Los pacientes rechazan la tintura 20% en las diarreas por su sabor desagradable. Objetivo: evaluar el estado del arte en la utilidad de los extractos de las hojas de esta especie. Métodos: se realizó una extensa revisión en bases de datos internacionales. Se revisaron las bases HighWire, DOAJ, EBSCO, Scielo, Scopus, Chemical abstract, Medline, PudMed, Pharmaceutical Abstract entre los años 1753 y 2012. Resultados: se reporta la composición química y existen ensayos preclínicos y clínicos que confirman actividad antidiarreica. No se reportan efectos tóxicos. Existen pocas formas farmacéuticas para esta especie. Conclusiones: las hojas de guayaba son inocuas. Hay que realizar más estudios para garantizar la eficacia. Se deben preparar otras variantes terapéuticas para su uso como antidiarreico.]]></p></abstract>

The patients reject the 20 % tincture because of its unpleasant flavor. Objective: to evaluate the state of the art in the usefulness of the guava leaf extracts. Methods: an extensive review of the national and international databases such as HighWire, DOAJ, EBSCO, Scielo, Scopus, Chemical abstract, Medline, PudMed Pharmaceutical Abstract from 1753 to 2012. Results: the chemical composition of the guava leaves was reported. There are preclinical and clinical assays that confirm their antidiarrheal effect. No toxic effects were reported. The pharmaceutical forms of this species are limited. Conclusions: the leaves of this plant are innocuous. It is necessary to conduct more studies to guarantee the effectiveness of this species. Other therapeutic variants of this species should be prepared to use them as antidiarrheal agents.]]></p></abstract>

<body><![CDATA[<p>&nbsp;</p>    <p>&nbsp;</p>    <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2"><B><font size="3">INTRODUCCI&Oacute;N</font></B>  </font></p>    <P><font face="Verdana, Arial, Helvetica, sans-serif" size="2">La especie  <I>Psidium guajava </I>L., conocida popularmente como guayaba, familia Myrtaceae,  se ha utilizado tradicionalmente como antidiarreico y para los c&oacute;licos  intestinales.<SUP>1,2,3,4</SUP> A pesar de ser una planta reconocida por las Autoridades  Regulatorias de Cuba, no son muchos los estudios que se le han realizado para  evaluar las actividades que le confiere el uso etnobot&aacute;nico. </font>     <P><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Los  pacientes rechazan el consumo de la tintura en las diarreas por su sabor desagradable.  Teniendo en cuenta esto y por la necesidad de formular una variante terap&eacute;utica,  para mejorar la aceptabilidad de los pacientes, en este trabajo se realiz&oacute;  una revisi&oacute;n de la bibliograf&iacute;a, para conocer el estado del arte  en relaci&oacute;n con la utilidad, la composici&oacute;n, estudios precl&iacute;nicos,  cl&iacute;nicos, as&iacute; como las formas farmac&eacute;uticas reportadas para  las hojas de esta especie. </font>     <P>&nbsp;     <P><font face="Verdana, Arial, Helvetica, sans-serif" size="2"><B><font size="3">M&Eacute;TODOS</font></B>  </font>     <P><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Se realiz&oacute;  una b&uacute;squeda de informaci&oacute;n en la literatura disponible acerca del  uso etnobot&aacute;nico, la utilidad, la composici&oacute;n qu&iacute;mica, estudios  precl&iacute;nicos, cl&iacute;nicos, as&iacute; como de las formas farmac&eacute;uticas  reportadas, solamente, para las hojas de esta planta. Se hizo la b&uacute;squeda  en idioma espa&ntilde;ol e ingl&eacute;s, utilizando las palabras claves guayaba,  <I>Psidiun guajava</I>, toxicolog&iacute;a, farmacolog&iacute;a, ensayos cl&iacute;nicos  y formas farmac&eacute;uticas y las combinaciones l&oacute;gicas entre estas.  Se revisaron las bases HighWire, DOAJ, EBSCO, Scielo, Scopus, Chemical abstract,  Medline, PudMed, Pharmaceutical Abstract entre los a&ntilde;os 1753 y 2012. </font>      <P>&nbsp;     <P><font face="Verdana, Arial, Helvetica, sans-serif" size="2"><B><font size="3">RESULTADOS  </font></B></font> <B>     ]]></body>
<body><![CDATA[<P><font face="Verdana, Arial, Helvetica, sans-serif" size="2">    <br>  Uso etnobot&aacute;nico</font> </B>     <P><font face="Verdana, Arial, Helvetica, sans-serif" size="2">El  uso etnobot&aacute;nico de esta planta es amplio. Por su astringencia, las hojas  se utilizan en enfermedades de la piel, en diarreas y para otras dolencias de  est&oacute;mago. Se ha utilizado como hemost&aacute;tica y antis&eacute;ptica.<SUP>1</SUP>  Otros reportes indican acci&oacute;n antimicrobiana, cicatrizante, hipoglic&eacute;mica  y espasmol&iacute;tica.<SUP>5,6 </SUP>Se reporta tambi&eacute;n actividad antioxidante,  hepatoptotectora, antial&eacute;rgica, genot&oacute;xica citot&oacute;xica, cardiot&oacute;nica,  antiinflamatoria y anticatarral y otras.<SUP>7</SUP> </font>     <P><font face="Verdana, Arial, Helvetica, sans-serif" size="2"><B>Estudio  farmacogn&oacute;stico</B> </font>     <P><font face="Verdana, Arial, Helvetica, sans-serif" size="2">La  droga cruda recolectada en tres regiones de Cuba fue estudiada. Se evaluaron las  condiciones de secado y almacenamiento. Se concluy&oacute; que la droga seca almacenada  en bolsas de polietileno, mantiene la estabilidad hasta 6 meses.<SUP>8,9</SUP>  Se encontr&oacute; diferencia en la composici&oacute;n de los extractos de cada  regi&oacute;n.<SUP>6</SUP> Se validaron m&eacute;todos anal&iacute;ticos (UV-V),  para cuantificar taninos y flavonoides en las hojas de <I>Psidium guajava, </I>L.<SUP>7,  10</SUP> Se han informado la estandarizaci&oacute;n de la droga, para su inclusi&oacute;n  en una farmacopea.<SUP>11</SUP> </font>     <P><font face="Verdana, Arial, Helvetica, sans-serif" size="2"><B>Composici&oacute;n  qu&iacute;mica</B> </font>     <P><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Las  hojas de esta planta contienen taninos y fenoles, flavonoides y triterpenos y  esteroides, as&iacute; como de saponinas y compuestos aminados. <SUP>7,12</SUP>  Se ha reportado un aceite esencial y otras sustancias vol&aacute;tiles.<SUP>13</SUP>  Contiene, adem&aacute;s, &aacute;cido guajanoico, &szlig;-sitosterol, uvaol, &aacute;cido  olean&oacute;lico y &aacute;cido urs&oacute;lico; &aacute;cido 2-&aacute;-hidroxiurs&oacute;lico,  morin-3-O-&aacute;-L-arabopiranosido, hiperina, miricetina-3-O-&acirc;-D-glucosido,  quercetin-3-O-&szlig;-D-glucuronopiranosido, 1-O-galoil-&acirc;-D-glucosa.<SUP>13</SUP>  Se ha informado la presencia de &aacute;cido asc&oacute;rbico y de otros flavonoides  as&iacute; como az&uacute;cares reductores y alcaloides.<SUP> 14</SUP> Se ha aislado  una nueva benzofenona y un flavonol de naturaleza galoil-glic&oacute;sido, conjuntamente  con 5 nuevos quercetin-glic&oacute;sidos.<SUP>15 </SUP>Se ha informado el aislamiento  de nuevos flavonoides<SUP>16 </SUP>y de 4 nuevos triterpenos.<SUP>17</SUP> </font>      <P><font face="Verdana, Arial, Helvetica, sans-serif" size="2"><B> Efecto anticatarral</B>  </font>     <P><font face="Verdana, Arial, Helvetica, sans-serif" size="2">El extracto  acuoso de las hojas de guayaba, disminuye la tos y tiene efecto anticatarral en  ratas y coballos, en dosis de 2 a 5 g/kg respectivamente y posee una DL50 mayor  de 5g/kg.<SUP>18</SUP> </font>     <P><font face="Verdana, Arial, Helvetica, sans-serif" size="2"><B>Efecto  antiinflamatorio y analg&eacute;sico</B> </font>     ]]></body>
<body><![CDATA[<P><font face="Verdana, Arial, Helvetica, sans-serif" size="2">El  extracto acuoso de las hojas, en dosis de 50-800 mg/kg, v&iacute;a intraperitoneal,  produce un efecto antiinflamatorio en ratas Wistar y analg&eacute;sico en ratones  Balb/c; (ambos efectos son dosis dependiente).<SUP>19</SUP> Se reporta actividad  antiinflamatoria, en el modelo de inflamaci&oacute;n aguda inducido por carragenina  en ratas Wistar.<SUP>20</SUP> </font>     <P><font face="Verdana, Arial, Helvetica, sans-serif" size="2"><b>Efecto  hipoglucemiante e hipotensor </b></font>     <P><font face="Verdana, Arial, Helvetica, sans-serif" size="2">El  extracto acuoso de las hojas en dosis de 50-800 mg/kg, v&iacute;a peroral, en  ratas y ratones tiene efecto hipoglucemiante e hipotensor.<SUP>21</SUP> El extracto  etan&oacute;lico produce efecto anti-heperglucemiante, frente a un modelo de diabetes  tipo-2 en ratas Sprague Dawley.<SUP>22</SUP> El extracto acuoso inhibe la enzima  alfa-glucosidasa in vitro, reduciendo la elevaci&oacute;n postpandrial de glucosa  en sangre y mejora la hiperglicemia en modelos de ratones.<SUP>23</SUP> Los extractos  de las hojas inhiben la glicaci&oacute;n de prote&iacute;nas en pacientes diab&eacute;ticos<SUP>24</SUP>  adem&aacute;s de un efecto hipoglucemiante en diabetes tipo 2, en ratones, en  dosis de 10mg/kg, por un mecanismo de inhibici&oacute;n de la prote&iacute;na  tirosin- fosfatasa 1B.<SUP>25</SUP> </font>     <P><font face="Verdana, Arial, Helvetica, sans-serif" size="2"><B>Efecto  genot&oacute;xico y mutag&eacute;nico</B> </font>     <P><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Se  ha evaluado en varios modelos la actividad genot&oacute;xica de extractos de las  hojas de guayaba. No se observ&oacute; actividad genot&oacute;xica.<SUP>26,27</SUP>  Se estudi&oacute; el efecto mutag&eacute;nico en dos modelos experimentales diferentes,  en ambos el efecto mutag&eacute;nico estuvo ausente.<SUP>28</SUP> Estos resultados  avalan la inocuidad de esta planta, ampliamente utilizada en la medicina tradicional.  </font>     <P><font face="Verdana, Arial, Helvetica, sans-serif" size="2"><B>Toxicolog&iacute;a  aguda oral </B> </font>     <P><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Se  evalu&oacute; la toxicidad aguda de extractos de las hojas por el m&eacute;todo  de la dosis letal media y por m&eacute;todo de las clases de toxicidad aguda.  Se ha declarado, en ambos estudios ausencia de toxicidad, en la dosis l&iacute;mite  de 2000 mg/kg.<SUP>27,29 </SUP>La DL<SUB>50</SUB> es mayor de 5g/kg en roedores.<SUP>18</SUP>  </font>     <P><font face="Verdana, Arial, Helvetica, sans-serif" size="2"><B>Actividad  antibacteriana</B> </font>     <P><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Se  ha reportado actividad antibacteriana de amplio espectro para el extracto de las  hojas<SUP>2</SUP> Comparando los extractos acuosos, alcoh&oacute;licos y cet&oacute;nicos  de las hojas, frente a veinte cepas de bacterias de inter&eacute;s cl&iacute;nico.  El extracto acuoso mostr&oacute; actividad en el 35% de los casos, el alcoh&oacute;lico  en un 65% y el cet&oacute;nico en el 100% de los casos.<SUP> <I>30</I>, 31</SUP>  </font>     <P><font face="Verdana, Arial, Helvetica, sans-serif" size="2"><B>Actividad  citot&oacute;xica</B> </font>     ]]></body>
<body><![CDATA[<P><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Se  reporta que el extracto acuoso no es citot&oacute;xico, el cet&oacute;nico posee  citotoxicidad baja (5.4%) y el alcoh&oacute;lico una citotoxicidad media (43.8%).<SUP>32</SUP>  Otro informe reporta que el extracto acuoso posee efecto citot&oacute;xico, in  vitro, a corto plazo<SUP>33</SUP> y que presenta actividad anti-proliferativa.<SUP>34</SUP>  Se ha planteado que este extracto, interfiere en la cascada de se&ntilde;alizaci&oacute;n  m&uacute;ltiple vinculada con la tumorog&eacute;nesis.<SUP>35</SUP> </font>     <P><font face="Verdana, Arial, Helvetica, sans-serif" size="2"><B>Efecto  antioxidante</B> </font>     <P><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Se  report&oacute; efecto antioxidante in vitro de las hojas. En mayo y agosto el  efecto es mayor que en octubre y diciembre.<SUP>36</SUP> Otros autores reportan  actividad antioxidante <I>in vitro</I> e <I>in vivo.<SUP>37, 38 </SUP></I>Resulta  vital un estudio din&aacute;mico de la acumulaci&oacute;n de metabolito, para  seleccionar la &eacute;poca de colecta del material vegetal. </font>     <P><font face="Verdana, Arial, Helvetica, sans-serif" size="2"><B>Actividad  hepatoprotectora</B> </font>     <P><font face="Verdana, Arial, Helvetica, sans-serif" size="2">En  2011 se report&oacute; efecto hepatoprotector de los extractos acuosos de las  hojas de guayaba, frente a un modelo de intoxicaci&oacute;n con CCl4, en dosis  de 200mg/kg, en ratas Sprague Dawley.<SUP>39</SUP> </font>     <P><font face="Verdana, Arial, Helvetica, sans-serif" size="2"><B>Actividad  antidiarreica</B> </font>     <P><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Se  se&ntilde;ala que la sustancia activa del extracto de hojas es la quercetina.<SUP>40,41,42</SUP>  Se ha verificado el efecto espasmol&iacute;tico que produce.<SUP>43,44</SUP> Se  ha reportado un efecto dosis dependiente en dosis de 50 a 400 mg/kg (oral) con  una disminuci&oacute;n de la motilidad intestinal y retardo en el vaciado g&aacute;strico.<SUP>45</SUP>  El extracto acuoso de las hojas, disminuye la producci&oacute;n de toxinas l&aacute;biles  de <I>E. coli</I> y del c&oacute;lera.<SUP>46</SUP> Un ensayo cl&iacute;nico realizado  concluy&oacute; que la tintura al 20% de hoja de <I>Psidium guajava </I>tiene  efecto antidiarreico importante.<SUP>47</SUP> En otro, que evalu&oacute; el polvo  de las hojas secas, tambi&eacute;n se comprob&oacute; este efecto.<SUP>48</SUP>  </font>     <P><font face="Verdana, Arial, Helvetica, sans-serif" size="2"><B>Otros  efectos reportados</B> </font>     <P><font face="Verdana, Arial, Helvetica, sans-serif" size="2">El  extracto acuoso de las hojas en dosis de 400 mg/dL, tiene efecto estabilizador  de la membrana eritroc&iacute;tica <I>in vitro</I>.<SUP>49</SUP> Se reporta, adem&aacute;s,  que mejora los niveles de triglic&eacute;ridos y colesterol en modelos de ratones  en tejido hep&aacute;tico, mejorando el balance del perfil lip&iacute;dico.<SUP>23</SUP>  </font>     <P><font face="Verdana, Arial, Helvetica, sans-serif" size="2"><B>Formas  farmac&eacute;uticas reportadas </B> </font>     ]]></body>
<body><![CDATA[<P><font face="Verdana, Arial, Helvetica, sans-serif" size="2">En  la literatura solo se reportan como formas farmac&eacute;uticas de las hojas,  la tintura 20%, el polvo y el talco de guayaba.<SUP>50</SUP> Se report&oacute;  una suspensi&oacute;n oral utilizando polvo de las hojas secas.<SUP>9</SUP> Se  ha reportado una formulaci&oacute;n de c&aacute;psulas duras de 500mg, a partir  del polvo de las hojas secas.<SUP>5151</SUP> </font>     <P>&nbsp;     <P><b><font face="Verdana, Arial, Helvetica, sans-serif" size="3">CONCLUSIONES</font></b>      <P><font face="Verdana, Arial, Helvetica, sans-serif" size="2">En esta revisi&oacute;n  se pudo constatar que existen estudios que avalan la inocuidad de las hojas de  esta planta. Que se deben realizar otros estudios precl&iacute;nicos, cl&iacute;nicos  y tecnol&oacute;gicos para garantizar seguridad y eficacia en sus usos.
