<body><![CDATA[<p align="justify">     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Datos  epidemiol&oacute;gicos revelan, que el c&aacute;ncer cut&aacute;neo no melanoma (CCNM) constituye un  problema fundamental en los servicios de salud, lo cual se evidencia en el  notable aumento de su morbilidad en el &aacute;mbito mundial y en la aparici&oacute;n de un  gran n&uacute;mero de pacientes afectados con ulceraciones y deformidades est&eacute;ticas.  Por esta raz&oacute;n, el conocimiento de sus causas adquiere especial importancia.<sup>1</sup></font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Entre  los factores etiol&oacute;gicos extr&iacute;nsecos o ambientales, se destacan la exposici&oacute;n  prolongada a radiaciones (ultravioletas, rayos X, Grenz y gamma), sustancias  qu&iacute;micas, infecciones virales y la irritaci&oacute;n cr&oacute;nica secundaria a la mala  higiene.<sup>2</sup></font></p> <font size="2" face="Verdana, Arial, Helvetica, sans-serif">     <p>Los  factores intr&iacute;nsecos incluyen genodermatosis (xeroderma pigmentoso y el  albinismo oculocut&aacute;neo), inmunosupresi&oacute;n (por enfermedad sist&eacute;mica, secundaria  a tratamiento medicamentoso inmunosupresor o a infecci&oacute;n por VIH) y cambios  cut&aacute;neos preexistentes.<sup>4,5</sup> Tambi&eacute;n se le confiere un gran valor a  las denominadas lesiones cut&aacute;neas precancerosas, premalignas o precursoras del  CCNM; consideradas por la mayor&iacute;a de los investigadores, carcinomas  espinocelulares (CEC) intraepiteliales <i>in  situ</i>. Estas lesiones incluyen a la queratosis: act&iacute;nica (QA), arsenical,  por hidrocarburos, t&eacute;rmica, por radiaciones cr&oacute;nicas y cicatricial cr&oacute;nica.<sup>6</sup> La QA se se&ntilde;ala mundialmente como la precancerosis  cut&aacute;nea con mayor incidencia y elevada tendencia a la degeneraci&oacute;n maligna. La  mayor&iacute;a de los autores coinciden en que son esencialmente carcinomas  escamocelulares intraepiteliales en evoluci&oacute;n.<sup>7</sup> Se a&ntilde;aden a este grupo la leucoplasia y eritoplasia, que  poseen una connotaci&oacute;n semiol&oacute;gica y no corresponden a diagn&oacute;sticos  histol&oacute;gicos.<sup>8</sup></p>     <p>Se  se&ntilde;ala adem&aacute;s el papel causal intr&iacute;nseco de los CEC <i>in situ</i>, que incluyen enfermedad de Bowen, eritoplasia de Queyrat y  la papulosis bowenoide. La enfermedad de Bowen aparece relacionada a la exposici&oacute;n  solar y arsenical significativa, radiaciones ionizantes, inmunodepresi&oacute;n y virus  del papiloma humano (VPH), especialmente el serotipo 1. Cuando la enfermedad de  Bowen afecta a las mucosas (principalmente las del pene en varones no cincunciados)  se denomina eritoplasia de Queyrat, descrita inicialmente en 1891 por  Tarnovsky, debe su nombre a Queyrat, quien en 1911 la denomin&oacute; <i>erythoplasie du gland</i>, se asocia  frecuentemente con fimosis, higiene deficiente que favorece la acumulaci&oacute;n de  esmegma, calor, fricci&oacute;n, traumatismos e infecci&oacute;n por VHP (serotipos 8 y 16),  evoluciona a un CEC invasivo con mayor frecuencia que la enfermedad de Bowen.  En 1979 se introdujo el t&eacute;rmino populosis bowenoide para referirse a una  malignidad que est&aacute; causada por VHP (serotipos 16, 18, 31-35, 39, 42, 45,  51-54). El serotipo 16 es el m&aacute;s frecuente y junto al 18 y 33 posee la mayor  capacidad oncog&eacute;nica<b>.</b><sup>8-10</sup></p>     <p>Actualmente  se conoce, que la radiaci&oacute;n UV (RUV) es el factor causal m&aacute;s importante en individuos  que poseen una predisposici&oacute;n gen&eacute;tica y caracter&iacute;sticas fenot&iacute;picas que los  hacen vulnerables (piel blanca, abundantes pecas, ojos y cabellos claros y poca  capacidad para broncearse). La radiaci&oacute;n solar que est&aacute; involucrada en el  desarrollo del CCNM est&aacute; dentro del espectro de la RUV con longitud de onda entre  290 y 400nm. Las radiaciones UVA (320&ndash;400nm), son 20 veces m&aacute;s abundantes que las  UVB (290&ndash;320nm) y potencian el efecto delet&eacute;reo de estas &uacute;ltimas, que ostentan  una acci&oacute;n carcinog&eacute;nica 1600 veces mayor.<sup>11,12</sup> La dosis total  acumulativa de las radiaciones ionizantes en un &aacute;rea cut&aacute;nea determinada,  constituye un factor importante en la aparici&oacute;n del CCNM y las dosis  adicionales aumentan el riesgo a padecer la enfermedad.<sup>13</sup></p>     <p>Las  sustancias qu&iacute;micas implicadas en la carcinog&eacute;nesis qu&iacute;mica incluyen al  ars&eacute;nico, <sup>14</sup> alquitr&aacute;n de hulla, tratamientos m&eacute;dicos con parafina,  creosota, mostaza nitrogenada t&oacute;pica, breas y psoralenos.<sup>2</sup> El  carcinoma epidermoide del labio y la boca, est&aacute;n unidos al empleo del tabaco  fumado o masticado y al consumo de alcohol y betel.<sup>15</sup></p>     <p>Las  infecciones por VPH de serotipos 6, 16 y 18 se relacionan con el c&aacute;ncer epidermoide  del pene.<sup>16</sup></p>     <p>Tambi&eacute;n  este proceso oncoproliferativo cut&aacute;neo se describe en el curso de enfermedades  que conllevan a un estado inflamatorio cr&oacute;nico de la piel, como el liquen  plano, liquen escleroatr&oacute;fico, tuberculosis cut&aacute;nea, infecciones f&uacute;ngicas, lupus  eritematoso, necrobiosis lipo&iacute;dica, hidradenitis supurativa, ictiosis  cong&eacute;nita, epiderm&oacute;lisis ampollosa distr&oacute;fica, acn&eacute; cicatricial (irradiada o  no), osteomielitis fistulizada, sinus pilonidal y&nbsp; &uacute;lceras cr&oacute;nicas varicosas.<sup>17</sup></p>     <p>Numerosos  son los factores que intervienen en la aparici&oacute;n del CCNM, y por tanto, diversas  sus causas. No parece existir un fen&oacute;meno &uacute;nico responsable, pero s&iacute; la  asociaci&oacute;n de varios que de una manera escalonada a lo largo de los a&ntilde;os,  conducen a la formaci&oacute;n de este tipo peculiar de tumor epitelial maligno. </p> </font> </p>     ]]></body>
