Objetivo: mostrar el comportamiento de pacientes con fractura de la meseta tibial. Método: se realizó un estudio observacional descriptivo en el servicio de Ortopedia y Traumatología del Hospital Universitario Manuel Ascunce Domenech de la ciudad de Camagüey, desde mayo de 2009 hasta octubre de 2012. La muestra no probabilística quedó conformada por 48 enfermos que reunieron los criterios de selección. Resultados: predominó el sexo masculino en una razón de 1,4 a 1. Las fracturas del cóndilo externo fueron las más encontradas en el 58,3 %. Las fracturas tipo I y V de Schatzker J fueron las más halladas para un 37,5 % y 20,8 % respectivamente. El tratamiento quirúrgico fue el más utilizado en el 56,2 % de los enfermos. Predominaron los resultados de excelente (22,2 %) y bueno (55,5 %) según la escala de Rasmussen PS. Conclusiones: predominó el sexo masculino en enfermos por debajo de 45 años de edad, el cóndilo tibial lateral fue el más afectado, el tratamiento quirúrgico fue el más utilizado. Los resultados de excelentes y bueno fueron los más encontrados.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[Background: tibial plateau fractures are very common. For treating this kind of fracture, it is necessary to know its mechanism of injury, degree of articular depression, state of associated soft tissue and joint stability. Objective: to show the behavior of patients with tibial plateau fractures. Methods: a descriptive, observational study was conducted in the Orthopedics service at Manuel Ascunce Domenech Provincial Teaching Hospital in Camagüey from May, 2009 to October, 2012. The non-probabilistic sample was composed of 48 patients that fitted the selection criteria. Results: male sex predominated in a male-female proportion of 1, 4 to 1. Fractures of the external condyle were the most frequent in the 58, 3 % of the patients. Fractures type I and V according to Schatzker J. classification were the most common with a 37, 5 % and 20, 8 % respectively. Surgical treatment was the most applied, indicated in the 56, 2 % of the cases. Excellent (22, 2 %) and good (55, 5 %) results according to Rasmussen PS scale predominated. Conclusions: male sex predominated in patients younger than 45 years old. The lateral tibial plateau was the most affected. Surgical treatment was the most used. <body><![CDATA[<p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">La Fractura  de la Meseta Tibial  (FMT) es una de las enfermedades m&aacute;s frecuentes en la articulaci&oacute;n de la  rodilla. Seg&uacute;n Langford JR ,et al, <sup>1</sup> las FMT representan del 1 al 2  % de todas las fracturas y el 8 % de todas las observadas en el anciano. </font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Por  otra parte, Koval KJ ,et al, <sup>2</sup> plantea que alrededor de un 50 % de  los pacientes presenta lesi&oacute;n de los meniscos y un 30 % de los ligamentos  asociados a la fractura, pero su diagn&oacute;stico es dif&iacute;cil debido al edema causado  por el traumatismo, por esta raz&oacute;n existen clasificaciones con patrones  radiogr&aacute;ficos, que permiten sospechar este tipo de lesi&oacute;n. <sup>3,4</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">El  mecanismo de producci&oacute;n de esta afecci&oacute;n traum&aacute;tica responde a los mecanismos  combinados de compresi&oacute;n axial y desviaci&oacute;n angular en valgo o varo. Las  fracturas del c&oacute;ndilo tibial externo son consideradas de bajo grado de energ&iacute;a,  sin embargo las del c&oacute;ndilo medial son consideradas de alta energ&iacute;a. <sup>5-7</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">El  tratamiento de esta afecci&oacute;n traum&aacute;tica es diverso y puede ser conservador o  quir&uacute;rgico en dependencia de varios factores como: edad, grado de depresi&oacute;n  articular, lesiones de partes blandas y estabilidad de la articulaci&oacute;n. <sup>8-10</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Las  modalidades de tratamiento quir&uacute;rgico son diversas con el uso de l&aacute;minas,  tornillos de hueso esponjosos, cortical, canulados. Hoy en d&iacute;a la artroscopia  desempe&ntilde;a un papel muy importante en esta enfermedad, mediante esta t&eacute;cnica de  acceso m&iacute;nimo se logra la reducci&oacute;n asistida de la fractura y la evacuaci&oacute;n de  la sangre acumulada dentro de la articulaci&oacute;n. <sup>11-13</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Es  el objetivo de la investigaci&oacute;n mostrar el comportamiento de pacientes con FMT  en cuanto a: grupo de edades, sexo, c&oacute;ndilo afectado, tipo seg&uacute;n la  clasificaci&oacute;n de Schatzker J, et  al, <sup>14</sup> tipo de tratamiento empleado, medios de fijaci&oacute;n utilizado en  caso de tratamiento quir&uacute;rgico y resultados seg&uacute;n la escala de Rasmussen PS. <sup>15</sup> </font></p>     <p>&nbsp;</p>     <p><font size="3" face="Verdana, Arial, Helvetica, sans-serif"><b>M&Eacute;TODOS</b></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Se realiz&oacute; un estudio observacional descriptivo en  pacientes con el diagn&oacute;stico de FMT ingresados en el servicio de Ortopedia y Traumatolog&iacute;a del Hospital  Universitario Manuel Ascunce Domenech de la ciudad de Camag&uuml;ey, desde mayo de  2009 hasta octubre de 2012, con un total de 44 meses. </font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">El  universo estuvo constituido por 56 pacientes y la muestra no probabil&iacute;stica  qued&oacute; conformada por 48 enfermos que reunieron los criterios de selecci&oacute;n  (inclusi&oacute;n, exclusi&oacute;n).</font></p>     ]]></body>
<body><![CDATA[<p><font size="2" face="Verdana, Arial, Helvetica, sans-serif"><b>Criterios  de inclusi&oacute;n:</b></font></p> <ol>       <li><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Pacientes  mayores de 18 a&ntilde;os de edad.</font></li>       <li><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Pacientes  ingresados en el servicio de Ortopedia y Traumatolog&iacute;a. </font></li>       <li><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Pacientes  que fueron tratados mediante alguna modalidad de tratamiento quir&uacute;rgico o  conservador.</font></li>       <li><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Seguimiento  de al menos 12 meses.</font></li>     </ol>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif"><b>Criterios  de exclusi&oacute;n</b>:</font></p> <ol>       <li><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Pacientes  con fracturas de meseta tibial asociada a otras fracturas o enfermedades  traum&aacute;ticas ipsilaterales.</font></li>     </ol>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif"><b>Criterios  de salida:</b></font></p> <ol>       ]]></body>
<body><![CDATA[<li><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Abandono  voluntario del estudio.</font></li>       <li><font size="2" face="Verdana, Arial, Helvetica, sans-serif">No cumplimiento del tratamiento e irregularidades  en la asistencia a las consultas. </font></li>     </ol>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">No  se encontraron pacientes durante la investigaci&oacute;n que reunieran los criterios  de salida. </font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Para  recopilar la informaci&oacute;n de confeccion&oacute; un formulario con las siguientes  variables: edad, sexo, miembro afectado, mecanismo de producci&oacute;n, platillo  tibial afectado, clasificaci&oacute;n de la fractura seg&uacute;n Schatzker J ,et al, 14  tipo de tratamiento, materiales quir&uacute;rgicos empleados, complicaciones y  funcionabilidad de la rodilla seg&uacute;n la clasificaci&oacute;n de Rasmussen PS. 15 </font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Para el  procesamiento de la informaci&oacute;n se utiliz&oacute; el programa estad&iacute;stico SPSS 15.0.1. Los resultados se mostraron en tablas y  gr&aacute;ficos. </font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif"><b>&nbsp;</b></font></p>     <p><font size="3" face="Verdana, Arial, Helvetica, sans-serif"><b>RESULTADOS</b></font></p>     <p align="justify"><span style='font-size:10.0pt;font-family:Verdana'>Predomin&oacute; el sexo masculino en relaci&oacute;n al femenino con una raz&oacute;n de 1,4 a 1. Los grupos de edades con mayor frecuencia fueron los de 45 a 59 y 30 a 44 a&ntilde;os con un 47,9 % y 25 % respectivamente. <body><![CDATA[<p align="justify"><span style="font-size:10.0pt;font-family:Verdana">Las fracturas del   c&oacute;ndilo externo fueron las m&aacute;s frecuentes en el 58,3 % de los   enfermos, seguida de las fracturas bicondilares en un 31,3 %. Las fracturas del   c&oacute;ndilo interno representaron solo el 10,4 %. <p align="justify"><span style="font-size:10.0pt;font-family:Verdana">En relaci&oacute;n al comportamiento seg&uacute;n la clasificaci&oacute;n de Schatzker J ,et al, 14 se observ&oacute; un predominio de las fracturas tipo I en un 37,5 %. Los restantes grupos mostraron las siguientes porcentajes, tipo II 8,3 %, tipo III 12,5 %, tipo IV 10,4 %, tipo V 20,8 % y tipo VI 10,4 %. <p align="justify"><span style="font-size:10.0pt;font-family:Verdana">El tratamiento quir&uacute;rgico fue el m&aacute;s empleado y represent&oacute; el 56,2 % de los pacientes. Por otra parte el conservador fue empleado en el 43,8 %. <p align="justify"><span style="font-size:10.0pt;font-family:Verdana">De los 27 pacientes que requirieron de tratamiento quir&uacute;rgico, en el 48,1 % se utiliz&oacute; la placa AO, en el 40,7 % el perno de Webb, los tornillo de compresi&oacute;n fueron empleados en el 37 %. De los enfermos intervenidos el 14,8 % necesit&oacute; de injerto &oacute;seo. <p align="justify"><span style="font-size:10.0pt;font-family:Verdana">Al aplicar la escala evaluativa de Rasmussen PS 15 en relaci&oacute;n al tratamiento conservador o quir&uacute;rgico se observ&oacute; un mayor porcentaje de pacientes con evaluaci&oacute;n de excelente (22,2 %) y bueno (55,5 %). Por otra parte, los resultados de regular (52,3 %) y malo (19 %) mostraron mayor frecuencia en los pacientes que recibieron el tratamiento conservador. <body><![CDATA[<p align="justify">&nbsp;</p>     <p><font size="3" face="Verdana, Arial, Helvetica, sans-serif"><b>DISCUSI&Oacute;N</b></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">El  sexo masculino es el m&aacute;s afectado por esta enfermedad traum&aacute;tica por debajo de  los 45 a&ntilde;os, los resultados de la investigaci&oacute;n concuerdan con los planteados  por Ali AM, <sup>16</sup> Persiani P, et al, <sup>17</sup> y Dall'oca, et al. <sup>18</sup>  En relaci&oacute;n a la edad esta afecci&oacute;n ocurre por lo general en enfermos entre 45  y 59 a&ntilde;os con una mayor predilecci&oacute;n por el sexo femenino. </font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">El  platillo tibial externo es el m&aacute;s afectado y este hecho responde a razones  anat&oacute;micas como: desviaci&oacute;n fisiol&oacute;gica en valgo, que es m&aacute;s marcado en el sexo  femenino, el c&oacute;ndilo femoral externo tiene forma rectangular, lo que act&uacute;a como  un fulcro sobre la meseta tibial a la compresi&oacute;n axial y el trabeculado del platillo  tibial&nbsp; interno es m&aacute;s fuerte en relaci&oacute;n  al del platillo tibial externo. <sup>19-21</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Las  fracturas que afectan el platillo tibial externo con peron&eacute; intacto por lo  general no colapsan secundariamente. Por otra parte, en caso de fracturas  bicond&iacute;leas o de ambos platillos usualmente no colapsan o se angulan cuando existe  fractura del peron&eacute;. <sup>22-24</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">La  clasificaci&oacute;n de Schatzker J, et al, <sup>14</sup> es de gran utilidad al permitir  mediante su aplicaci&oacute;n conocer la posibilidad de lesiones asociadas de partes  blandas como: los meniscos y ligamentos. <sup>25-27</sup> El predominio en los  tipos del I al III encontrado en la investigaci&oacute;n concuerda con lo planteado  por Raza H, et al, <sup>28</sup> y Sament R, et al. <sup>29</sup> </font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">En la antig&uuml;edad, esta enfermedad  traum&aacute;tica era tratada por una gran cantidad de cirujanos ortop&eacute;dicos de forma  conservadora, mediante tracci&oacute;n esquel&eacute;tica y enyesados. Sin embargo, la  conducta respecto al tratamiento ha cambiado considerablemente a partir de los  a&ntilde;os 80, donde hasta la actualidad la gran mayor&iacute;a de los autores prefiere el  tratamiento quir&uacute;rgico, debido a sus resultados superiores basados en una mayor  movilidad articular, alineaci&oacute;n en el eje mec&aacute;nico de la extremidad y  correcci&oacute;n de la superficie articular. <sup>30-32</sup></font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Los objetivos  fundamentales del tratamiento son obtener como resultado una articulaci&oacute;n  estable, m&oacute;vil, alineada, no dolorosa y disminuir en lo posible el desarrollo  de la artrosis postraum&aacute;tica. <sup>33-35</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Para  evaluar la decisi&oacute;n del tipo de tratamiento es de gran utilidad los criterios  de Schatzker J, et al,  <sup>14</sup> los que se describen a continuaci&oacute;n: los pacientes con FMT  inmovilizados por un periodo mayor de cuatro semanas usualmente traen  aparejadas rigidez articular, la reducci&oacute;n cruenta y osteos&iacute;ntesis asociada a  inmovilizaci&oacute;n crean a&uacute;n m&aacute;s rigidez articular, independientemente del m&eacute;todo o  t&eacute;cnica utilizada la movilizaci&oacute;n de la rodilla debe empezar lo antes posible,  mientras tanto se preserva la movilidad articular los procedimientos de  reconstrucci&oacute;n secundarios son factibles a realizar, los fragmentos articulares  impactados no pueden ser reducidos por tracci&oacute;n o manipulaci&oacute;n por si solos ya  que los mismos no est&aacute;n adosados a las partes blandas que los pueden levantar o  reducir, los defectos articulares no se rellenan con cart&iacute;lago hialino y s&iacute;  permanecen provocan depresi&oacute;n articular e inestabilidad. <sup>36-38</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Los tipos de implantes  utilizados dependen de la configuraci&oacute;n de la fractura, del adiestramiento de  los cirujanos en las diferentes t&eacute;cnicas de tratamiento y de la disponibilidad  de la instituci&oacute;n. <sup>39-40</sup></font></p>     ]]></body>
<body><![CDATA[<p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">La clasificaci&oacute;n de  Rasmussen PS <sup>15</sup> es empleada en la literatura con frecuencia para  pacientes con FMT, lo que es apoyado por Chiu CH ,et al,<sup>41</sup> Malakasi  A, et al, <sup>42</sup> Mankar SH, et al, <sup>43</sup> El predominio de los  resultados favorables en la investigaci&oacute;n enti&eacute;ndase aquellos favorables los  enfermos con evaluaciones de excelente y bueno, son debidos a que predomin&oacute; la  aplicaci&oacute;n del tratamiento quir&uacute;rgico que a la vez permiti&oacute; la incorporaci&oacute;n  m&aacute;s temprana de los enfermos a la rehabilitaci&oacute;n. </font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif"><b>&nbsp;</b></font></p>     <p><font size="3" face="Verdana, Arial, Helvetica, sans-serif"><b>CONCLUSIONES</b></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">La FMT  es una entidad traum&aacute;tica observada con regularidad en los servicios de  Ortopedia y Traumatolog&iacute;a.
