<body><![CDATA[<p>&nbsp;</p>     <P>      <P> <font face="Verdana" size="3"><B>INTRODUCCI&Oacute;N </B></font>     <P> <B>     <P>      <P><font face="Verdana" size="2">Antecedentes de dengue en las Am&eacute;ricas    y en Cuba antes de 1981</font> </B>      <P>      <P><font face="Verdana" size="2">El dengue, enfermedad viral febril aguda causada    por cualquiera de los 4 serotipos del virus dengue (Den 1, 2, 3, 4) y transmitida    al hombre por la picada del mosquito <I>Aedes aegypti</I>, se reconoce en la    regi&oacute;n de las Am&eacute;ricas desde el siglo xviii; aunque epidemias    de una enfermedad tipo dengue han sido reportadas tan temprano como en 1699    en Panam&aacute;. En 1780 se describe al dengue por primera vez durante una    epidemia ocurrida en Filadelfia. Del per&iacute;odo 1780 a 1800 hay poca informaci&oacute;n    disponible. Durante los siglos xix y primera mitad del siglo xx la literatura    recoge epidemias ocurridas principalmente en el Caribe y sur de EE. UU.<SUP>1,2</SUP>    </font>     <P><font face="Verdana" size="2">A principios de los a&ntilde;os 50, el virus    dengue (Den 2) es aislado por primera vez en la regi&oacute;n, en Trinidad Tobago    en condiciones no epid&eacute;micas; este representa el primer aislamiento de    dengue en la regi&oacute;n. En el per&iacute;odo de 1960 a 1980, la regi&oacute;n    reconoce 3 grandes epidemias: Den 3 en 1963-1964, Den 2 y Den 3 en 1968-1969    y la epidemia de Den 1 de 1977-1978. Estas epidemias caracterizadas por el cuadro    benigno de la enfermedad (la fiebre del dengue, FD) afectaron principalmente    a pa&iacute;ses del Caribe, Centroam&eacute;rica y algunos pa&iacute;ses de    Sudam&eacute;rica.<SUP>2,3</SUP> Este per&iacute;odo se caracteriz&oacute; por    una baja endemicidad y la presencia de casos aislados de dengue hemorr&aacute;gico    (DH), la forma severa de la enfermedad.<SUP>1</SUP> </font>     <P><font face="Verdana" size="2">Despu&eacute;s de 1945, en que se observa una    epidemia de dengue en La Habana, no se reportaron casos cl&iacute;nicos sospechosos    de dengue hasta la introducci&oacute;n del virus Den 1 durante la pandemia de    1977.<SUP>4,5</SUP> Estudios serol&oacute;gicos realizados a inicios de los    a&ntilde;os setenta confirman la no circulaci&oacute;n de virus dengue en ese    per&iacute;odo.<SUP>6</SUP> Durante la epidemia de Den 1 de 1977 se registraron    m&aacute;s de 400 000 casos, todos con el cuadro benigno de la enfermedad.<SUP>5,7</SUP>    Estudios serol&oacute;gicos realizados con posterioridad demostraron que 44,46    % de la poblaci&oacute;n cubana se hab&iacute;a infectado con este serotipo    viral y estaba a riesgo de una infecci&oacute;n secundaria si otro serotipo    del virus dengue entraba al pa&iacute;s.<SUP>6</SUP> </font>      ]]></body>
<body><![CDATA[<P>     <P><font face="Verdana" size="2"><B><font size="3">LA EPIDEMIA CUBANA DE DENGUE    HEMORR&Aacute;GICO DE 1981</font></B> </font>      <P><font face="Verdana" size="2"><I>Antecedentes</I>: a finales de 1980, en la    regi&oacute;n de las Am&eacute;ricas se observaba una baja circulaci&oacute;n    del virus Den 2. Por otra parte, Cuba contaba con casi la mitad de su poblaci&oacute;n    a riesgo de una segunda infecci&oacute;n por otro serotipo de dengue. Finalmente,    a principios de 1981 se reporta por primera vez en la regi&oacute;n la introducci&oacute;n    del virus Den 4 (Dominica) y la primera epidemia de DH, en este caso en Cuba.<SUP>1,3,8</SUP>    </font>     <P><font face="Verdana" size="2"><I>Datos de la epidemia</I>: a finales de mayo    de 1981 se reconoce la epidemia. Estudios epidemiol&oacute;gicos retrospectivos    demostraron que comenz&oacute; a finales de 1980 en 3 municipios del este, centro    y oeste del pa&iacute;s; se reportaron casos en la misma semana epidemiol&oacute;gica    en personas sin antecedentes de viajar fuera del pa&iacute;s ni de su localidad.    En total se registraron 344 203 casos con 10 312 casos graves y muy graves,    y 158 fallecidos (de ellos 101 ni&ntilde;os) (<a href="/img/revistas/mtr/v64n1/t0101112.gif">tabla</a>).    </font>      
<P><font face="Verdana" size="2"><I>Cuadro cl&iacute;nico</I>: el DH se observ&oacute;    tanto en ni&ntilde;os como en adultos.<SUP>8,9</SUP> En los primeros, el cuadro    cl&iacute;nico fue similar al reportado en el sudeste asi&aacute;tico.<SUP>9-11</SUP>    En los adultos se registr&oacute; por primera vez en forma epid&eacute;mica.<SUP>12</SUP>    El choque fue m&aacute;s frecuente en los ni&ntilde;os pero m&aacute;s severo    en los adultos. Estudios serol&oacute;gicos demostraron que el DH se observ&oacute;    en casos con infecci&oacute;n secundaria Den1/Den 2, con una frecuencia de 1    por cada 23 infecciones secundarias en ni&ntilde;os y en 1 por cada 79 adultos.<SUP>13</SUP>    El estudio de los casos permiti&oacute; identificar por primera vez los signos    de alarma cl&iacute;nicos de la enfermedad.<SUP>9</SUP> </font>     <P><font face="Verdana" size="2"><I>Procedimiento, control y eliminaci&oacute;n    de la epidemia</I>: como parte de las acciones para su enfrentamiento se establecieron    las gu&iacute;as cl&iacute;nicas de diagn&oacute;stico y manejo cl&iacute;nico    de los casos donde la hospitalizaci&oacute;n temprana y la hidrataci&oacute;n    apropiada desempe&ntilde;aron un papel fundamental en la evoluci&oacute;n adecuada    de los enfermos. Para la mejor atenci&oacute;n y vigilancia de los pacientes,    las escuelas se habilitaron como hospitales, con una red para el traslado inmediato    de los pacientes que lo requirieran a los hospitales. El personal m&eacute;dico    fue redistribuido para una mejor atenci&oacute;n a los pacientes. Se crearon    grupos de expertos para el entrenamiento y la capacitaci&oacute;n r&aacute;pida    del personal m&eacute;dico y param&eacute;dico. En este contexto, los estudiantes    de medicina desempe&ntilde;aron un papel fundamental en la vigilancia cl&iacute;nica    (seguimiento de los par&aacute;metros cl&iacute;nicos y vigilancia de los signos    de alarma) de los pacientes hospitalizados. </font>     <P><font face="Verdana" size="2">Para el control y la eliminaci&oacute;n de la    epidemia se estableci&oacute; una campa&ntilde;a intensiva para la erradicaci&oacute;n    del vector, donde la comunidad tuvo un papel fundamental.<SUP>7,14</SUP> Las    acciones establecidas permitieron eliminar la epidemia en 4 meses, report&aacute;ndose    el &uacute;ltimo caso el 10 de octubre de 1981. </font>     <P><font face="Verdana" size="2"><I>Diagn&oacute;stico del agente etiol&oacute;gico    y vigilancia de laboratorio</I>: los estudios serol&oacute;gicos a partir de    sueros colectados en los primeros d&iacute;as de comienzo de la fiebre en pacientes    con un cuadro cl&iacute;nico sospechoso de la enfermedad permitieron detectar    niveles elevados de anticuerpos a flavivirus. Estos resultados, unidos al cuadro    cl&iacute;nico de la enfermedad y los antecedentes epidemiol&oacute;gicos en    la poblaci&oacute;n, permitieron brindar el diagn&oacute;stico presuntivo de    dengue y comunicar que se estaba en presencia de una epidemia de DH. Los estudios    de aislamiento viral en muestras colectadas en la etapa aguda de la enfermedad,    permitieron aislar e identificar al virus Den 2 como el agente etiol&oacute;gico    de la epidemia en los primeros d&iacute;as de identificada esta.<SUP>15-17</SUP>    </font>     <P><font face="Verdana" size="2">Para un mejor seguimiento y caracterizaci&oacute;n    de la epidemia se estableci&oacute; la vigilancia serol&oacute;gica que transcurri&oacute;    en 3 etapas: </font>     <P><font face="Verdana" size="2">1. La primera hasta el 24 de julio, tuvo como    objetivo identificar el agente etiol&oacute;gico de la epidemia y confirmar    la presencia de la transmisi&oacute;n en las diferentes provincias. Durante    esta etapa se procesaron 4 000 monosueros de pacientes. </font>     ]]></body>
<body><![CDATA[<P><font face="Verdana" size="2">2. Durante la segunda etapa, se trabaj&oacute;    en la confirmaci&oacute;n de los casos cl&iacute;nicos mediante el estudio de    sueros pareados con el fin de determinar la eficacia del diagn&oacute;stico.    En esta etapa se procesaron 2 017 pares de sueros. </font>     <P><font face="Verdana" size="2">3. En la tercera y &uacute;ltima etapa se estudi&oacute;    100 % de los casos sospechosos de dengue con el objetivo de certificar la eliminaci&oacute;n    de la epidemia. Durante todo el per&iacute;odo de la epidemia se mantuvo la    vigilancia virol&oacute;gica que permiti&oacute; aislar el virus en muestras    de 22 pacientes de FD, DH y fallecidos.<SUP>15,17</SUP> </font>     <P><font face="Verdana" size="2"><I>Investigaciones realizadas en el transcurso    de la epidemia</I>: las primeras investigaciones realizadas durante y despu&eacute;s    de eliminada la epidemia permitieron a) caracterizar el cuadro cl&iacute;nico    de DH tanto en ni&ntilde;os como adultos con una infecci&oacute;n confirmada    por Den 2; b) caracterizar cl&iacute;nica, virol&oacute;gica y patol&oacute;gicamente    el cuadro observado en 13 ni&ntilde;os fallecidos por DH; c) identificar los    factores de riesgo de DH en varios grupos de pacientes con una infecci&oacute;n    confirmada; d) caracterizar el agente etiol&oacute;gico de la enfermedad; e)    establecer los criterios serol&oacute;gicos de infecci&oacute;n primaria y secundaria    en las condiciones de Cuba; f) introducir la nueva tecnolog&iacute;a para el    diagn&oacute;stico.<SUP>9-12,15-23</SUP> </font>     <P>      <P>      <P><font face="Verdana" size="3"><B>PER&Iacute;ODO 1982-2011</B> </font>      <P><font face="Verdana" size="2">En los 30 a&ntilde;os transcurridos despu&eacute;s    de la epidemia de DH de 1981, se ha observado a escala global el incremento    del dengue y el DH, lo que se refleja en el aumento del reporte de casos, epidemias    y la extensi&oacute;n geogr&aacute;fica del vector y de la transmisi&oacute;n    de dengue. Hoy se estima que 1/3 de la poblaci&oacute;n mundial vive en &aacute;reas    de riesgo de infecci&oacute;n. La enfermedad se reporta en el Sudeste asi&aacute;tico,    Pac&iacute;fico Occidental, las Am&eacute;ricas, &Aacute;frica y Mediterr&aacute;neo    Oriental. Los 4 serotipos virales se han identificado en todas estas &aacute;reas.<SUP>24,25</SUP>    </font>     <P><font face="Verdana" size="2">Solo en la regi&oacute;n de las Am&eacute;ricas,    el dengue se ha incrementado en aproximadamente 6 veces y el DH en 12 veces.    En 2010 se reportaron 1 663 276 casos con 48 954 casos de DH y 1 194 fallecidos.    Treinta pa&iacute;ses reportan co-circulaci&oacute;n de 2 serotipos o m&aacute;s.    Las epidemias se han hecho m&aacute;s frecuentes, se acortan los ciclos entre    estas (<U><FONT  COLOR="#0000ff"><a href="http://new.paho.org/hq/index.php?option=com_content&task=view&id=4497&Itemid=3526&limit=1&limitstart=3" target="_blank">http://new.paho.org/hq/index.php?option=com_content&amp;task=view&amp;id=4497&amp;Itemid=3526&amp;limit=1&amp;limitstart=3</a></FONT></U>).<SUP>26</SUP>    </font>      <P><font face="Verdana" size="2">En este contexto tan complejo, Cuba ha trabajado    durante todos estos a&ntilde;os para mantener al pa&iacute;s libre de transmisi&oacute;n.    Para ello cuenta con un fuerte programa de vigilancia cl&iacute;nico-epidemiol&oacute;gico    con soporte de laboratorio, el cual de conjunto con el programa de control y    erradicaci&oacute;n del vector han permitido que el pa&iacute;s se mantenga    libre de endemia de dengue en un contexto regional muy complejo. Para ello la    voluntad pol&iacute;tica, la participaci&oacute;n de la comunidad y la intersectorialidad    de las acciones han sido decisivas. </font>     <P><font face="Verdana" size="2"><I>Diagn&oacute;stico y vigilancia</I>: en estos    30 a&ntilde;os adem&aacute;s del fortalecimiento del diagn&oacute;stico, la    vigilancia y las actividades de referencia, se han desarrollado investigaciones    en las diferentes disciplinas relacionadas con dengue que han apoyado los programas    de vigilancia y control del pa&iacute;s pero tambi&eacute;n han brindado nuevo    conocimiento para Cuba y el mundo, que es reconocida hoy como el &uacute;nico    pa&iacute;s que ha sido capaz de mantenerse libre de dengue y de eliminar las    transmisiones cuando estas se han producido. A su vez es considerada un centro    de excelencia para la capacitaci&oacute;n del personal; en su actividad de colaboraci&oacute;n    internacional y referencia ha apoyado a un gran n&uacute;mero de pa&iacute;ses    de la regi&oacute;n en el enfrentamiento y control del dengue. </font>     ]]></body>
<body><![CDATA[<P><font face="Verdana" size="2">El pa&iacute;s trabaja en el perfeccionamiento    de su vigilancia integrada (ambiental, entomol&oacute;gica, epidemiol&oacute;gica,    cl&iacute;nica y de laboratorio) con la participaci&oacute;n de todos los niveles    del sistema de salud. La vigilancia de laboratorio est&aacute; dirigida, en    particular, a la identificaci&oacute;n temprana y confirmaci&oacute;n de la    transmisi&oacute;n, seguimiento y caracterizaci&oacute;n de las epidemias y    su agente etiol&oacute;gico, y a la certificaci&oacute;n del cese de la transmisi&oacute;n.    A partir de 1997 se estableci&oacute; la vigilancia activa para la b&uacute;squeda    de casos y confirmaci&oacute;n del diagn&oacute;stico. Para ello, han sido fundamentales:    el papel de la atenci&oacute;n primaria de salud, la red de laboratorios provinciales    con capacidad para el diagn&oacute;stico serol&oacute;gico utilizando una tecnolog&iacute;a    cubana, el UMELISA Dengue IgM (desarrollada por el Centro de Inmunoensayos,    CIE), la actividad de referencia nacional (a cargo de los Laboratorios Nacionales    de Referencia del Instituto de Medicina Tropical &quot;Pedro Kour&iacute;&quot;,    IPK) y la integraci&oacute;n activa de todos los niveles del sistema para el    an&aacute;lisis din&aacute;mico de las situaciones epidemiol&oacute;gicas. Como    parte de la atenci&oacute;n integrada, la vigilancia del vector desempe&ntilde;a    un papel decisivo, para identificar su incremento y sus sitios de cr&iacute;a    para la toma de acciones inmediatas que permitan disminuir el riesgo, adem&aacute;s    de la vigilancia de la resistencia a insecticidas. Para el desarrollo de estas    actividades se cuenta con una red de laboratorios de entomolog&iacute;a a lo    largo de todo el pa&iacute;s, as&iacute; como el laboratorio nacional de referencia    de entomolog&iacute;a en el IPK y la Direcci&oacute;n Nacional de Vigilancia    y Lucha Antivectorial. Como parte de la vigilancia integrada, la vigilancia    cl&iacute;nico epidemiol&oacute;gica, desarrollada a todos los niveles del sistema    nacional de salud permite identificar tempranamente los riesgos, notificar los    casos de manera oportuna, estudiar la distribuci&oacute;n temporo-espacial de    estos, as&iacute; como trazar y evaluar las estrategias para su eliminaci&oacute;n.    Estas acciones est&aacute;n insertadas en el programa cubano de control sanitario    internacional. </font>      <P><font face="Verdana" size="2"><I>Resultados de algunas investigaciones desarrolladas    en el per&iacute;odo</I>: </font>     <P><font face="Verdana" size="2">A diferencia de lo observado en el Sudeste asi&aacute;tico    y la mayor&iacute;a de los pa&iacute;ses de la regi&oacute;n de las Am&eacute;ricas,    las epidemias en Cuba han sido eliminadas. Con posterioridad a la epidemia de    1981, se han reportado las epidemias de 1997 (Den 2) en Santiago de Cuba y la    de 2001-2002 (Den 3) que afect&oacute; principalmente a la capital.<SUP>27-31    </SUP>Con posterioridad a 2004 se han reportado transmisiones a partir de casos    importados que han sido controladas y eliminadas. En 2006 se report&oacute;    una epidemia que tambi&eacute;n fue eliminada (<a href="http://www.paho.org/spanish/ad/dpc/cd/eid-eer-2006-oct-24.htm" target="_blank">http://www.paho.org/spanish/ad/dpc/cd/eid-eer-2006-oct-24.htm</a>).    En este contexto epidemiol&oacute;gico, las investigaciones realizadas han permitido    brindar nuevo conocimiento en el campo del dengue. Algunos de los principales    resultados se resumen a continuaci&oacute;n: </font>      <P><font face="Verdana" size="2">1. Los estudios virol&oacute;gicos, inmunol&oacute;gicos    y cl&iacute;nico-epidemiol&oacute;gicos realizados han permitido caracterizar    las epidemias de 1981, 1997 y 2001-2002. Han brindado datos de importancia para    su diagn&oacute;stico temprano, seguimiento y conocimiento de factores determinantes    en su control y eliminaci&oacute;n, as&iacute; como en el mejor conocimiento    del dengue. </font>     <P><font face="Verdana" size="2">2. La infecci&oacute;n secundaria por un serotipo    diferente al de la primoinfecci&oacute;n ha sido confirmada como factor de riesgo    principal en el desarrollo de DH. En el contexto de una infecci&oacute;n secundaria,    se han identificado secuencias virales de mayor riesgo de DH (Den 1/Den 2; Den    1/Den 3) y se ha observado la mayor producci&oacute;n de IL-10 en estos pacientes.    Estudios realizados demostraron las mayores tasas de DH en ni&ntilde;os comparados    con adultos con una infecci&oacute;n secundaria, as&iacute; como la identificaci&oacute;n    de casos de DH durante las infecciones terciarias. Finalmente, los estudios    sero-epidemiol&oacute;gicos han permitido conocer las tasas de casos de dengue    y DH seg&uacute;n tipo de infecci&oacute;n y serotipo para las condiciones de    Cuba.<SUP>13,20,30-35</SUP> </font>     <P><font face="Verdana" size="2">3. Se demostr&oacute; por primera vez el mayor    riesgo de DH en la medida en que el intervalo entre las infecciones es mayor.    Asociada a esta observaci&oacute;n se demostr&oacute; el incremento de la especificidad    de la respuesta humoral en el tiempo con disminuci&oacute;n de los anticuerpos    neutralizantes heter&oacute;logos y una respuesta de c&eacute;lulas T de memoria    de reactividad cruzada aun despu&eacute;s de 20 a&ntilde;os de la primoinfecci&oacute;n.<SUP>36-38</SUP>    </font>     <P><font face="Verdana" size="2">4. Identificaci&oacute;n de la raza blanca, el    asma bronquial, la diabetes mellitus y la anemia de c&eacute;lulas falciformes    como factores de riesgo de DH.<SUP>20,23</SUP> </font>     <P><font face="Verdana" size="2">5. Demostraci&oacute;n de la mayor asociaci&oacute;n    de DH en individuos europoides, con una respuesta de c&eacute;lulas T de memoria    m&aacute;s fuerte y de reactividad cruzada en blancos comparados con negros    inmunes a dengue y con mayor producci&oacute;n de IFN alfa en individuos blancos.<SUP>39-43</SUP>    </font>     <P><font face="Verdana" size="2">6. Demostraci&oacute;n del incremento y diferentes    niveles de expresi&oacute;n de quemokinas y citoquinas (CCL2, CCL3, TNFa, IFNg,    IL-10 y TGFb) en individuos con diferentes antecedentes inmunes de dengue, lo    cual sugiere que la inmunidad previa a un serotipo tiene una fuerte influencia    en la respuesta inmune temprana en la reinfecci&oacute;n.<SUP>34,35,44</SUP>    </font>     <P><font face="Verdana" size="2">7. Identificaci&oacute;n de genes de respuesta    inmune asociados a severidad (HLA clase I A*31,B*15, Fc g RIIa HH131, MICA*008    y MICB*008) o protecci&oacute;n (HLA clase II DRB1*04, DRB1*07, Fc g RIIa RR31).<SUP>41,45</SUP>    </font>     ]]></body>
<body><![CDATA[<P><font face="Verdana" size="2">8. Demostraci&oacute;n del papel de los virus    en la severidad de las epidemias cubanas con cepas y genotipos con capacidad    para el desarrollo de DH. Demostraci&oacute;n de un patr&oacute;n de evoluci&oacute;n    en las prote&iacute;nas no estructurales en cepas aisladas en diferentes momentos    de la epidemia.<SUP>46-52</SUP> </font>     <P><font face="Verdana" size="2">9. Incremento de la severidad de la epidemia    en la medida en que transcurre en el tiempo con posible selecci&oacute;n de    variantes virales de mayor virulencia, ya sea por escape a la neutralizaci&oacute;n    (hip&oacute;tesis de mutantes de escape) o por evoluci&oacute;n viral.<SUP>20,23,49,53</SUP>    </font>     <P><font face="Verdana" size="2">10. Hip&oacute;tesis integral que explica el    desarrollo de epidemias de DH, en los cuales las caracter&iacute;sticas del    hospedero, el virus y las condiciones epidemiol&oacute;gicas desempe&ntilde;an    un papel fundamental.<SUP>8,23,54</SUP> </font>     <P><font face="Verdana" size="2">11. Las investigaciones dirigidas al mejor conocimiento    cl&iacute;nico de la enfermedad han permitido caracterizarla, conocer sus complicaciones,    reconocer y validar los signos de alarma cl&iacute;nicos y demostrar las secuelas    cl&iacute;nicas de la infecci&oacute;n por dengue aun 1 a&ntilde;o despu&eacute;s    de la infecci&oacute;n, as&iacute; como su posible relaci&oacute;n a un fen&oacute;meno    de autoinmunidad.<SUP>9,21,55-61</SUP> </font>     <P><font face="Verdana" size="2">12. Los estudios dirigidos al desarrollo e implementaci&oacute;n    de m&eacute;todos de diagn&oacute;stico, la caracterizaci&oacute;n de la respuesta    humoral en pacientes con infecci&oacute;n primaria o secundaria, y el estudio    de la utilidad diagn&oacute;stica de diferentes muestras cl&iacute;nicas, han    sido de importancia en la vigilancia de laboratorio (serol&oacute;gica, virol&oacute;gica    y molecular) y en el mejor conocimiento de la enfermedad.<SUP>62-73</SUP> </font>     <P><font face="Verdana" size="2">13. Se han producido reactivos biol&oacute;gicos    como anticuerpos monoclonales, estuches diagn&oacute;sticos para la determinaci&oacute;n    de anticuerpos IgM a dengue mediante el sistema ultramicroanal&iacute;tico (SUMA),    entre otros, que han contribuido al diagn&oacute;stico serol&oacute;gico y a    un mejor conocimiento de este agente.<SUP>74-76</SUP> </font>      <P><font face="Verdana" size="2">14. Se ha demostrado adem&aacute;s, que la sostenibilidad    de las acciones en la lucha contra <I>Aedes aegypti</I>, solo es posible a trav&eacute;s    de una acci&oacute;n consciente, sistem&aacute;tica, promotora y preventiva    de las personas y los grupos, lo cual en el contexto cubano se ha dado en llamar    &quot;autofocal&quot; y que se refiere al &quot;conjunto de acciones encaminadas    a detectar y eliminar posibles criaderos de mosquitos. Tiene como eje central    la autoinspecci&oacute;n que deben realizar las familias y los colectivos laborales    en sus viviendas y centros, con periodicidad semanal&quot;. </font>      <P><font face="Verdana" size="2">15. En este per&iacute;odo se han realizado investigaciones    para la mejor implementaci&oacute;n de la vigilancia integrada, la institucionalizaci&oacute;n    de las estrategias comunitarias en la lucha contra el dengue y la evaluaci&oacute;n    de la intervenci&oacute;n de la comunidad, as&iacute; como el estudio de nuevos    m&eacute;todos de control.<SUP>77-82</SUP> </font>     <P><font face="Verdana" size="2">16. La estimaci&oacute;n de los par&aacute;metros    epidemiol&oacute;gicos de inter&eacute;s durante las epidemias, la estacionalidad    de las poblaciones de <I>Ae. aegypti</I> y <I>Ae. albopictus, </I>las investigaciones    dirigidas a conocer el peso entomol&oacute;gico para la transmisi&oacute;n de    dengue, el desarrollo de m&eacute;todos predictivos de la severidad de dengue,    el desarrollo de modelos para el an&aacute;lisis de las condiciones sociales    que favorecen la transmisi&oacute;n, as&iacute; como el desarrollo de modelos    para medir la evoluci&oacute;n de la infestaci&oacute;n<I> </I>de <I>Ae. aegypti    </I>y del resultado de las intervenciones, han formado parte de las principales    investigaciones realizadas en este per&iacute;odo.<SUP>78,79,83-89</SUP> </font>     <P><font face="Verdana" size="2">17. Las investigaciones dirigidas a conocer el    impacto econ&oacute;mico del dengue han permitido conocer el costo en per&iacute;odos    de transmisi&oacute;n y de no transmisi&oacute;n, el costo de los programas    de prevenci&oacute;n, del control del caso cl&iacute;nico, los costos directos    e indirectos para la familia, el costo/efectividad de las estrategias de participaci&oacute;n    de la comunidad y el costo/efectividad de las acciones vectoriales para el control    de <I>Ae. aegypti</I>.<SUP>90-93</SUP> </font>     ]]></body>
<body><![CDATA[<P><font face="Verdana" size="2">18. El desarrollo de un candidato vacunal cubano    para el control del dengue constituye una de las principales l&iacute;neas de    investigaci&oacute;n del pa&iacute;s. Como parte de estas investigaciones se    estableci&oacute; un modelo para la obtenci&oacute;n de vacunas atenuadas, se    caracteriz&oacute; la respuesta inmune humoral y celular inducida por las prote&iacute;nas    de membrana y su precursor, se estableci&oacute; el modelo de monos para la    evaluaci&oacute;n de candidatos vacunales de dengue, se evalu&oacute; la capacidad    inmunog&eacute;nica y protectora de la prote&iacute;na de envoltura del virus    Den 4 expresada en <I>Pichia pastoris</I>. Hoy se cuenta con 2 candidatos vacunales    basados en una estrategia recombinante (dominio III de la prote&iacute;na de    envoltura de virus dengue y capside/dominio III) que han brindado resultados    satisfactorios en los estudios precl&iacute;nicos.<SUP>74,94-121</SUP> </font>     <P>      <P><font face="Verdana" size="2"><B><font size="3">COLABORACI&Oacute;N INTERNACIONAL</font></B>    </font>      <P><font face="Verdana" size="2">En este per&iacute;odo, el pa&iacute;s ha mantenido    una fuerte actividad de colaboraci&oacute;n con pa&iacute;ses de esta y otras    regiones geogr&aacute;ficas, as&iacute; como con las organizaciones internacionales    m&aacute;s involucradas en el estudio del dengue (Organizaci&oacute;n Panamericana    de la Salud [OPS], Organizaci&oacute;n Mundial de la Salud [OMS], Programa Mundial    para la Investigaci&oacute;n y Fortalecimiento en Enfermedades Tropicales [TDR],    Iniciativa para una Vacuna para Dengue [DVI], y otros). Hay varios elementos    que han permitido por una parte que el pa&iacute;s aporte sus conocimientos    y capacidades para el mejor control del dengue en otros pa&iacute;ses y, por    otra parte, tener un mejor conocimiento de la situaci&oacute;n mundial y de    los aportes de las estrategias e investigaciones que otros pa&iacute;ses y organizaciones    internacionales est&aacute;n realizando para el enfrentamiento del dengue; estos    elementos son los siguientes: la participaci&oacute;n en el diagn&oacute;stico,    manejo cl&iacute;nico de pacientes, enfrentamiento y control de epidemias, las    evaluaciones de los programas de control en pa&iacute;ses de la regi&oacute;n,    la coordinaci&oacute;n de las pruebas de control de calidad, la introducci&oacute;n    de metodolog&iacute;as diagn&oacute;sticas y de evaluaci&oacute;n de la resistencia    a insecticidas, la capacitaci&oacute;n del personal en las diferentes disciplinas    relacionadas con el dengue, la introducci&oacute;n y evaluaci&oacute;n de la    nueva clasificaci&oacute;n cl&iacute;nica de dengue, la participaci&oacute;n    en la implementaci&oacute;n en la regi&oacute;n de la estrategia de gesti&oacute;n    integrada (EGI) que promueve la OPS, la participaci&oacute;n en grupos de expertos    internacionales (OPS, OMS, TDR, DVI entre otras), la participaci&oacute;n en    proyectos de investigaci&oacute;n multic&eacute;ntricos y la colaboraci&oacute;n    con instituciones nacionales e internacionales de gran nivel cient&iacute;fico.</font>      <P>      <P><font face="Verdana" size="2"><B><font size="3">CONCLUSIONES</font></B> </font>      <P><font face="Verdana" size="2">En estos 30 a&ntilde;os, a diferencia de la mayor&iacute;a    de los pa&iacute;ses de la regi&oacute;n, Cuba ha sido capaz de mantenerse libre    de dengue como una enfermedad end&eacute;mica. Las epidemias y transmisiones    ocurridas han sido interrumpidas. El pa&iacute;s ha seguido los 5 principios    b&aacute;sicos que la OMS/OPS recomiendan para el enfrentamiento del dengue    siendo la voluntad pol&iacute;tica el pilar fundamental y promotor del &eacute;xito    obtenido. No obstante, la creciente complejidad de la situaci&oacute;n epidemiol&oacute;gica    del dengue en la regi&oacute;n y el amplio y creciente intercambio que Cuba    mantiene con pa&iacute;ses end&eacute;micos incrementan el riesgo. En este contexto,    el pa&iacute;s trabaja en el fortalecimiento y mejoramiento de sus programas.    La aplicaci&oacute;n de los resultados cient&iacute;ficos obtenidos y la continua    y creciente colaboraci&oacute;n internacional deben desempe&ntilde;ar un papel    fundamental para el mejor enfrentamiento del dengue. </font>     <P>      <P>      <P><font face="Verdana" size="3"><B>DEDICATORIA</B> </font>      ]]></body>
