Se estudiaron textos y artículos publicados sobre este tipo de tratamiento en las enfermedades y traumas de córnea, conjuntiva, esclera y párpados. Se estudiaron textos y art&iacute;culos publicados sobre este tipo de tratamiento en las enfermedades y traumas de c&oacute;rnea, conjuntiva, esclera y p&aacute;rpados. Se concluye que es una alternativa terap&eacute;utica valida que puede ser de gran utilidad en nuestro medio. </p>     <p><em>Palabras clave</em>: Membrana amni&oacute;tica, c&oacute;rnea. </p>     <p align="justify">Las membranes fetales fueron utilizadas desde el principio del siglo XX para los transplantes de piel en quemaduras y &uacute;lceras cut&aacute;neas.<span class="superscript">1-3</span> Por primera vez en 1940 se utilizaron en oftalmolog&iacute;a, colocando la superficie del corion sobre la superficie ocular en pacientes con quemaduras con &aacute;lcalis, pero fueron pobres los resultados.<span class="superscript">4,5 </span></p>     <p align="justify"><em>Sorsby </em> y <em>Simons </em> fueron los primeros en utilizar membrana amni&oacute;tica posiblemente sin corion y reportaron r&aacute;pida recuperaci&oacute;n con pocas complicaciones, en pacientes con quemaduras oculares ocasionadas por agentes qu&iacute;micos.<span class="superscript">6</span> </p>     <p align="justify">Hubo un per&iacute;odo de casi medio siglo en el que no se publicaron reportes sobre transplante de membrana amni&oacute;tica humana (TMAH) hasta que en 1993 <em>Batle </em>y <em>Perdomo </em>lo retoman.<span class="superscript">7</span> Actualmente numerosos autores reportan los efectos beneficiosos del TMAH por lo que hemos decidido profundizar en el conocimiento de este tema, por sus potenciales aplicaciones en nuestro medio. </p>     <p align="justify">Esta revisi&oacute;n se realiz&oacute; con el prop&oacute;sito de analizar la fisiolog&iacute;a, mecanismo de acci&oacute;n y principios de aplicaci&oacute;n de la membrana amni&oacute;tica humana; dominar las indicaciones del transplante de membrana amni&oacute;tica humana en oftalmolog&iacute;a; conocer los resultados registrados con esta modalidad terap&eacute;utica, con la intenci&oacute;n de aplicarla en nuestros pacientes. </p>     <p align="justify">Se recopil&oacute; literatura internacional sobre el uso de la membrana amni&oacute;tica en oftalmolog&iacute;a, a partir textos, revistas y art&iacute;culos publicados. Se estudi&oacute; la informaci&oacute;n recopilada y se organiz&oacute; atendiendo a los objetivos planteados. La literatura se obtuvo en la Biblioteca M&eacute;dica Nacional y en la biblioteca del Hospital “Hermanos Ameijeiras”. </p> <h4>Actualizaci&oacute;n</h4>     ]]></body>
<body><![CDATA[<p align="justify">La membrana amni&oacute;tica humana (MAH) es una membrana avascular con grosor de 0,02 a 0,5 mm compuesta por una capa &uacute;nica de c&eacute;lulas cuboideas y su membrana basal que descansa sobre una capa de tejido conectivo pr&oacute;xima al corion.<span class="superscript">8-10 </span></p>     <p align="justify">El uso de la MAH en la superficie ocular facilita la curaci&oacute;n con m&iacute;nima inflamaci&oacute;n y cicatrizaci&oacute;n al combinar la protecci&oacute;n mec&aacute;nica con factores biol&oacute;gicos. </p>     <p align="justify">Se conoce que la MAH facilita la migraci&oacute;n de c&eacute;lulas epiteliales, promueve la diferenciaci&oacute;n epitelial, refuerza la adhesi&oacute;n de las c&eacute;lulas epiteliales y previene la apoptosis celular.<span class="superscript">11-13</span> Adem&aacute;s contiene prote&iacute;nas antiinflamatorias, factores de crecimiento y factores inhibidores de la fibrosis.<span class="superscript">14-17</span> </p>     <p align="justify">Se ha postulado que la permeabilidad al ox&iacute;geno y la hidrataci&oacute;n epitelial pueden explicar la efectividad de los transplantes de membrana amni&oacute;tica humana (TMAH).<span class="superscript">5,18,19 </span></p>     <p align="justify">La MAH se obtiene de placentas procedentes de ces&aacute;reas en madres donantes seronegativas para virus de inmunodeficiencia humana, virus de la hepatitis B y C y s&iacute;filis, generalmente se utilizan criopreservada a menos 80 &deg;C.<span class="superscript">5</span> </p> <h6>Principios de aplicaci&oacute;n </h6>     <p align="justify">La orientaci&oacute;n quir&uacute;rgica de la MAH es con la capa de tejido conectivo sobre la superficie ocular y la capa de c&eacute;lulas epiteliales alejada de esta.<span class="superscript">20 </span></p>     <p align="justify">El epitelio corneal se regenera por debajo de la MAH trasplantada. A medida que progresa la curaci&oacute;n de la superficie ocular la MAH se va desprendiendo.<span class="superscript">21</span> </p>     <p align="justify">En algunos casos el epitelio se regenera sobre la MAH incorporando a esta sobre el estroma; en estos casos la membrana es visible cl&iacute;nicamente.<span class="superscript">22,23</span> </p> <h6>Indicaciones del transplante de membrana amni&oacute;tica humana<span class="superscript">7</span> </h6> <em>C&oacute;rnea </em> <ul>       <li>Defectos epiteliales persistentes. </li>       <li>&Uacute;lceras corneales. </li>       ]]></body>
<body><![CDATA[<li>Descemetocele. </li>       <li>Perforaci&oacute;n corneal. </li>       <li>Queratopatia bulosa. </li>       <li>Queratopatia en banda despu&eacute;s de la remoci&oacute;n quir&uacute;rgica de los dep&oacute;sitos calcificados. </li>       <li>Quemaduras t&eacute;rmicas o qu&iacute;micas en estado agudo. </li>       <li>Prevenci&oacute;n de la cicatrizaci&oacute;n corneal despu&eacute;s de cirug&iacute;a refractiva con l&aacute;ser excimer. </li>       <li>Necrolisis epid&eacute;rmica t&oacute;xica aguda y s&iacute;ndrome de Stevens Johnson. </li>     </ul>     <p><em>Conjuntiva </em></p> <ul>       <li> Reconstrucci&oacute;n de la conjuntiva bulbar y/o fondo de saco conjuntival por cicatrizaci&oacute;n o grandes lesiones. </li>       ]]></body>

<body><![CDATA[</ul>     <p align="justify">Para su utilizaci&oacute;n sobre la c&oacute;rnea se prefiere sutura no absorbible, continua o en puntos sueltos, colocados paralelos al borde del transplante, cuyo tama&ntilde;o debe exceder al menos algunos mil&iacute;metros el defecto a recubrir. </p>     <p align="justify">Se remueve el epitelio alrededor del defecto y la base de este se limpia de detritos con aplicador. Seg&uacute;n la magnitud del defecto corneal se aplica una capa &uacute;nica, o varias capas plegadas de MAH. En este &uacute;ltimo caso otra capa &uacute;nica de MAH se coloca recubriendo toda la c&oacute;rnea. Los nudos de la sutura de Nylon 10-0 no se entierran para facilitar su remoci&oacute;n. Se coloca un lente de contacto blando.<span class="superscript">7 </span></p>     <p align="justify">Para los transplantes conjuntivales  se prefiere sutura absorbible. Cuando se desea recubrir toda la superficie ocular se anclan las primeras suturas a la conjuntiva tarsal con Vicryl 10-0, a continuaci&oacute;n se ancla la membrana 360&deg; a nivel del limbo c&oacute;rneo-conjuntival con Nylon 10-0. Se concluye colocando m&uacute;ltiples suturas sobre la superficie interna de los p&aacute;rpados y la conjuntiva bulbar, con puntos sueltos de Vicryl.<span class="superscript">24</span> </p>     <p align="justify">Para cubrir la superficie de los m&aacute;rgenes palpebrales  se utilizan tiras de MAH, con sutura continua de Vicryl 10-0. En los defectos cut&aacute;neos palpebrales se utilizan segmentos de MAH que excedan 1 mm el tama&ntilde;o del defecto en la piel.<span class="superscript">24</span> </p> <h6>Resultados regristrados en la literatura con los transplantes de membrana amni&oacute;tica humana en oftalmolog&iacute;a </h6>     <p align="justify">Se ha observado que entre 60 y 90 % de los pacientes con defectos epiteliales persistentes corneales curan exitosamente.<span class="superscript">25-27 </span></p>     <p align="justify">La realizaci&oacute;n del TMAH en los primeros 10 d&iacute;as despu&eacute;s de quemaduras qu&iacute;micas y t&eacute;rmicas de la superficie ocular maximiza su efecto beneficioso, y se propone como tratamiento de primera l&iacute;nea.<span class="superscript">28-31</span> Durante los estadios agudos de necr&oacute;lisis epid&eacute;rmica toxica se han realizados TMAH, se han reportado efectos beneficiosos se ha se&ntilde;alado que pueden convertirse en el tratamiento de elecci&oacute;n.<span class="superscript">24,32</span> </p>     <p align="justify">Se ha logrado alivio del dolor intolerante en pacientes con queratopat&iacute;a bulosa cr&oacute;nica entre 80 y 90 % despu&eacute;s del TMAH como alternativa para la queratoplastia penetrante en ausencia de tejido donante.<span class="superscript">33-36</span> </p>     <p align="justify">Igualmente ha resultado &uacute;til en pacientes con queratopatia en banda sintom&aacute;tica despu&eacute;s de la remoci&oacute;n de los dep&oacute;sitos corneales calcificados, lo cual ha favorecido la epitelizaci&oacute;n completa y la observaci&oacute;n de la superficie corneal lisa de manera estable.<span class="superscript">37 </span></p>     <p align="justify">Estudios recientes han demostrado menos tasa de recurrencia despu&eacute;s de la escisi&oacute;n del <em>pterigium</em>, combinado con el TMAH.<span class="superscript">36-42</span> </p>     ]]></body>
<body><![CDATA[<p align="justify">La reconstrucci&oacute;n de la superficie ocular en pacientes con penfigoide cicatrizal ha demostrado una buena efectividad en la mayor&iacute;a de los casos.<span class="superscript">32,43,44 </span></p> <h4>Conclusiones </h4>     <p align="justify">El uso de la membrana amni&oacute;tica humana en la superficie ocular facilita su curaci&oacute;n con m&iacute;nima inflamaci&oacute;n y cicatrizaci&oacute;n al combinar la protecci&oacute;n mec&aacute;nica con factores biol&oacute;gicos. </p>     <p align="justify">Las indicaciones del transplante de membrana amni&oacute;tica humana incluyen enfermedades y traumatismos de c&oacute;rnea, conjuntiva, esclera y p&aacute;rpados. </p>     <p align="justify">Los estudios analizados demuestran que con esta modalidad de tratamiento se han obtenido resultados muy satisfactorios; se ha logrado la curaci&oacute;n y/o reconstrucci&oacute;n de la superficie ocular. </p>     <p>Consideramos que constituye una alternativa terap&eacute;utica valida que puede ser de gran utilidad en nuestro medio. </p> <h4>Summary</h4> <h6>Some uses of human amniotic membrane in Ophthalmology </h6>     <p align="justify"><strong> </strong>A literature review was made on the use of amniotic membrane in ophthalmology, with the objective of finding out its mechanisms of action and applications as well as the results of human amniotic membrane transplantation for ocular surface reconstruction.
