El tratamiento quirúrgico oportuno es la piedra angular para evitar las complicaciones y reincorporar al enfermo a las actividades que realizaba con anterioridad. Objetivo: mostrar el comportamiento de un grupo de pacientes operados por esta enfermedad y determinar los factores que más influyen en el retardo de la cirugía y en la presencia de complicaciones. Método: se realizó un estudio observacional analítico de corte transversal en 201 pacientes diagnosticados por fractura de la cadera y tratados mediante cirugía en el Hospital Universitario Manuel Ascunce Domenech de la ciudad de Camagüey desde enero a diciembre del año 2011. Resultados: se encontró significación estadística en pacientes del sexo femenino con una razón en relación al masculino de 2,4 a 1. En la mayoría de los enfermos (91 %) la cirugía se realizó antes de las 24 horas. El 30,3 % de los enfermos presentó algún tipo de complicación. Los factores relacionados con los pacientes complicados y que mostraron significación estadística fueron estadía mayor a tres, sexo femenino y cirugía pasadas las 24 horas. Conclusiones: la cirugía tardía más allá de las 24 horas y la estadía mayor a tres se relacionan con un mayor número de complicaciones en pacientes con fractura de la cadera.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[Background: due to aging increasing population, hip fracture is a health problem of major proportions. Adequate surgical treatment is the cornerstone to avoid complications and to return the patient to the activities carried out previously. Objective: to show the behavior of a group of patients operated on this disease. Method: an observational, analytical, cross section study was performed in 201 patients diagnosed as having hip fracture and treated by surgery in the University Hospital Manuel Ascunce Domenech in Camagüey city, from January to December 2011. Female patients outnumbered male patients in the ratio of 2, 4 to 1. In the majority of patients (91%), surgery was carried out before 24 hours. The 30, 3 % of patients showed some kind of complication. Factors related to complicated patients and that showed statistical significance had a hospital stay longer than three days, were female patients and underwent surgery after 24 hours. Conclusions: delayed surgery further than 24 hours and hospital stay more than three days were related to a greater number of complications in patients with hip fracture.]]></p></abstract>

<body><![CDATA[<p>&nbsp;</p>    <p>&nbsp;</p>    <p><font size="3" face="Verdana, Arial, Helvetica, sans-serif"><b>INTRODUCCI&Oacute;N</b></font></p>    <p align="justify">      <div align="justify">    <P><font size="2" face="Verdana, Arial, Helvetica, sans-serif">La  fractura de cadera (FC) constituye una enfermedad muy frecuente en la actualidad.  Para el manejo correcto de pacientes con esta afecci&oacute;n se necesita de la  coordinaci&oacute;n de varias especialidades m&eacute;dicas con el fin de reducir  el n&uacute;mero de complicaciones y propiciar la rehabilitaci&oacute;n precoz  de estos enfermos. <sup>1-3</sup></font></P>    <P><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Seg&uacute;n  Lucas Nikkel BA, et al, <sup>4</sup> los pacientes con FC en los Estados Unidos  superan la cifra de 250 000 al a&ntilde;o y se estima que esta cifra seguir&aacute;  en ascenso debido al envejecimiento de la poblaci&oacute;n que es cada vez mayor.  <sup>5,6</sup></font></P>    <P><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Por  lo general, los enfermos con esta entidad se asocian a otras enfermedades entre  las que se encuentran insuficiencia cardiaca congestiva y diabetes mellitus en  uno de cada cinco pacientes. 8,9 La presencia de tres o m&aacute;s enfermedades  cr&oacute;nicas asociadas incrementa de manera sustancial la mortalidad, de all&iacute;  la importancia del control y manejo adecuado para lograr la compensaci&oacute;n  del </font><font size="2" face="Verdana, Arial, Helvetica, sans-serif">enfermo  lo antes posible y poder realizar la intervenci&oacute;n quir&uacute;rgica. <sup>10,11</sup></font></P>    <P><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Para  el diagn&oacute;stico de esta enfermedad es fundamental el m&eacute;todo cl&iacute;nico,  que orienta al cirujano desde el primer momento sobre el tipo de fractura que  tiene el enfermo, lo que puede ser corroborado mediante la realizaci&oacute;n  de una radiograf&iacute;a simple de la articulaci&oacute;n. <sup>12-14</sup></font></P>    <P><font size="2" face="Verdana, Arial, Helvetica, sans-serif">El  tratamiento de pacientes con FC no termina con la operaci&oacute;n, a partir de  este momento se deben evitar un grupo de complicaciones que llevan al enfermo  en ocasiones a la muerte. <sup>15-16</sup></font></P>    ]]></body>
<body><![CDATA[<P><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Debido  al gran n&uacute;mero de pacientes afectados por FC, los autores se proponen la  siguiente investigaci&oacute;n con el objetivo de mostrar el comportamiento de  un grupo de pacientes operados por esta afecci&oacute;n y determinar los factores  que m&aacute;s influyen en el retardo de la cirug&iacute;a y en la presencia de  complicaciones.</font> </P></div>    <p>&nbsp;</p><font size="3" face="Verdana, Arial, Helvetica, sans-serif"><b>M&Eacute;TODOS</b></font>    <p></p>    <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Se  realiz&oacute; un estudio observacional anal&iacute;tico de corte transversal  en 201 pacientes diagnosticados por fractura de cadera y tratados mediante cirug&iacute;a  en el Hospital Universitario Manuel Ascunce Domenech de la ciudad de Camag&uuml;ey  desde enero a diciembre del a&ntilde;o 2011. </font></p>    <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">El  universo fue de 221 pacientes y la muestra no probabil&iacute;stica estuvo constituida  por 201 pacientes que reunieron los siguientes criterios de inclusi&oacute;n:</font></p><ol>      <li><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Edad mayor o igual  a 60 a&ntilde;os.</font></li>    <li><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Pacientes  tratados mediante cirug&iacute;a.</font></li>    </ol>    <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Criterios  de exclusi&oacute;n:</font></p><ol>     <li><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Pacientes  con negaci&oacute;n personal o familiar a participar en la investigaci&oacute;n.</font></li>    ]]></body>
<body><![CDATA[</ol>    <p align="justify">    <P><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Los  pacientes fueron tratados mediante cirug&iacute;a seg&uacute;n el tipo de fractura  en caso de las extracapsulares, la reducci&oacute;n cruenta y osteos&iacute;ntesis,  para las intracapsulares no desplazadas y menores de 65 a&ntilde;os, la fijaci&oacute;n  y en caso de las fracturas desplazadas, artroplastia unipolar o bipolar. Se aplic&oacute;  el algoritmo del programa nacional para la atenci&oacute;n a enfermos con FC.      <br> Para la recolecci&oacute;n y evaluaci&oacute;n de los resultados se confeccion&oacute;  un formulario a partir de los expedientes cl&iacute;nicos, con los siguientes  datos: edad, sexo, tipo de fractura, tiempo en que se realiz&oacute; la cirug&iacute;a,  causa de la demora, estad&iacute;a hospitalaria y complicaciones. </font></P>    <P><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Los  datos se analizaron estad&iacute;sticamente mediante la prueba de la x2, se consider&oacute;  todo valor menor a p&lt; 0.05 como significativo desde el punto de vista estad&iacute;stico  y se calcul&oacute; el <u>Odds Ratio</u>. Se utiliz&oacute; el programa estad&iacute;stico  SPSS 15.0.1.</font></P>    <P><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Los  resultados se mostraron en tablas y gr&aacute;ficos.</font></P><font size="2" face="Verdana, Arial, Helvetica, sans-serif">    <p>&nbsp;</p></font><font size="3" face="Verdana, Arial, Helvetica, sans-serif"><b>RESULTADOS</b></font>    <p></p>    <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Predomin&oacute;  el sexo femenino en el 70,6 % de los enfermos y se encontr&oacute; significaci&oacute;n  estad&iacute;stica para el sexo femenino con un 0,005. La raz&oacute;n sexo femenino-masculino  fue de 2,4 a 1. El grupo de edades que mostr&oacute; la mayor frecuencia fue el  de 80 a&ntilde;os o m&aacute;s representado por el 51,2 %. Los restantes grupos  de edades mostraron frecuencia similar entre ellos. El promedio de edades fue  de 78,3 a&ntilde;os. <body><![CDATA[<p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">    <br>  Las fracturas extracapsulares predominaron sobre las intracapsulares en una raz&oacute;n  de 1,4 a 1. El promedio de estad&iacute;a hospitalaria fue de 3,2. </font></p>    <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">La  cirug&iacute;a se realiz&oacute; en la mayor&iacute;a de los enfermos, 91 % antes  de las 24 horas de llegado al hospital. El resto de los enfermos fue intervenido  despu&eacute;s de las 25 horas. <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">    <br>  Dentro las causas que demoraron la intervenci&oacute;n antes de las 24 horas,  se destac&oacute; la hipertensi&oacute;n arterial con 21 enfermos y un <u>odds  ratio</u> de 4,93, lo que evidenci&oacute; que todo paciente con hipertensi&oacute;n  arterial preoperatoria tiene un 4,93 m&aacute;s probabilidades de no ser operado  antes de las 24 horas que aquel paciente que no presenta esta enfermedad. Otras  causas con significaci&oacute;n estad&iacute;stica y que se les calcul&oacute;  el <u>odds ratio</u> fueron la anemia preoperatoria, cardiopat&iacute;a isqu&eacute;mica  descompensada y la bronconeumon&iacute;a. Se encontraron otras causas, pero no  presentaron significaci&oacute;n estad&iacute;stica como: la enfermedad diarreica  aguda, la enfermedad pulmonar obstructiva cr&oacute;nica y la epilepsia. <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">    <br>  Un total de 61 pacientes para un 30,3 % presentaron complicaciones entre las que  se destac&oacute; la anemia posoperatoria con un 62,2 % y la descompensaci&oacute;n  de la enfermedad de base en el 16,3 %. El fallo del material de osteos&iacute;ntesis  y la muerte se encontr&oacute; en el 13,1 y 11,4 % respectivamente. Otras complicaciones  presentadas fueron: la bronconeumon&iacute;a, trombosis venosa profunda, luxaci&oacute;n  de la pr&oacute;tesis, infecci&oacute;n y hematoma de la herida adem&aacute;s  de la seudoartrosis del cuello femoral. <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">    <br>  Los factores que mostraron significaci&oacute;n estad&iacute;stica en relaci&oacute;n  a los pacientes complicados fueron: estad&iacute;a mayor a tres con un <u>odds  ratio</u> de 3,4, el sexo femenino 1,1 y los pacientes operados m&aacute;s all&aacute;  de las 24 horas de producida la fractura con 1,8. El odds ratio significa la probabilidad  en presencia de este factor de presentar complicaciones en relaci&oacute;n al  enfermo que no presenta el factor. <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif"><b>&nbsp;</b></font></p>    <p><font size="3" face="Verdana, Arial, Helvetica, sans-serif"><b>DISCUSI&Oacute;N</b></font></p>    <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">La  frecuencia marcada del sexo femenino encontrada en la investigaci&oacute;n coincide  con autores cubanos como P&eacute;rez Carbajal A, et al, <sup>17</sup>Escarpanter  Buli&eacute;s JC <sup>18</sup> y Quesada Musa JV, et al, <sup>19</sup>. Existen  factores anat&oacute;micos que predisponen en mayor medida a sufrir de esta enfermedad  entre los que se encuentran: una mayor expectativa de vida en relaci&oacute;n  a los hombres, disposici&oacute;n en varo de la cadera y sufrir del proceso osteopor&oacute;tico  en mayor medida por la edad y la menopausia. <sup>20-22</sup></font></p>    <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Debido  al envejecimiento marcado de la poblaci&oacute;n, esta enfermedad se presenta  en pacientes cada vez de mayor edad, lo que constituye un verdadero problema de  salud. <sup>23-25</sup></font></p>    <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">En  la investigaci&oacute;n predominaron los enfermos por encima de 80 a&ntilde;os,  lo que guard&oacute; estrecha relaci&oacute;n con lo planteado por P&eacute;rez  Rivera OM, et al, <sup>26</sup></font></p>    <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Las  fracturas extracapsulares predominaron en la investigaci&oacute;n, lo que coincide  con lo planteado por Collazo &Aacute;lvarez C, et al, <sup>27</sup> y Perera Rodr&iacute;guez  NR, et al. <sup>28</sup></font></p>    ]]></body>
<body><![CDATA[<p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">La  cirug&iacute;a temprana en pacientes con FC se asocia a la presencia de un menor  n&uacute;mero de complicaciones. Sin embargo, Smektala R, et al, <sup>29</sup>  y Simunnovic N, et al, <sup>30</sup> no encontraron en sus investigaciones una  relaci&oacute;n directa en relaci&oacute;n al tiempo de la cirug&iacute;a y complicaciones,  y si de otros factores como: la edad, sexo femenino y presencia de enfermedades  cr&oacute;nicas. Por otra parte, algunos autores plantean complicaciones que var&iacute;an  seg&uacute;n la cirug&iacute;a temprana o tard&iacute;a. <sup>31,32</sup></font></p>    <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">La  hipertensi&oacute;n arterial es la enfermedad cr&oacute;nica encontrada con mayor  frecuencia en pacientes con fractura de cadera, lo que coincide con lo planteado  por P&eacute;rez Rivera OM, et al,<sup>26</sup>. La descompensaci&oacute;n de  enfermedades de base son las que con m&aacute;s frecuencia demoran la intervenci&oacute;n  quir&uacute;rgica lo que coincide con lo planteado por Quesada Musa JV, et al,  <sup>19</sup> el cu&aacute;l adem&aacute;s plantea que la anemia preoperatoria  constituy&oacute; la sexta causa de retraso de la cirug&iacute;a y existe discrepancia  entre los datos obtenidos en esta investigaci&oacute;n con los de este autor.      <br> El comportamiento de los pacientes complicados fue algo superior a los reportados  por Collazo &Aacute;lvarez H, et al, <sup>27</sup> que report&oacute; un 21,4  %. De estas complicaciones la anemia posoperatoria fue la m&aacute;s encontrada  y guarda relaci&oacute;n con lo reportado por P&eacute;rez Rivera OM et al, 26.  El resto de las complicaciones mostraron un comportamiento muy similar en comparaci&oacute;n  con la bibliograf&iacute;a consultada. <sup>33-35</sup></font></p>    <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">La  cirug&iacute;a temprana determina la baja frecuencia de complicaciones y favorece  la evoluci&oacute;n satisfactoria de estos enfermos, ya que permite incorporarlos  m&aacute;s r&aacute;pido al proceso de rehabilitaci&oacute;n. <sup>36,37</sup>  </font></p>    <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Por  otra parte, la utilizaci&oacute;n de medidas profil&aacute;cticas en la prevenci&oacute;n  de complicaciones como las infecciones y la enfermedad tromboemb&oacute;lica disminuyen  de manera significativa la presencia de estas enfermedades. <sup>38</sup></font></p>    <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">La  estad&iacute;a hospitalaria prolongada favorece el desarrollo de complicaciones  al permanecer por un mayor tiempo el enfermo en la instituci&oacute;n y exponerse  a g&eacute;rmenes propios del hospital, que por lo general son m&aacute;s agresivos  y no se utiliz&oacute; en primera instancia los antimicrobianos profil&aacute;cticos  con acci&oacute;n espec&iacute;fica, para evitar el desarrollo de infecciones  locales. Por otra parte, al permanecer en la instituci&oacute;n se expone en mayor  medida a presentar infecciones respiratorias por g&eacute;rmenes nosocomiales,  que a esta edad son graves y en muchas ocasiones llevan a la muerte del enfermo.  <sup>39-41</sup></font></p>    <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">El  sexo femenino se relaciona con un mayor n&uacute;mero de complicaciones, no solo  por la gran cantidad de enfermos sino adem&aacute;s porque su promedio de edad  es por lo general mayor, lo que explica su relaci&oacute;n con las complicaciones.  <sup>42,43</sup></font></p><font size="3" face="Verdana, Arial, Helvetica, sans-serif"><b>    <p>&nbsp;</p>CONCLUSIONES</b></font>    <p></p>    <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">La  cirug&iacute;a temprana en pacientes con FC antes de las 24 horas constituye un  elemento de gran importancia para disminuir las complicaciones en este grupo de  enfermos, entre los factores que influyen en el retraso de la intervenci&oacute;n  quir&uacute;rgica se encuentran la descompensaci&oacute;n de enfermedades cr&oacute;nicas  de base las que necesitan de valoraci&oacute;n por el equipo multidisciplinario  relacionado con el manejo de estos enfermos.</font></p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">      ]]></body>
