Los pacientes fueron divididos en 3 grupos de acuerdo con: reciente captación, tratamiento solo con dieta o con dieta más hipoglicemiantes orales. La seroprevalencia de anticuerpos anti-H. pylori en los grupos de pacientes diabéticos fue de 90, 94, y 77 % respectivamente, en relación con el valor de 58 % del grupo control. La infección por H. pylori estuvo altamente asociada con la diabetes mellitus en los pacientes de reciente captación o en los tratados solo con dieta (p= 0,003, p= 0,01).]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[The seroprevalence of IgG antibodies against Helicobacter pylori detected by immunoassay was determined in a case-control study that included 59 patients with diabetes mellitus and 19 healthy individuals from Havana City that were incorporated as a control group. Patients were divided into 3 groups according to recent recruitment, treatment only with diet, or with diet plus hypoglycemic agents. The seroprevalence of anti- Helicobacter pylori antibodies in the groups of diabetic patients was 90, 94 and 77 %, respectively, in relation to the value of 58 % of the control group. Helicobacter pylori infection was markedly associated with diabetes mellitus in recently recruited patients with diabetes mellitus, or in those treated with diet only (p= 0.03, p= 0.01).]]></p></abstract>

A esta bacteria se le ha adjudicado un posible papel patogénico en diversas      manifestaciones <span class=SpellE>extragástricas</span>: <span style='color:black'>vasculares (aterosclerosis y enfermedad <span class=SpellE>isquémica</span>      del corazón, fenómeno de <span class=SpellE>Raynaud</span>, migraña primaria),      <span class=SpellE>autoimmunes</span> (síndrome de <span class=SpellE>Sjogren</span>,      púrpura <span class=SpellE>Henoch</span>-<span class=SpellE>Schonlein</span>,      tiroiditis <span class=SpellE>autoimmune</span>, arritmias <span class=SpellE>idiopáticas</span>,      enfermedad de Parkinson, neuropatía óptica <span class=SpellE>isquémica</span>      no arterial) y cutáneas (urticaria crónica <span class=SpellE>idiopática</span>,      rosácea, alopecia <span class=SpellE>areata</span>), anemia <span class=SpellE>sideropénica</span>,      retardo del crecimiento, menarquia tardía, linfoma de MALT <span class=SpellE>extragástrico</span>,      anorexia del envejecimiento, encefalopatía hepática, síndrome de la muerte      súbita del lactante y diabetes <span class=SpellE>mellitus</span>.</span><span style='mso-ansi-language:ES-MX'><span class=GramE><sup>1</sup></span></span></span><span lang=EN-GB style='color:black;mso-ansi-language:ES-MX'> </span></p>       <p class=MsoNormal><span style='color:black;mso-ansi-language:ES-MX'>Los pacientes      afectados de diabetes <span class=SpellE>mellitus</span> son más susceptibles      a infecciones. Junto a ello la peculiaridad de la <span class=SpellE>gastroparesis</span> del diabético, puede conducir adicionalmente      al crecimiento bacteriano exacerbado en el tracto gastrointestinal superior.      </span></p>       <p class=MsoNormal><span style='mso-ansi-language:ES-MX'>En los pacientes afectados      de diabetes <span class=SpellE>mellitus</span>, las infecciones crónicas son      frecuentes y severas como consecuencia de desajustes del sistema inmunológico.      Sin embargo, los datos sobre la <span class=SpellE>prevalencia</span> de infección      por <i style='mso-bidi-font-style:normal'>H. <span class=SpellE>pylori</span>      </i>en pacientes con diabetes <span class=SpellE>mellitus</span> son escasos      y contradictorios. Numerosos estudios informan sobre un nivel de infección      elevado<span class=GramE>,<sup>2</sup></span><sup>-8</sup> mientras que por      otra parte, otros estudios no informan relación alguna en personas afectadas      por diabetes tipo 1 o 2.<sup>9-18</sup></span></p>       <p class=MsoNormal><span style='color:black;mso-ansi-language:ES-MX'>La comprensión      de las relaciones entre la infección por <i style='mso-bidi-font-style: normal'>H. <span class=SpellE>pylori</span></i> y la diabetes <span class=SpellE>mellitus</span> impresionan como complicadas y contradictorias. La      <span class=SpellE>prevalencia</span> de gastritis por <i style='mso-bidi-font-style: normal'>H. <span class=SpellE>pylori</span> </i>es alta en pacientes diabéticos      <span class=SpellE>asintomáticos</span>, comparados con personas aparentemente      sanas.</span><sup><span lang=ES style='color:black;mso-ansi-language:ES'>19      </span></sup><span style='color:black;mso-ansi-language:ES-MX'>Algunos de los cambios generados por      la infección por <i style='mso-bidi-font-style:normal'>H. <span class=SpellE>pylori</span> </i>pueden afectar los niveles de glucosa en ayunas      de la población. Por otra parte, las modificaciones del metabolismo de la      glucosa en los diabéticos han sido propuestas como factores favorecedores      de la colonización por esta bacteria. La erradicación de la infección en pacientes      con diabetes tipo 1 se asocia con un mejor control de los niveles de glicemia.<sup>20      </sup>Niños con diabetes tipo 1 y colonizados por <i style='mso-bidi-font-style: normal'>H. <span class=SpellE>pylori</span> </i>tienen un requerimiento diario      de insulina incrementado en comparación con sus similares no infectados<span class=GramE>,<sup>21</sup></span> pero solamente los pacientes diabéticos con      una larga duración del padecimiento muestran una elevada<span class=SpellE>prevalencia</span>      de esta infección.<sup>22</sup> </span></p>       ]]></body>
<body><![CDATA[<p class=MsoNormal><span style='color:black;mso-ansi-language:ES-MX'>En pacientes      afectados de diabetes <span class=SpellE>mellitus</span> se describe una elevada      <span class=SpellE>prevalencia</span> de manifestaciones sintomáticas del      tracto digestivo superior y, por lo menos en parte, esto ha sido atribuido      a un vaciamiento gástrico desajustado. En pacientes dispépticos con diabetes      <span class=SpellE>mellitus</span> tipo 2 se ha descrito una elevada <span class=SpellE>prevalencia</span> de infección por <i style='mso-bidi-font-style: normal'>H. <span class=SpellE>pylori</span> </i>asociada con neuropatía autonómica.<sup>23</sup>      También, una asociación entre la <span class=SpellE>seroprevalencia</span>      del <i style='mso-bidi-font-style:normal'>H. <span class=SpellE>pylori</span>      </i>y la duración de diabetes <span class=SpellE>mellitus</span> insulinodependiente      ha sido informada.<sup>24</sup> En pacientes diabéticos, la alta frecuencia      de infección por esta bacteria no siempre ha sido explicada por diferencias      en el estado socioeconómico o por el uso de antibióticos.<sup>25</sup> <span style='mso-bidi-font-weight:bold'></span></span></p>       <p class=MsoNormal><span style='color:black;mso-ansi-language:ES-MX;mso-bidi-font-weight: bold'>El <i style='mso-bidi-font-style:normal'>H. <span class=SpellE>pylori</span>      </i>es capaz también de modificar el normal decurso de los procesos bioquímicos      en el organismo humano. Algunos de estos cambios pueden afectar la concentración      normal de glucosa en sangre de grupos poblacionales e introducir factores      de confusión adicionales en la interpretación de los resultados. Individuos      saludables e infectados muestran valores más elevados de <span class=SpellE>gastrina</span>      sérica basal y <span class=SpellE>posestimulación</span> a la ingestión de      alimentos por un período de 24 h en comparación con individuos no infectados.</span><sup><span style='color:black;mso-ansi-language:ES-MX'>26</span></sup><span style='color:black;mso-ansi-language:ES-MX'> La <span class=SpellE>gastrina</span>      es capaz de inhibir la absorción de glucosa en el intestino delgado<sup>27</sup>      y amplificar así la secreción de insulina estimulada por la glucosa.<sup>28</sup>      La relación entre la infección por <span class=SpellE><i style='mso-bidi-font-style: normal'>H. pylori</i></span>, la <span class=SpellE>gastrina</span> sérica, y      las concentraciones de insulina y glucosa séricas ha sido demostrada en pacientes      dispépticos.<sup>29</sup> A causa del efecto de la <span class=SpellE>gastrina</span> sobre la secreción de insulina y la absorción de      la glucosa, los individuos saludables infectados pueden tener niveles más      bajos de glucosa plasmática en ayunas y glucosa <span class=SpellE>posprandial</span>      que los individuos no infectados. </span></p>       <p class=MsoNormal><span style='color:black;mso-ansi-language:ES-MX'>El objetivo      de este estudio fue evaluar la <span class=SpellE>prevalencia</span> de la      infección por <i style='mso-bidi-font-style:normal'>H. <span class=SpellE>pylori</span> </i>en pacientes adultos con diabetes <span class=SpellE>mellitus</span> de Ciudad de La Habana en comparación con los valores      de individuos aparentemente sanos. </span></p>   <h4 class=MsoNormal><span style='mso-ansi-language:ES-MX;mso-bidi-font-style: italic'>Métodos</span></h4>       <p class=MsoNormal><span style='mso-ansi-language:ES-MX'>El estudio fue llevado      a cabo en pacientes atendidos en el Servicio de Atención al Paciente Diabético      del Instituto Nacional de Endocrinología. Los sujetos incluidos finalmente      en el estudio fueron clasificados en 4 grupos diferentes: un grupo control      (19 individuos), un grupo de pacientes de reciente captación en el servicio      (± 1 mes) (19 individuos), un grupo de individuos con tratamiento dietético      exclusivo (18 individuos) y otro con </span><span lang=ES style='mso-ansi-language:ES'>dieta      e <span class=SpellE>hipoglicemiantes</span> orales (22 individuos).</span></p>       <p class=MsoNormal><span lang=ES style='mso-ansi-language:ES'>Se realizaron      determinaciones de glucosa en suero a los pacientes, posterior a un ayuno      de 8 h, mediante el método <span class=SpellE>colorimétrico</span> <span class=SpellE>RapiGluco</span> <span class=SpellE>Test</span>, de producción nacional.</span></p>       <p class=MsoNormal><span lang=ES style='mso-ansi-language:ES'>La detección de      anticuerpos IgG específicos para </span><i style='mso-bidi-font-style:normal'><span style='mso-ansi-language:ES-MX'>H. <span class=SpellE>pylori</span> </span></i><span lang=ES style='mso-ansi-language:ES'>se realizó <span style='color:black'>con      el método de <span class=SpellE>inmunoensayo</span> de flujo lateral de doble      antígeno</span><span style='color:red'> </span><span class=SpellE>Instant</span>-<span class=SpellE>View<sup>TM</sup></span> (<i style='mso-bidi-font-style:normal'>Alfa      <span class=SpellE>Scientific</span> <span class=SpellE>Designs</span></i>,      <span class=SpellE><i style='mso-bidi-font-style:normal'>Inc</i></span>, CA, USA), sensibilidad      98 % para resultados positivos y negativos, 100 % de precisión y no reacciones      cruzadas con <i style='mso-bidi-font-style:normal'>C. <span class=SpellE>jejuni</span>, C. <span class=SpellE>fettus</span>, C. coli o E.      coli</i>). El método informa solo resultados preliminares cualitativos, los      cuales deben ser posteriormente confirmados. Los resultados positivos del      <span class=SpellE><i style='mso-bidi-font-style:normal'>test</i></span> no distinguen      entre infección activa y colonización por </span><i style='mso-bidi-font-style:normal'><span style='mso-ansi-language:ES-MX'>H. <span class=SpellE>pylori</span></span></i><span lang=ES style='mso-ansi-language: ES'>. Se emplearon muestras de suero conservadas en congelación, de las que se      tomaron, después de llevadas a temperatura ambiente, 200 </span><span lang=EN-GB>&#956;</span><span lang=ES style='mso-ansi-language:ES'>L para el ensayo.      La banda de migración fue siempre observada 30 s después de la aplicación      en la placa y los resultados finales registrados dentro de los 10 <span class=SpellE>min</span> posteriores. </span></p>       <p class=MsoNormal><i style='mso-bidi-font-style:normal'><span style='mso-ansi-language:ES-MX;mso-bidi-font-weight:bold'>Análisis estadístico</span></i><span style='mso-ansi-language:ES-MX;mso-bidi-font-weight:bold'>:<i style='mso-bidi-font-style: normal'> </i>los datos fueron analizados por el sistema SPSS<i style='mso-bidi-font-style:normal'> </i></span><span style='mso-ansi-language: ES-MX;mso-fareast-language:JA'>versión 11.0. Análisis <span class=SpellE>bivariados</span>      fueron realizados mediante las pruebas t de <span class=SpellE>Student</span>,      <span class=SpellE>Mann</span>-<span class=SpellE>Whitney</span> y <span class=SpellE>chi</span>-cuadrado.</span><span style='mso-ansi-language:ES-MX'>      Los intervalos de confianza para las proporciones fueron calculados utilizando      una aproximación normal de la distribución <span class=SpellE>binomial</span>. No se encontró asociación en los      pacientes tratados con dieta e <span class=SpellE>hipoglicemiantes</span>      orales (<span class=SpellE>p=</span> 0,184). </span></p>   <h4 class=MsoNormal><span lang=ES style='mso-ansi-language:ES'>Discusión</span><span style='mso-ansi-language:ES-MX'></span></h4>       <p class=MsoNormal><span style='mso-ansi-language:ES-MX'>Los estudios realizados      hasta la fecha han arrojado resultados contradictorios sobre la asociación      entre diabetes <span class=SpellE>mellitus</span> e infección por <i style='mso-bidi-font-style:normal'>H. <span class=SpellE>pylori</span></i>. Un      estudio realizó la comparación de diabéticos e individuos saludables y detectó      una significativa más baja <span class=SpellE>prevalencia</span> de infección      por <i style='mso-bidi-font-style:normal'>H. <span class=SpellE>pylori</span>      </i>en adultos mayores con diabetes tipo 1.<sup><span style='color:black'>3      </span></sup><span style='color:black'>El mismo estudio encontró que individuos jóvenes con diabetes      tipo 1 tenían niveles de <span class=SpellE>prevalencia</span> más elevados.      No se encontró asociación entre diabetes tipo 2 e infección por esta bacteria,      pero el número de individuos estudiados fue reducido. </span></span></p>       <p class=MsoNormal><span style='color:black;mso-ansi-language:ES-MX'>Otros 3      estudios, en los cuales la mayor parte de las mujeres infectadas y no infectadas      fueron pacientes con enfermedad coronaria, no detectaron asociación alguna      entre infección y diabetes.<sup>30-33</sup> </span></p>       <p class=MsoNormal><span style='mso-ansi-language:ES-MX'>Muchas enfermedades      pueden estar asociadas con la infección por <i style='mso-bidi-font-style:normal'>H.      <span class=SpellE>pylori</span></i>, como por ejemplo, enfermedades vasculares,      <span class=SpellE>autoinmunes</span>, anemia <span class=SpellE>sideropénica</span>,      diabetes, enfermedad de Parkinson y bronquiectasias. El elevado número de      estudios que ponen de manifiesto estas asociaciones sugiere que los mecanismos      patogénicos pueden conectar esta infección con muchas enfermedades de etiología      desconocida. Desafortunadamente, la calidad de los estudios desarrollados      no es homogénea y se requiere de investigaciones más detalladas con el objetivo      de poder demostrar si existe un nexo definitivo causal con la infección por      esta bacteria.
