Los craneofaringiomas son considerados una enfermedad molecular compleja, con mayor invasividad y adherencia a las estructuras neurovasculares de la base del cráneo. Entre los objetivos de la investigación destacó mostrar un modelo endoscópico de disección quirúrgica basado en nuevas clasificaciones imagenológicas del tumor.  Método.  Se realizó un estudio descriptivo, longitudinal y prospectivo en 50 pacientes con craneofaringiomas. Se realizó un abordaje endonasal endoscópico extendido siguiendo la regla de 3 de la disección quirúrgica: exposición del complejo trianatómico, disección trilaminar en el plano sagital y disección del trípode vascular en el plano axial, según nuevas clasificaciones imagenológicas.  Resultados y discusión.  En el 90 % de los casos se alcanzó una resección macroscópica total y el 80 % se incorporó a una vida normal y al trabajo. Las complicaciones quirúrgicas fueron mínimas, la diabetes insípida fue la más frecuente. La supervivencia libre de enfermedad a los 8 años fue de 81,6 %.  Conclusiones.  El abordaje endonasal endoscópico extendido y la disección quirúrgica según la regla de 3 han constituido un paradigma en el tratamiento de los craneofaringiomas.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[ABSTRACT  Introduction. Craniopharyngiomas are regarded as a complex molecular disease, with great neurovascular invasiveness and adherence in the skull base. Objective: to show an endoscopic model of surgical dissection based on new tumoral imagenological classification.  Methods.  A descriptive, longitudinal and prospective study was performed in 50 patients operated by Extended Endoscopic Endonasal Approach following the rule of 3 of surgical dissection: trianatomic complex exposition, sagittal trilaminar dissection, and axial vascular tripod dissection based on new imagenological classification.  Results and discussion.  Gross total resection was performed in 90 % of patients and 80 % was incorporated to normal live and work. Surgical complication was minimal, and insipidus diabetes was the most frequent. The disease-free survival during eight years was 81,6 %.  Conclusions.  Extended Endoscopic Endonasal Approach following the rule of three of surgical dissection represents a paradigm in the craniopharyngioma treatment.]]></p></abstract>
