<abstract abstract-type="short" xml:lang="en"><p><![CDATA[Some recent publications on the topic of diabetic foot that show the magnitude of the problem from a social, medical and family point of view were reviewed. The main mechanisms involved in the pathogeny: peripheral neuropathy, macroangiopathy and microangiopathy, sepsis and foot deformities; as well as the most important elements to be taken into account by the health personne to prevent amputations were explained. It was concluded that diabetes education as a way of prevention from the primary health care level is the milestone to attain a 50 % reduction of the amputations related to diabetes, as it was agreed upon in San Vicente.]]></p></abstract>

</front><body><![CDATA[ <p>Hospital General "Ciro Redondo Garc&iacute;a" <h2> El pie del diab&eacute;tico</h2><i>Dr.  Mayque Guzm&aacute;n Cayado</i> <h4> RESUMEN</h4>Se revisaron algunas publicaciones  recientes sobre el tema que ponen de manifiesto la magnitud del problema desde  un enfoque social, m&eacute;dico y familiar. Se expusieron los principales mecanismos  implicados en la patogenia: neuropat&iacute;a perif&eacute;rica, macroangiopat&iacute;a  y microangiopat&iacute;a, sepsis y deformidades pod&aacute;licas, as&iacute; como  los principales elementos a tener en cuenta por parte del personal de salud para  la prevenci&oacute;n de las amputaciones. Se concluy&oacute; que la educaci&oacute;n  diabetol&oacute;gica, como v&iacute;a de prevenci&oacute;n desde la atenci&oacute;n  primaria, constituye la piedra angular para el logro de los acuerdos de San Vicente  de reducir en 50 % las amputaciones relacionadas con la diabetes.     <p><i>Descriptores  DeCS: </i>PIE DIABETICO/prevenci&oacute;n &amp; control EDUCACION DEL PACIENTE;  ATENCION PRIMARIA DE SALUD. <h4> Magnitud del problema</h4>En las Am&eacute;ricas  viven alrededor de 30 000 000 de diab&eacute;ticos seg&uacute;n datos de la Organizaci&oacute;n  Panamericana de la Salud (OPS).<sup>1</sup> En Cuba, seg&uacute;n datos de dispensarizaci&oacute;n  del a&ntilde;o 1996, la prevalencia de la enfermedad fue de 1,9 %,<sup>2</sup>  lo que indica que el n&uacute;mero de pacientes sobrepasa los 200 000, m&aacute;s  a&uacute;n si se considera el subregistro que existe de la enfermedad.     <p>En Virginia  (EE.UU.), en un estudio retrospectivo de 14 a&ntilde;os, las &uacute;lceras de  los miembros inferiores constituyeron 14 % de todos los ingresos por diabetes  y 14 % de los pacientes fueron amputados.<sup>3</sup> Los pacientes diab&eacute;ticos  con complicaciones de los miembros inferiores son los que con mayor frecuencia  ocupan los servicios de cirug&iacute;a general.<sup>4,5</sup>     <p>La amputaci&oacute;n  menor o mayor constituye el desenlace m&aacute;s desagradable y temido por el  paciente diab&eacute;tico, su costo afecta emocional y econ&oacute;micamente al  paciente, sus familiares, los m&eacute;dicos de asistencia y a la sociedad.<sup>6,8</sup>      <p>Se ha descrito que la diabetes incrementa en 40 veces el riesgo de amputaci&oacute;n  al ser comparado con sujetos no diab&eacute;ticos.<sup>9</sup> El costo de los  ingresos por amputaciones en el estado de California en los EE.UU., en 1991, fue  de 141 000 000 de d&oacute;lares (27 930 por paciente) y la estad&iacute;a media  15,9 d; result&oacute; mayor en los pacientes sometidos a amputaciones m&uacute;ltiples  (44 731 d&oacute;lares por paciente y 23,4 d).<sup>7</sup> En Australia la estad&iacute;a  media por motivo de amputaciones en diab&eacute;ticos entre 1992 y 1994 fue de  48 d.<sup>10</sup> El costo promedio por una amputaci&oacute;n en los EE.UU. en  1992 fue de 11 000 y 27 000 d&oacute;lares por paciente en el programa <i>Medicare</i>  y en las cl&iacute;nicas privadas, respectivamente.<sup>11</sup>     <p>La tasa de  supervivencia fue baja (apenas 40 % a los 5 a&ntilde;os) despu&eacute;s de una  am-putaci&oacute;n, resultaron predictores negativos la edad del paciente y las  amputaciones m&uacute;ltiples.<sup>12-15</sup> <h4> Mecanismos fisiopatog&eacute;nicos</h4>Cuando  se hace referencia al pie diab&eacute;tico (PD) debemos incluir en este concepto  una serie de alteraciones: neurol&oacute;gicas, vasculares, infecciosas y &oacute;seas  que a pesar de su origen variado convergen en los miembros inferiores y pueden  ocasionar su amputaci&oacute;n. <h4> Neuropat&iacute;a</h4>Se ha observado que  existe una asociaci&oacute;n significativa entre la percepci&oacute;n vibratoria  (Raz&oacute;n de ventaja [RV]= 4,38), el examen cl&iacute;nico alterado (RV =  2,3) y la hemoglobina glicosilada (HbA1C) (RV = 1,30) y el desarrollo posterior  de complicaciones en miembros inferiores.<sup>16</sup> En un estudio realizado  en Inglaterra (1998) se detect&oacute; que 42 % de los diab&eacute;ticos tipo  2 presentaban evidencias cl&iacute;nicas de afectaci&oacute;n neurol&oacute;gica  perif&eacute;rica.<sup>17</sup>     <p>La toma auton&oacute;mica predispone a la anhidrosis  y sequedad secundaria lo que favorece la formaci&oacute;n de fisuras y la infecci&oacute;n.  Se ha descrito incluso asociaci&oacute;n entre la presencia de neuropat&iacute;a  auton&oacute;mica cardiovascular y la aparici&oacute;n de &uacute;lceras en los  miembros inferiores.<sup>18</sup> La polineuro-pat&iacute;a sim&eacute;trica distal,  en sus formas sensitiva y motora, es otro elemento clave en la g&eacute;nesis  del pie diab&eacute;tico. La p&eacute;rdida de la sensibilidad condiciona que  el paciente no se percate de peque&ntilde;os traumas mec&aacute;nicos, qu&iacute;micos  o t&eacute;rmicos que da&ntilde;an la integridad de la piel y pueden convertirse  en puertas de entrada para las infecciones. Hace poco se ha descrito que hasta  el color del calzado puede influir en los traumas t&eacute;rmicos.<sup>19</sup>      <p>Aunque la diabetes mellitus (DM) es una enfermedad bioqu&iacute;mica tiene  consecuencias biomec&aacute;nicas en las extremidades inferiores. La neuroartropat&iacute;a  de Charcot se debe, en la mayor&iacute;a de las ocasiones, a la DM y predispone  a las ulceraciones e infecciones.<sup>20</sup> La toma motora provoca atrofia  muscular lo que altera la marcha y provoca deformidades en el pie, con aparici&oacute;n  de nuevos puntos de presi&oacute;n plantar y el riesgo de ulceraciones. <h4> Macroangiopat&iacute;a</h4>Junto  a los lechos vasculares cerebral y card&iacute;aco, la circulaci&oacute;n hacia  los miembros inferiores es tambi&eacute;n afectada en los pacientes con DM. La  enfermedad constituye un elemento clave en la aterog&eacute;nesis, toda vez que  la glicoxidaci&oacute;n de las lipoprote&iacute;nas, el hiperinsulinismo y las  alteraciones hemorreol&oacute;gicas favorecen la formaci&oacute;n de la placa  de ateroma.     <p>La ausencia de una irrigaci&oacute;n eficiente hacia los miembros  inferiores impide que los mecanismos de defensa ante infecciones funcionen, retarda  la cicatrizaci&oacute;n de las heridas e interfiere en que los antimicrobianos  puedan llegar al &aacute;rea infectada. De esta forma, desencadenan, la gangrena  y la amputaci&oacute;n subsecuente.     <p>El l&aacute;ser y la radioterapia antiinflamatoria  han probado ser &uacute;tiles en la reducci&oacute;n de las amputaciones al mejorar  la microcirculaci&oacute;n.<sup>21,22</sup> La revascularizaci&oacute;n aut&oacute;gena  constituye un proceder que, aplicado precozmente en los casos en que se constate  la isquemia o cuando el tratamiento conservador no surte efecto, disminuye la  incidencia de las amputaciones.<sup>23-25</sup> <h4> SEPSIS</h4><i>Wahid</i> y  otros, 1988, al buscar lesiones dermatol&oacute;gicas en diab&eacute;ticos encontraron  que 49 % presentaban infecciones y 30 % de &eacute;stas inclu&iacute;an los pies.<sup>26</sup>  La DM predispone a las infecciones por varios mecanismos: metab&oacute;licos (hiperglicemia,  deshidrataci&oacute;n por diuresis osm&oacute;tica, pobre estado nutricional,  cetoacidosis), factores de defensa del hu&eacute;sped (funci&oacute;n alterada  de los leucocitos polimorfonucleares -quimiotaxis, fagocitosis y actividad bactericida-,  disminuci&oacute;n en la producci&oacute;n de radicales super&oacute;xido) y otros  (microangiopat&iacute;a, macroangiopat&iacute;a, y neuropat&iacute;a).<sup>27</sup>      ]]></body>
<body><![CDATA[<p>La presencia de potenciales pat&oacute;genos como el <i>Staphylococcus aureus</i>  se encuentra incrementada en diab&eacute;ticos tipo 1, lo que aumenta la susceptibilidad  a infecciones locales posteriores a traumas menores de la piel. Este microorganismo  ha sido el m&aacute;s com&uacute;nmente aislado en las osteomielitis, seguido  por otros cocos grampositivos aerobios, anaerobios y bacilos gramnegativos.<sup>28</sup>  El estreptococo, no del grupo A, tambi&eacute;n ha aumentado su incidencia en  las infecciones necrotizantes.<sup>29</sup>     <p>La afectaci&oacute;n &oacute;sea  por la sepsis (osteomielitis) es un evento grave que anuncia la amputaci&oacute;n.<sup>30</sup>  En centros especializados se usan medios diagn&oacute;sticos sofisticados en su  detecci&oacute;n como la gammagraf&iacute;a con el uso de leucocitos marcados  con Indio 111 y la RMN.<sup>28,31</sup> Una vez hecho el diagn&oacute;stico de  la osteomielitis no deben hacerse esperar las correcciones metab&oacute;licas,  la antibioticoterapia, la debridaci&oacute;n quir&uacute;rgica precoz y en&eacute;rgica  o la resecci&oacute;n del hueso infectado.<sup>28,32-38</sup> <h4> Deformidades  pod&aacute;licas</h4>&nbsp;Diversas alteraciones en el pie (<i>hallux valgus</i>,  dedos en martillo, en garra, en maza, subluxaciones de dedos, pie equino, pie  varo, valgo o sus combinaciones) as&iacute; como los trastornos de la marcha condicionan  la aparici&oacute;n de nuevos puntos de presi&oacute;n en el pie, por dem&aacute;s  desprotegido, y ocasionan la aparici&oacute;n de &uacute;lceras. Esto ha determinado  que estas alteraciones sean consideradas, junto a la neuropat&iacute;a y a la  macroangiopat&iacute;a de miembros inferiores, un factor de mayor riesgo de amputaci&oacute;n.<sup>37</sup>      <p>La correcci&oacute;n quir&uacute;rgica de las deformidades &oacute;seas del  pie es una alternativa viable en aquellos casos que no puedan ser sometidos a  una amputaci&oacute;n.<sup>38</sup> <h4> Consideraciones finales</h4>A pesar de  los recientes avances en el tratamiento del PD en los &uacute;ltimos a&ntilde;os,  no ha sucedido lo mismo con su prevenci&oacute;n. La concientizaci&oacute;n de  este fen&oacute;meno por parte de los proveedores de salud en la atenci&oacute;n  primaria constituye un elemento vital. El pie del diab&eacute;tico ha de ser enfocado  desde una visi&oacute;n multidisciplinaria que agrupe a una serie de trabajadores  de la salud: endocrin&oacute;logos, internistas, orto-p&eacute;dicos, angi&oacute;logos,  cirujanos, quiropedistas y enfermeras, con una meta com&uacute;n: evitar la amputaci&oacute;n.      <p>El conocimiento de los elementos implicados en su patogenia debe sobrepasar  todo obst&aacute;culo que impliquen las clasificaciones, muchas veces divergentes  y carentes de equivalencia.     <p>Resulta de vital importancia el diagn&oacute;stico  y tratamiento precoces de los casos con DM del tipo 2, pues muchos de ellos ya  habr&aacute;n transitado a&ntilde;os de hiperglicemia previo al diagn&oacute;stico,  lo que pudiera constituir un freno para el cumplimiento de los acuerdos de la  declaraci&oacute;n de San Vicente de reducir, en 50 % o m&aacute;s, las amputaciones  relacionadas con la DM.     <p>La educaci&oacute;n del paciente diab&eacute;tico,  el control metab&oacute;lico, la higiene de los pies, el cuidado de las u&ntilde;as,  el uso de un calzado adecuado, la correcci&oacute;n de las deformidades pod&aacute;licas  y las callosidades, la supresi&oacute;n del alcoholismo y el h&aacute;bito de  fumar, as&iacute; como el diagn&oacute;stico y tratamiento precoces de la neuropat&iacute;a,  las dislipidemias y la vasculopat&iacute;a perif&eacute;rica constituyen la piedra  angular en la prevenci&oacute;n de algo tan desagradable como una amputaci&oacute;n.  <h4> SUMMARY</h4>Some recent publications on the topic of diabetic foot that show  the magnitude of the problem from a social, medical and family point of view were  reviewed.
