;as. La consolidaci&oacute;n del foco distal es m&aacute;s demorada que la del proximal.
