El <em>Helicobacter pylori</em> es una bacteria gramnegativa, agente etiol&oacute;gico  de la infecci&oacute;n cr&oacute;nica m&aacute;s frecuente en los humanos. Al parecer este germen  tiene una notable influencia sobre el metabolismo del hierro y en la actualidad  existen muchas evidencias de su relaci&oacute;n directa con la anemia ferrop&eacute;nica. <body><![CDATA[<div align="justify">El <em>Helicobacter pylori</em> (HP) es una bacteria  gramnegativa con forma de espiral que mide aproximadamente 3,5 x 0,5 micrones.<span class="superscript">2</span> En relaci&oacute;n con el metabolismo del hierro, parece tener una  influencia notable y se plantea que su presencia aumenta la prevalencia del  d&eacute;ficit del mineral hasta en el 40 %.<span class="superscript">3</span> </div>     <p align="justify">En la actualidad existen muchas evidencias de la  relaci&oacute;n directa de este germen con la anemia ferrop&eacute;nica.<span class="superscript">4-18</span></p> <h4>Infecci&oacute;n por HP</h4>     <p>Esta bacteria  presenta una serie de adaptaciones que le permiten sobrevivir y proliferar en  el est&oacute;mago humano:</p> <ul>       <li>         <div align="justify">La ureasa bacteriana  hidroliza la urea luminal para formar amonio y bicarbonato que neutraliza el  &aacute;cido g&aacute;strico, forma una envoltura productora y le permite penetrar en la capa  de <em>mucus</em> g&aacute;strico.<span class="superscript">19</span></div>   </li>       <li>         <div align="justify">La forma espiral, los  flagelos y las enzimas mucol&iacute;ticas que produce, le facilitan atravesar la capa  de mucus.<span class="superscript">2</span></div>   </li>       <li>         <div align="justify">Su uni&oacute;n al epitelio  depende de una interacci&oacute;n de la bacteria con receptores espec&iacute;ficos en la  superficie celular que pueden expresarse de manera diferente en los individuos  y hacerlos m&aacute;s o menos susceptibles a la infecci&oacute;n.<span class="superscript">20-22 </span></div>   </li>     </ul>     ]]></body>
<body><![CDATA[<p align="justify">Es la infecci&oacute;n  cr&oacute;nica m&aacute;s frecuente en los humanos y se ha se&ntilde;alado que solo es superada en  n&uacute;mero por las caries dentales,<span class="superscript">23-25 </span>Se ha demostrado su presencia en todas partes  del mundo y en todas las edades.</p>     <p align="justify">Estimados  conservadores indican que el 50 % de la poblaci&oacute;n mundial est&aacute; afectada por  esta infecci&oacute;n, que en los pa&iacute;ses en desarrollo es m&aacute;s frecuente a edades  tempranas, lo que se relaciona con factores socioecon&oacute;micos, al contrario de lo  que sucede en los pa&iacute;ses desarrollados, donde predomina en los adultos.<span class="superscript">24</span></p>     <p align="justify">Aunque la forma de  transmisi&oacute;n permanece desconocida, las v&iacute;as persona a persona, fecal-oral y  oral-oral parecen ser las m&aacute;s probables. A pesar de extensas investigaciones,  no se ha podido encontrar ning&uacute;n reservorio en el ambiente que no sea el  est&oacute;mago humano.<span class="superscript">23,26</span></p>     <p align="justify">El agua contaminada  constituye una fuente de contagio y los ni&ntilde;os que nadan en piscinas, r&iacute;os o  ingieren alimentos contaminados est&aacute;n m&aacute;s expuestos a la infecci&oacute;n.<span class="superscript">27</span>  Las secreciones g&aacute;stricas infectadas tambi&eacute;n son fuente de infecci&oacute;n y se han  documentado infecciones iatrog&eacute;nicas mediante el uso de accesorios endosc&oacute;picos.<span class="superscript">28,29</span> De los individuos afectados, menos del 25 % desarrolla  s&iacute;ntomas cl&iacute;nicos. Esto puede deberse al grado de exposici&oacute;n al germen, a la  susceptibilidad gen&eacute;tica individual a la infecci&oacute;n y&nbsp; a la virulencia de la cepa. </p>     <p>Actualmente hay  varias formas de diagnosticar la infecci&oacute;n por HP:<span class="superscript">30</span></p> <ul>       <li>         <div align="justify">Endoscopia con toma de muestras de la mucosa g&aacute;strica para la  observaci&oacute;n directa de la bacteria.</div>   </li>       <li>         <div align="justify">Estudio del aire exhalado o prueba de la ureasa que detecta la  infecci&oacute;n por HP activa con una sensibilidad del 90 al 100 %, por lo que  constituye el examen de elecci&oacute;n.</div>   </li>       <li>         ]]></body>
<body><![CDATA[<div align="justify">Detecci&oacute;n de los anticuerpos que el organismo produce contra esta  bacteria.</div>   </li>       <li>         <div align="justify">Detecci&oacute;n por ELISA de la bacteria en muestras de heces fecales.</div>   </li>     </ul> <h4>Asociaci&oacute;n de la  infecci&oacute;n por HP con otras enfermedades </h4>     <p align="justify">Adem&aacute;s de la  deficiencia de hierro, la infecci&oacute;n se ha asociado con otras condiciones, tales  como la &uacute;lcera p&eacute;ptica, la gastritis cr&oacute;nica atr&oacute;fica, el linfoma asociado con  la mucosa g&aacute;strica, el c&aacute;ncer g&aacute;strico, la dispepsia funcional, la p&uacute;rpura  trombocitop&eacute;nica inmunol&oacute;gica, el s&iacute;ndrome hemol&iacute;tico ur&eacute;mico, la p&uacute;rpura  trombocitop&eacute;nica tromb&oacute;tica, la urticaria cr&oacute;nica, la cardiopat&iacute;a isqu&eacute;mica y  la anemia megalob&aacute;stica.<span class="superscript">31-33</span></p> <h4>Papel del HP en la etiolog&iacute;a de la anemia  ferrop&eacute;nica </h4> Los&nbsp;  mecanismos por los cuales el HP es capaz de interferir con el  metabolismo del hierro son: <ul>       <li>         <div align="justify">La  disminuci&oacute;n de la secreci&oacute;n &aacute;cida e incremento del PH intrag&aacute;strico como  resultado de una gastritis infecciosa. En individuos con anemia ferrop&eacute;nica y  HP se ha encontrado que el pH g&aacute;strico es superior que en los an&eacute;micos sin  infecci&oacute;n.<span class="superscript">22,34,35</span></div>   </li>       <li>         <div align="justify">Disminuci&oacute;n  de la concentraci&oacute;n de &aacute;cido asc&oacute;rbico en el jugo g&aacute;strico. </div>         <p align="justify">Cuando se comparan las concentraciones del  &aacute;cido asc&oacute;rbico intrag&aacute;strico de grupos de individuos infectados y no infectados,  se encuentra una disminuci&oacute;n de aproximadamente 50 % de los valores de &aacute;cido  asc&oacute;rbico en los infectados en relaci&oacute;n con los obtenidos en los individuos no  infectados.<span class="superscript">36,37</span></p>   </li>     ]]></body>
<body><![CDATA[</ul>     <div align="justify">   <ul>         <li>Incremento  en la producci&oacute;n hep&aacute;tica de la hepcidina como respuesta al incremento de la  interleucina 6 inducida a su vez por la gastritis.<span class="superscript">38</span> Esta prote&iacute;na  es producida exclusivamente en el h&iacute;gado y secretada a la circulaci&oacute;n en  respuesta a la inflamaci&oacute;n o al aumento de las reservas de hierro.&nbsp;Su sobre exposici&oacute;n conduce a la anemia.&nbsp; Dentro de la homeostasis del hierro, se  plantea que la hepcidina es un regulador negativo de la absorci&oacute;n intestinal  del mineral y de la liberaci&oacute;n del hierro por los macr&oacute;fagos.<span class="superscript">39</span></li>         <li>Utilizaci&oacute;n  del hierro por el microorganismo para su crecimiento.<span class="superscript">40</span></li>         <li>Secuestro  del hierro en la lactobacteria de la mucosa g&aacute;strica.&nbsp; Esta prote&iacute;na sirve como fuente de hierro  para ser utilizado por el HP ya que &eacute;l posee receptores espec&iacute;ficos para la  lactoferrina sobre la superficie celular.<span class="superscript">41-43</span></li>       </ul> </div> <ul>     </ul> <h4>Consideraciones terap&eacute;uticas </h4>     <p align="justify">En la actualidad se considera que  la investigaci&oacute;n del HP en individuos sin s&iacute;ntomas con el objeto de prevenir  una enfermedad ulcerosa o una eventual enfermedad neopl&aacute;sica, no est&aacute;  plenamente justificada.</p>     <p align="justify">Existe todav&iacute;a controversia sobre  qui&eacute;nes deben ser tratados y qui&eacute;nes no. Algunos expertos consideran que deben  ser tratados todos los individuos con una prueba positiva para HP, mientras que  otros plantean que deben tratarse solo aquellos con una enfermedad ulcerosa,  tanto presente como pasada.</p>     <p align="justify">El Grupo Europeo de Estudio del  HP recomienda la erradicaci&oacute;n de la infecci&oacute;n en casos de anemia ferrop&eacute;nica de  causa no precisada, linfoma MALT, gastritis atr&oacute;fica, familiares de primer  grado de consanguinidad de pacientes con c&aacute;ncer g&aacute;strico, PTI cr&oacute;nica y &uacute;lcera  p&eacute;ptica.<span class="superscript">44</span></p>     ]]></body>
<body><![CDATA[<p align="justify">La anemia ferrop&eacute;nica refractaria  de etiolog&iacute;a no precisada, recurrente o sencillamente concomitante con una  infecci&oacute;n por HP, se beneficia con su erradicaci&oacute;n, a tal punto que existen  reportes de casos con anemias resueltas solamente con el tratamiento  antibi&oacute;tico.<span class="superscript">16,17</span></p>     <p>De todo lo anterior puede  apreciarse la actualidad del tema y la importancia que tiene desde el punto de  vista pr&aacute;ctico en la asistencia m&eacute;dica.</p> <h4>Referencias  bibliogr&aacute;fias</h4>     <div align="justify">        <p>1.
