Las lesiones intrarticulares indetectables a simple vista son las responsables de los resultados de estos tratamientos. Para este tipo de lesiones la artroscopia ofrece ventajas diagnósticas y terapéuticas. Objetivo: realizar una revisión sobre el tratamiento artroscópico de pacientes con lesiones traumáticas del tobillo. Métodos: se realizó una revisión bibliográfica de un total de 330 artículos publicados en Pubmed, Hinari y Medline mediante el localizador de información Endnote. Se utilizaron 50 citas seleccionadas para realizar la revisión, 44 de ellas de los últimos cinco años y se incluyeron cuatro libros. Desarrollo: se analizaron las ventajas de la artroscopia en la articulación del tobillo en las lesiones traumáticas, sus indicaciones y contraindicaciones. Se describió la técnica quirúrgica a emplear apoyado en dos imágenes propias del autor. Se abordaron las enfermedades traumáticas que más inciden en la articulación, así como sus peculiaridades. Conclusiones: la artroscopia como tratamiento complementario en lesiones traumáticas del tobillo ofrece ventajas al permitir el diagnóstico y tratamiento de lesiones intrarticulares que interfieren en el resultado funcional de los enfermos.]]></p></abstract>

Objective: to make a review on the arthroscopic treatment of patients with traumatic lesions of the ankle. Methods: a bibliographic review of 330 articles published in Pubmed, Hinari and Medline was made by means of the reference management software Endnote. Fifty citations were selected to make the review, 44 of them were from the last five years; four books were included. Development: the advantages of arthroscopy in the ankle joint to treat traumatic lesions, its indications and contraindications were analyzed. The surgical technique to use, based on two pictures by the author, was described. The traumatic diseases that most affect this joint were discussed, as well as its peculiarities. Las  fracturas que afectan esta articulaci&oacute;n son las relacionadas con los mal&eacute;olos  de la tibia y el peron&eacute;, adem&aacute;s de las del pil&oacute;n tibial y el astr&aacute;galo. Por  otra parte existen lesiones sobre las partes blandas como las que afectan la  sindesmosis tibioperonea. <sup>1-3</sup></font></p>     ]]></body>
<body><![CDATA[<p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">El mecanismo de  producci&oacute;n de estas enfermedades es muy amplio: compresiones axiales,  rotaciones y cizallamiento. La sintomatolog&iacute;a est&aacute; reflejada por la presencia  inmediata de dolor e impotencia funcional, que impide la marcha de los enfermos  acompa&ntilde;ado de limitaci&oacute;n o incapacidad para la funci&oacute;n articular. <sup>4-6</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">El tratamiento de estos  padecimientos es por lo general de tipo quir&uacute;rgico y aunque en la mayor&iacute;a de  las ocasiones se logra la reducci&oacute;n anat&oacute;mica, no siempre el resultado es el  esperado. <sup>7-9</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Dentro de las  posibles causas de resultados desfavorables a pesar de la reducci&oacute;n anat&oacute;mica  se encuentra la presencia de lesiones intrarticulares, que pasan inadvertidas y  son responsables de dolor despu&eacute;s del tratamiento quir&uacute;rgico. <sup>10-12</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">La artroscopia de  la articulaci&oacute;n del tobillo permite detectar y dar soluci&oacute;n a un gran n&uacute;mero de  lesiones asociadas aparentemente ocultas, entre las que se encuentran lesiones  osteocondrales, cuerpos libres y de la sindesmosis tibioperonea. <sup>13-15</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Hasta la actualidad  no existen investigaciones que demuestren la superioridad del tratamiento al  emplear la artroscopia, de all&iacute; que debido a ser una lesi&oacute;n frecuente y no  existir un consenso sobre el uso de la artroscopia en las lesiones traum&aacute;ticas  que afectan la articulaci&oacute;n del tobillo, los autores de este trabajo se  proponen llevar a cabo una actualizaci&oacute;n sobre este tema.</font></p>     <p align="justify">&nbsp;</p>     <p align="justify"><font size="3" face="Verdana, Arial, Helvetica, sans-serif"><b>M&Eacute;TODOS</b></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Se realiz&oacute; una revisi&oacute;n bibliogr&aacute;fica de un  total de 330 art&iacute;culos publicados en Pubmed (130 at&iacute;culos), Hinari (110  art&iacute;culos) y Medline (90 art&iacute;culos) mediante el localizador de informaci&oacute;n  Endnote, de ellos se utilizaron 50 citas seleccionadas para realizar la  revisi&oacute;n, 44 de ellas de los &uacute;ltimos cinco a&ntilde;os donde se incluyeron cuatro  libros.</font></p>     <p align="justify">&nbsp;</p>     <p align="justify"><font size="3" face="Verdana, Arial, Helvetica, sans-serif"><b>DESARROLLO</b></font></p>     ]]></body>
<body><![CDATA[<p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Las ventajas de la  artroscopia en las lesiones traum&aacute;ticas del tobillo (LTT) no solo est&aacute;n basadas  en la detecci&oacute;n de lesiones intrarticulares y su tratamiento, sino que adem&aacute;s,  permite la reducci&oacute;n asistida de las fracturas y eval&uacute;a las causas de dolor  cr&oacute;nico con o sin p&eacute;rdida del movimiento articular. <sup>16-18</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Las indicaciones de  la artroscopia en las fracturas y lesiones de partes blandas que afectan la  articulaci&oacute;n del tobillo seg&uacute;n Ferkel RD <sup>19</sup> son: desplazamiento  m&iacute;nimo o reductible mediante manipulaci&oacute;n, presencia de edema ligero o  moderado, ausencia de da&ntilde;o neurovascular y aunque controversial en todas las  fracturas que afectan la articulaci&oacute;n del tobillo en busca de cuerpos libres  articulares, lesiones osteocondrales y de ligamentos.</font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Las  contraindicaciones para la artroscopia en estas lesiones seg&uacute;n Ferkel RD, et al,  <sup>20</sup> son: edema severo de la articulaci&oacute;n, presencia de da&ntilde;o  neurovascular, fracturas abiertas y muy desplazadas.</font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">La t&eacute;cnica  quir&uacute;rgica para realizar la artroscopia en las LTT consiste en realizar primero  la artroscopia del tobillo con el paciente en dec&uacute;bito supino bajo anestesia  general o regional, la pierna esta sujetada con un soporte, que permite la  posici&oacute;n de 90 grados de la rodilla. Se coloca isquemia neum&aacute;tica a nivel del  muslo, para garantizar la visualizaci&oacute;n de las superficies articulares. <sup>21-23</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">El m&eacute;todo de  distracci&oacute;n utilizado es el no invasivo, mediante cuerdas comercializadas o con  asa de vendaje est&eacute;ril. Mediante una guja n&uacute;mero 18 se identifica el  portal anteromedial a trav&eacute;s del cual se instila dentro de la articulaci&oacute;n de 10 a 15 mil&iacute;metros de  soluci&oacute;n salina, para lograr la distensi&oacute;n articular. Una vez  identificado este portal, despu&eacute;s de la incisi&oacute;n de piel, se introduce el  trocar dentro de la articulaci&oacute;n en sentido al mal&eacute;olo peroneo. Los  artroscopios m&aacute;s empleados son de 2, 7 y 4 mil&iacute;metros y se  emplea la gravedad, para colocar las soluciones irrigadoras. Los otros portales  m&aacute;s empleados son el anterolateral y el posterolateral (<a href="#figura1">figura 1</a> <a href="#figura2">y 2</a>). <sup>24-26</sup></font></p>     <p align="center"><img src="/img/revistas/amc/v20n1/f01130116.jpg" alt="figura 1" width="330" height="311" longdesc="../img/f01130116.jpg"><a name="figura1"></a></p>     <p align="center"><img src="/img/revistas/amc/v20n1/f02130116.jpg" alt="figura 2" width="346" height="340" longdesc="../img/f02130116.jpg"><a name="figura2"></a></p>     <p align="center">&nbsp;</p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">El primer  procedimiento es la extracci&oacute;n de la hemartrosis y el tejido fibroso dentro de  la articulaci&oacute;n, luego se visualiza el labio anterior de la tibia, mal&eacute;olo  peroneo, ligamentos laterales, el astr&aacute;galo en su regi&oacute;n lateral, sindesmosis  tibioperonea, regi&oacute;n central de astr&aacute;galo, pil&oacute;n tibial posterior, mal&eacute;olo  medial, ligamento deltoideo y regi&oacute;n medial del astr&aacute;galo. Los hallazgos m&aacute;s  encontrados son: disrupci&oacute;n del ligamento tibioperoneo antero-inferior, cuerpos  libres articulares, lesiones osteocondrales que provienen del astr&aacute;galo y  platillo tibial, interposici&oacute;n del tend&oacute;n tibial posterior en las fracturas del  mal&eacute;olo medial y fragmentaci&oacute;n del labio posterior del pil&oacute;n tibial con  disrupci&oacute;n del ligamento tibioperoneo posterior. Una vez terminada la  artroscopia se retira el dispositivo de tracci&oacute;n y el soporte de la pierna,  para realizar la reducci&oacute;n cruenta y osteos&iacute;ntesis. <sup>27-29</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">A continuaci&oacute;n se  abordan los padecimientos traum&aacute;ticos m&aacute;s frecuentes en la articulaci&oacute;n del  tobillo en las que se emplea la artroscopia.</font></p>     ]]></body>
<body><![CDATA[<p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif"><b>Disrupci&oacute;n  de la sindesmosis tibioperonea    <br> </b></font><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Seg&uacute;n Bonasia DE,  et al, <sup>30</sup> la disrupci&oacute;n de la sindesmosis tibioperonea ocurre del 47  % al 66 % de los pacientes que tienen fractura del tobillo, que de no ser  tratadas evolucionan hacia el dolor cr&oacute;nico de la articulaci&oacute;n del tobillo. El  mecanismo de producci&oacute;n que aporta la mayor cantidad de pacientes es el causado  por: supinaci&oacute;n y rotaci&oacute;n externa basado en la clasificaci&oacute;n de Lauge Hansen N  o en las fracturas tipos B y C del sistema de Danis R Weber BG citadas por Chen  DW, et al. <sup>31</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Las lesiones de los  ligamentos son m&aacute;s frecuentes y severas en el sexo masculino; y tienen peor  pron&oacute;stico cuando ocurren por debajo de los 30 a&ntilde;os o encima de los 60. <sup>32, 33</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">El diagn&oacute;stico de  esta afecci&oacute;n traum&aacute;tica se realiza mediante ex&aacute;menes radiogr&aacute;ficos  preoperatorios en vistas anteroposterior, lateral y de mortaja, adem&aacute;s de las  vistas de estr&eacute;s transoperatorias. Sin embargo, el examen artrosc&oacute;pico es el  m&aacute;s confiable de todos. <sup>34, 35</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">El tratamiento  quir&uacute;rgico de esta enfermedad est&aacute; justificado cuando existen evidencias  radiogr&aacute;ficas y artrosc&oacute;picas de franca separaci&oacute;n tibioperonea, el m&eacute;todo  empleado es el desbridamiento artrosc&oacute;pico de la zona, seguido de la fijaci&oacute;n  con uno o dos tornillos del sistema AO a trav&eacute;s de la sindesmosis. <sup>36-38</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Despu&eacute;s del  tratamiento quir&uacute;rgico, el paciente es inmovilizado por seis semanas, despu&eacute;s  del cual se realiza el apoyo parcial del peso corporal. Los tornillos son  retirados de ocho a 10 semanas. <sup>37, 38</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif"><b>Fracturas  del mal&eacute;olo peroneo y tibial</b>    <br> El tratamiento de  esta enfermedad es quir&uacute;rgico y est&aacute; basado en la reducci&oacute;n cruenta y  osteos&iacute;ntesis. En un estudio realizado por Ono A, et al, <sup>24</sup> de 105  pacientes, 54 presentaron lesi&oacute;n de los ligamentos y 21 de los cart&iacute;lagos, de  all&iacute; la importancia de la artroscopia para identificar y tratar estas  afecciones articulares, para evitar el dolor cr&oacute;nico del tobillo.</font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">En caso de fractura  del mal&eacute;olo medial es importante descartar la interposici&oacute;n del tend&oacute;n del  tibial posterior, que dificulta la reducci&oacute;n. <sup>39, 40</sup>    <br>   Mediante la  artroscopia se evacua la sangre intrarticular y los fragmentos libres, adem&aacute;s  que permite la reducci&oacute;n asistida de la fractura, la que tiene las siguientes  ventajas: exposici&oacute;n m&iacute;nima, preserva la irrigaci&oacute;n sangu&iacute;nea y facilita  identificar lesiones asociadas. Sin embargo, se necesita un mayor tiempo  quir&uacute;rgico y entrenamiento. <sup>41, 42</sup></font></p>     ]]></body>
<body><![CDATA[<p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif"><b>Fracturas  osteocondrales    <br> </b>Los fragmentos  osteocondrales provienen con mayor frecuencia del astr&aacute;galo o platillo tibial. <sup>43</sup>  En caso de afecci&oacute;n del astr&aacute;galo el 58 % son postero-mediales, profundas en  forma de copa, las anterio-laterales ocupan el 42 % y son superficiales y  delgadas. <sup>44</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">El diagn&oacute;stico  cl&iacute;nico de esta enfermedad en un paciente con fractura del tobillo resulta  dif&iacute;cil, por lo que es necesario un alto &iacute;ndice de sospecha. La radiograf&iacute;a  simple tiene solo un 50 % de efectividad para este diagn&oacute;stico, por lo que se  necesita de estudios imaginol&oacute;gicos m&aacute;s sofisticados como la imagen de  resonancia magn&eacute;tica o la tomograf&iacute;a axial computarizada. <sup>43, 44 </sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">En relaci&oacute;n a las  clasificaciones empleadas para la osteocondrtis del astr&aacute;galo, la m&aacute;s usada es  la descrita por Berndt AL y Harty M, <sup>45 </sup>que las divide en cuatro  grados.    <br>   El tratamiento de  las lesiones osteocondrales es diferente a las lesiones cr&oacute;nicas, de all&iacute; que  el primer paso es hacer el diagn&oacute;stico diferencial entre estas dos enfermedades.  <sup>44, 46</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Las lesiones agudas  del tipo del I y II son tratadas de forma conservadora, no as&iacute; las tipo III y  IV, en las tipo III se realiza la fijaci&oacute;n con tornillos canulados y/o  biodegradables, en caso de lesiones tipo IV, si son menores de dos cent&iacute;metros  se emplea el curetaje, la microfractura o las perforaciones retrogradas. Las  lesiones por encima de dos cent&iacute;metros son tratadas mediante cirug&iacute;a abierta  con injerto osteocondral o el injerto de condrocitos de forma diferida. <sup>45-47</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif"><b>Fracturas  del pil&oacute;n tibial</b>    <br> El uso de la  artroscopia en pacientes con fracturas del pil&oacute;n tibial no es muy difundido en  la literatura cient&iacute;fica actual. Sin embargo, existen art&iacute;culos que justifican  el empleo de esta modalidad en especial en fracturas tipo II seg&uacute;n la  clasificaci&oacute;n de R&uuml;edi  T y Allg&ouml;wer M, citado por Schweigkofler U, et  al. <sup>48</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Las ventajas de  esta t&eacute;cnica est&aacute;n basadas en que permite evacuar la hemartrosis, la extracci&oacute;n  de cuerpos libres articulares, garantiza la visualizaci&oacute;n de la superficie  articular para la reducci&oacute;n y preserva la irrigaci&oacute;n sangu&iacute;nea. Para las  fracturas con depresi&oacute;n de la superficie articular se emplea una gu&iacute;a de  ligamento cruzado anterior, de manera similar como se realiza en pacientes con  facturas del platillo tibial. <sup>49</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Seg&uacute;n Lonjon G, et  al, <sup>50</sup> la combinaci&oacute;n de la artroscopia con la reducci&oacute;n y  osteos&iacute;ntesis tiene una menor incidencia de rigidez articular posoperatoria, al  compararlos con los resultados de los enfermos a los que no se les realiz&oacute; esta  t&eacute;cnica.</font></p>     ]]></body>
<body><![CDATA[<p align="justify">&nbsp;</p>     <p align="justify"><font size="3" face="Verdana, Arial, Helvetica, sans-serif"><b>CONCLUSIONES</b></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Las lesiones &oacute;sea y  de ligamentos a nivel de la articulaci&oacute;n del tobillo son frecuentes.
