El tratamiento de esta enfermedad es difícil, debido a la poca capacidad biológica de los cartílagos para su cicatrización. Desarrollo: se revisaron más de 300 artículos sobre el tema, de ellos 44 de los últimos cinco años. Se discutieron los aspectos relacionados con las características propias de los enfermos que presentan lesiones del cartílago y cómo decidir o sugerir las variantes de tratamientos según los factores propios de la lesión y del enfermo. Se abordaron técnicas tales como: el desbridamiento artroscópico, el injerto autólogo de condrocitos asociados a andamios y matrices sintéticas, en cada caso se analizaron las ventajas y desventajas, así como las indicaciones específicas de cada procedimiento, basado en los reportes de la literatura internacional. Conclusiones: el tratamiento de pacientes con lesiones del cartílago depende de varios factores, entre los que se conjugan los propios del enfermo y la preparación del médico. La intervención oportuna y efectiva en pacientes con esta enfermedad disminuye los síntomas y signos, además de la necesidad de utilizar técnicas quirúrgicas más complejas.]]></p></abstract>

The treatment for the disease is difficult because of the poor biological capacity of cartilages to heal. Development: we looked through more than 300 articles related to the topic, 44 of which are from the last five years. Aspects related to the common characteristics of the patients that have lesions of the cartilage were discussed, and how to decide and suggest the different treatments according to the common factors of the lesion and of the patient as well. Techniques such as: arthroscopic debridement, autologous transplantation of chondrocytes associated to scaffolds and synthetic arrays were tackled. In each case, the advantages and disadvantages were analyzed, as well as the specific indications to each procedure based on reports from the international literature. Conclusions: the treatment of patients with lesions of the cartilage of the knee depends on some factors that combine those related to the common characteristics of the patient and the training of the doctor. Timely and effective operation in patients with the disease reduces the symptoms and the need to use more complex surgical techniques.]]></p></abstract>

Seg&uacute;n Curl W, et al, <sup>1</sup> en un estudio retrospectivo en que evalu&oacute;  los informes operatorios de 31 516 artroscopias de la rodilla, el 63 % de los  pacientes presentaron lesi&oacute;n de los cart&iacute;lagos, de ellos el 19 %  con lesiones tipo IV, seg&uacute;n la clasificaci&oacute;n artrosc&oacute;pica  de Outerbridge RE. <sup>2</sup></font></p>    <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">El  cart&iacute;lago articular es un tejido aneural, avascular y alinf&aacute;tico  de all&iacute; las razones por la que resulta muy dif&iacute;cil su cicatrizaci&oacute;n  y tratamiento. Las causas de esta enfermedad son variadas, se incluyen los traumatismos,  as&iacute; como trastornos de tipo degenerativos, infecciones intraarticulares,  osteocondritis disecantes, osteonecrosis entre otras. <sup>3-5</sup> </font></p>    <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Los  s&iacute;ntomas y signos de pacientes con lesiones de cart&iacute;lagos pueden  ser diversos entre los que se encuentran: el dolor de variada intensidad, sensaci&oacute;n  de chasquidos articulares, sinovitis y bloqueo articular provocado por el desprendimiento  de fragmentos de cart&iacute;lagos que interfieren en la mec&aacute;nica de la  articulaci&oacute;n. <sup>6-7</sup></font></p>    <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Las  variantes utilizadas en el tratamiento de la lesiones de cart&iacute;lago pueden  ser seg&uacute;n Williams RJ y Brophy RH <sup>8</sup> paliativas que incluyen  el desbridamiento, por lo general por v&iacute;a artrosc&oacute;pica; las que  favorecen la reparaci&oacute;n intr&iacute;nseca como: la microfractura, perforaciones  y la artroplastia de abrasi&oacute;n; las basadas en el trasplante total del tejido  que aqu&iacute; se encuentran la mosaicoplastia y el injerto osteocondral; la  aplicaci&oacute;n de c&eacute;lulas con el ejemplo cl&aacute;sico del injerto  aut&oacute;logo de condrocitos y por &uacute;ltimo aquellas basadas en el uso  de andamios sint&eacute;ticos asociados a la siembra de condrocitos. <sup>9,10</sup></font></p>    ]]></body>
<body><![CDATA[<p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Debido  a la gran cantidad de pacientes afectados por esta enfermedad, es necesario actualizar  al personal m&eacute;dico, sobre aspectos esenciales relacionados con las caracter&iacute;sticas  de varios factores en pacientes con lesiones de cart&iacute;lagos, que a su vez  determinan en la selecci&oacute;n de las t&eacute;cnicas quir&uacute;rgicas a  utilizar.</font></p>    <p align="justify">&nbsp;</p>    <p align="justify"><font size="3" face="Verdana, Arial, Helvetica, sans-serif"><b>DESARROLLO</b></font></p>    <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Para  decidir la conducta a seguir en pacientes con lesiones de los cart&iacute;lagos  es necesarios conjugar varios par&aacute;metros o factores entre los que se encuentran:  la causa de la lesi&oacute;n; cualidades propias de la injuria del cart&iacute;lago,  que a su vez incluye: localizaci&oacute;n, grado, tama&ntilde;o, morfolog&iacute;a  y car&aacute;cter; condiciones de la extremidad inferior y caracter&iacute;sticas  propias del paciente. <sup>10-12</sup></font></p>    <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">En  relaci&oacute;n a las cualidades propias de la injuria del cart&iacute;lago es  necesario definir varios aspectos entre los que se encuentra la localizaci&oacute;n,  la superficie articular de la rodilla puede ser dividida en seis partes: r&oacute;tula,  tr&oacute;clea, c&oacute;ndilo femoral medial y lateral, adem&aacute;s de platillo  tibial medial y lateral, este aspecto es de vital importancia para determinar  si existe afecci&oacute;n de la superficie de apoyo de la articulaci&oacute;n.  El grado de la lesi&oacute;n de cart&iacute;lago puede ser medida a trav&eacute;s  de dos clasificaciones, las m&aacute;s usadas son la descrita por Outerbridge  RE <sup>2</sup> y la propuesta por la Sociedad Internacional de la Reparaci&oacute;n  de Cart&iacute;lagos, para realizar la gradaci&oacute;n de la lesi&oacute;n es  necesario la utilizaci&oacute;n de la v&iacute;a artrosc&oacute;pica. El tama&ntilde;o  de la lesi&oacute;n por lo general es descrita en cent&iacute;metros cuadrados  y para esto el cirujano se apoya en el uso del explorador artrosc&oacute;pico.<sup>8-11</sup></font></p>    <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">En  relaci&oacute;n a la morfolog&iacute;a se debe diferenciar entre una lesi&oacute;n  condral u osteocondral, ya que en esta &uacute;ltima existe afecci&oacute;n del  hueso subcondral y se pierde la arquitectura de la superficie articular, por lo  que se necesita de t&eacute;cnicas como la mosaicoplastia e injertos. <sup>13,14</sup></font></p>    <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Las  condiciones de la extremidad inferior deben ser tambi&eacute;n evaluadas, muy  en especial las referentes a: la estabilidad articular, signos de lesi&oacute;n  de menisco y alineaci&oacute;n tibiofemoral. <sup>15</sup> Estos factores influyen  en el resultado final de los procedimientos realizados para la reparaci&oacute;n  de cart&iacute;lagos y por ende deben ser corregidos con antelaci&oacute;n, mediante  cirug&iacute;as espec&iacute;ficas, en caso de las deformidades angulares; las  osteotom&iacute;as correctoras y en caso de inestabilidad anterior la reconstrucci&oacute;n  del ligamento cruzado anterior. <sup>16,17</sup></font></p>    <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">En  relaci&oacute;n al paciente, se deben valorar aspectos como: edad, &iacute;ndice  de masa corporal, factores hereditarios, grado de demanda f&iacute;sica, necesidad  funcional del enfermo, expectativa del paciente al tratamiento y la posibilidad  de realizar la rehabilitaci&oacute;n una vez operado. La edad constituye un factor  a considerar para la realizaci&oacute;n de ciertos procedimientos, entre los que  se encuentra la microfractura, ya que esta t&eacute;cnica es m&aacute;s efectiva  en pacientes por debajo de 55 a&ntilde;os. El &iacute;ndice de masa corporal por  encima de 30 constituye un factor de mal pron&oacute;stico para la mayor&iacute;a  de los procedimientos encaminados a mejorar el cart&iacute;lago articular y este  factor puede ser modificable antes de la intervenci&oacute;n. <sup>8,17</sup></font></p>    <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif"><b>Tratamiento</b></font></p>    <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">El  tratamiento de pacientes con lesiones del cart&iacute;lago seg&uacute;n Drakos  MC y Allen AA <sup>18</sup> comienza con las medidas conservadoras, pero esta  modalidad se relaciona con resultados menos favorables y no detiene el curso natural  de la enfermedad. Por esta raz&oacute;n, la mayor&iacute;a de estos enfermos son  sometidos a diferentes modalidades de tratamientos quir&uacute;rgicos, para los  cuales William RJ y Brophy RH <sup>8</sup> proponen las siguientes indicaciones:</font></p><ol start="1" type="1">      ]]></body>
<body><![CDATA[<li><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Presencia de s&iacute;ntomas  de disfunci&oacute;n de la rodilla (dolor, sinovitis a repetici&oacute;n, s&iacute;ntomas  mec&aacute;nicos)</font></li>    <li><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Lesi&oacute;n  condral u osteocondral aislada localizada en c&oacute;ndilos, tr&oacute;clea o  r&oacute;tula.</font></li>    <li><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Pacientes  con alienaci&oacute;n tibiofemoral normal o corregible. </font></li>    <li><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Pacientes  con estabilidad de la rodilla normal o corregible.</font></li>    <li><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Presencia  de tejido de menisco funcional (&ge; 50 % de menisco nativo)</font></li>    <li><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Edad  entre 15 a 55 a&ntilde;os. </font></li>    <li><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Por  otra parte, las t&eacute;cnicas de reparaci&oacute;n de cart&iacute;lagos est&aacute;n  contraindicadas en los siguientes pacientes con: <sup>8,17</sup></font></li>    <li><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Artrosis  degenerativa de la rodilla ( afecci&oacute;n de varios compartimentos)</font></li>    <li><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Enfermedad  inflamatoria sist&eacute;mica (artritis reumatoide)</font></li>    <li><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Enfermedades  vasculares y del col&aacute;geno.</font></li>    ]]></body>
<body><![CDATA[<li><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Pacientes  obesos con &iacute;ndice de masa corporal por encima de 35.</font></li>    <li><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Uso  de medicamentos inmunosupresores como los esteroides. </font></li>    </ol>    <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif"><b>T&eacute;cnicas  quir&uacute;rgicas</b></font></p>    <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">La  t&eacute;cnica quir&uacute;rgica paliativa m&aacute;s utilizada en los pacientes  con lesiones de cart&iacute;lagos es el desbridamiento artrosc&oacute;pico. Sin  embargo, esta modalidad se asocia con muy pocos resultados favorables, su indicaci&oacute;n  est&aacute; justificada en pacientes con lesiones grados III y IV de la clasificaci&oacute;n  de Outerbridge RE <sup>2</sup> y un tama&ntilde;o que puede variar de 0,5 a 2  cent&iacute;metros cuadrados, adem&aacute;s de pacientes con 60 a&ntilde;os o  m&aacute;s, con baja demanda f&iacute;sica y afecci&oacute;n de gran parte de  la articulaci&oacute;n. El objetivo de este procedimiento es mejorar la congruencia  articular y minimizar el da&ntilde;o de las superficies articulares no afectadas.  Esta t&eacute;cnica es muy &uacute;til en pacientes con trastornos degenerativos  <sup>8, 17, 19 </sup></font></p>    <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">La  microfractura constituye una de las modalidades de tratamiento que favorece la  reparaci&oacute;n intr&iacute;nseca de la superficie articular. La penetraci&oacute;n  de la placa subcondral provoca sangramiento y por ende la migraci&oacute;n de  c&eacute;lulas con otros factores anab&oacute;licos que favorecen la reparaci&oacute;n  del tejido. <sup>20-22</sup> La microfractura es un procedimiento muy &uacute;til  seg&uacute;n Wright JM et al,<sup>17</sup> por diferentes razones: es una t&eacute;cnica  que est&aacute; bien documentada en la literatura internacional con buenos resultados,  no es dif&iacute;cil desde el punto de vista t&eacute;cnico y no consume mucho  tiempo quir&uacute;rgico, puede ser realizada en un solo tiempo, es llevada a  cabo por lo general mediante la v&iacute;a artrosc&oacute;pica, es barata, no  necesita de zona donante y es aplicada en el mismo acto quir&uacute;rgico cuando  el paciente es intervenido por otras enfermedades: como reconstrucci&oacute;n  de ligamentos cruzados y reparaci&oacute;n de meniscos. <sup>8,17</sup> Esta t&eacute;cnica  puede ser utilizada en pacientes con lesiones de cart&iacute;lagos grados III  y IV seg&uacute;n Outerbridge RE, <sup>2</sup> que conserven la integridad de  la placa subcondral y un &aacute;rea de afecci&oacute;n menor de cuatro cent&iacute;metros  cuadrados de los c&oacute;ndilos femorales y la tr&oacute;clea, en pacientes con  baja demanda f&iacute;sica se puede aplicar esta t&eacute;cnica en lesiones con  mayor &aacute;rea de afecci&oacute;n. 20 Una vez realizada la microfractura se  debe observar la salida de sangre o part&iacute;culas de grasa, este procedimiento  debe ser realizado al final de otras t&eacute;cnicas porque reduce el campo visual  una vez que comienza el sangramiento. La microfractura puede ser combinada con  otras t&eacute;cnicas como: el uso de plasma rico en plaquetas y asociar en un  mismo tiempo quir&uacute;rgico la osteotom&iacute;a correctora en caso de existir  deformidad angular. <sup>23-25</sup></font></p>    <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">La  mosaicoplastia es una t&eacute;cnica que est&aacute; indicada en enfermos con  lesiones focales de cart&iacute;lagos del f&eacute;mur con un &aacute;rea que  var&iacute;a de uno a cuatro cent&iacute;metros cuadrados, es de bajo costo, pero  tiene la desventaja que la zona donante es muy limitada y requiere por lo general  de artrotom&iacute;a. <sup>26-28</sup> Est&aacute; t&eacute;cnica fue descrita  por primera vez por Yamashita F en el a&ntilde;o 1985 citado por Miniaci A et  al <sup>29</sup> y consiste en el traslado de cilindros osteocartilaginosos desde  las zonas de no apoyo de la articulaci&oacute;n patelofemoral y la tr&oacute;clea  hacia las zonas de carga de peso. La mosaicoplastia puede ser realizada en un  solo tiempo quir&uacute;rgico y no tiene riesgo de enfermedades de transmisi&oacute;n  ya que el injerto es aut&oacute;logo. <sup>30,31</sup> </font></p>    <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">El  uso del injerto osteocondral est&aacute; justificado en pacientes con lesiones  de cart&iacute;lagos que pueden ser de tres a 12 cent&iacute;metros cuadrados  o mayores con afecci&oacute;n de la integridad de la placa subcondral. <sup>8,32,33</sup>  Las causas que por lo general provocan lesiones de este tipo son: la necrosis  avascular, osteocondritis disecantes y traumatismos. Las ventajas de este m&eacute;todo  es que permite tratar lesiones de mayor tama&ntilde;o, puede ser utilizada la  fijaci&oacute;n y no necesita de sitio donante del propio enfermo. Por otra parte  tiene la desventaja que se necesita de donantes y conservaci&oacute;n adecuada,  lo que incrementa el costo de manera sustancial, adem&aacute;s del peligro de  transmisi&oacute;n de enfermedades. <sup>34-36</sup></font></p>    <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">El  injerto aut&oacute;logo de condrocitos es otra t&eacute;cnica utilizada en el  tratamiento de pacientes con lesiones de los cart&iacute;lagos grados III y IV,  puede ser aplicada en lesiones con un &aacute;rea de dos hasta 12cm &nbsp;cuadrados,  en todas las zonas de la articulaci&oacute;n de la rodilla y en pacientes con  alta demanda f&iacute;sica. <sup>37-39</sup> Es un procedimiento que puede ser  realizado en uno o dos tiempos, en dependencia del estado de la placa subcondral,  ya que si existe afecci&oacute;n de esta mayor a seis mil&iacute;metros se necesita  de injertos para luego colocar los condrocitos. Las desventajas en la utilizaci&oacute;n  de esta t&eacute;cnica incluyen: alta incidencia de reoperaci&oacute;n y muy costosa.  Es la t&eacute;cnica de elecci&oacute;n cuando han fallado otras como: la microfractura  y la mosaicoplastia. <sup>40-42</sup></font></p>    <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">La  utilizaci&oacute;n de andamios y matrices sint&eacute;ticas asociadas a la siembra  de condrocitos constituyen otras de las t&eacute;cnicas que pueden ser utilizadas  en pacientes con lesiones de los cart&iacute;lagos. <sup>43,44</sup> Estas t&eacute;cnicas  son consideradas como procedimientos de tercera generaci&oacute;n y son superiores  al uso de periostio para la contenci&oacute;n de los condrocitos, debido a que  reducen la hipertrofia operatoria de los tejidos, es menos invasiva, menor tiempo  quir&uacute;rgico y no necesita de sitio donante. Sin embargo son muy costosas  y se necesita de condiciones muy especiales.<sup>45-47</sup></font></p>    ]]></body>
<body><![CDATA[<p>&nbsp;</p>    <p><font size="3" face="Verdana, Arial, Helvetica, sans-serif"><b>CONCLUSIONES</b></font></p>    <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">El  tratamiento de pacientes con lesiones del&nbsp; cart&iacute;lago de la rodilla  depende de varios factores entre los que se encuentran: la edad, demanda f&iacute;sica,  &aacute;rea de la lesi&oacute;n y estado de la placa subcondral.
