Objetivo: profundizar sobre la importancia de la fijación externa combinada con la reducción y fijación del peroné en el tratamiento de pacientes con fractura del pilón tibial. Métodos: se realizó una revisión bibliográfica de un total de 326 artículos publicados en Pubmed, Hinari, Scielo y Medline mediante el localizador de información Endnote, de ellos se utilizaron 51 citas seleccionadas para realizar la revisión, 49 de ellas de los últimos cinco años donde se incluyeron seis libros. Desarrollo: se discuten los aspectos relacionados con el mecanismo de producción, el diagnóstico clínico e imaginológico y las clasificaciones más empleadas. Se abordan las ventajas de la fijación externa, así como las de la fijación del peroné asociada a la fijación externa. En relación a las complicaciones se describen las más importantes divididas en las relacionadas con la fijación externa y propias de la fracturas. Conclusiones: el empleo de la fijación externa asociada a la osteosíntesis del peroné constituye un método aplicable y reproducible, que ofrece resultados satisfactorios a pacientes que sufren de fractura del pilón tibial. Al seguir los pasos de esta modalidad de tratamiento la incidencia de complicaciones es baja y se logran altos índices de consolidación ósea.]]></p></abstract>

Some of them are conservative and others are surgical. External fixation plays a key role in the latter, with a great number of advantages. Objective: to deepen the importance of external fixation combined with fibular reduction and fixation in the treatment of patients with tibial pilon fracture. Methods: a bibliographic review of 326 published articles in Pubmed, Hinari, Scielo and Medline was carried out through software program Endnote. From that amount, 51 selected citations were used to conduct the review, and 49 of them are from the last five years, including six books. Development: aspects related to the production mechanism, imaging clinical diagnosis, and the most used classifications were discussed. Advantages of external fixation were tackled, as well as the advantages linked with fibular fixation together with external fixation. Complications were described and divided into ones related to external fixation and other to fractures. Conclusions: the use of external fixation associated with fibular osteosynthesis represents an applicable method, which offers satisfactory outcomes in patients with tibial pilon fracture. When following the steps of this kind of treatment, complications decrease and high rates of bone consolidation are achieved.]]></p></abstract>

La  primera descripci&oacute;n de esta lesi&oacute;n fue realizada por el radi&oacute;logo Franc&eacute;s  Destot en el a&ntilde;o 1911. Posteriormente en el a&ntilde;o 1968 R&uuml;edi publica la primera  serie de casos y propone los principios del tratamiento, as&iacute; como la  clasificaci&oacute;n que lleva su nombre y es empleada en la actualidad. <sup>1-3</sup></font></p>     ]]></body>
<body><![CDATA[<p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Seg&uacute;n Egol KA, et al, <sup>4</sup> las FPT  ocupan una incidencia del 7 % al 10 % de todas las fracturas de tibia, ocurren  como resultado de traumatismos de alta energ&iacute;a por lo que es necesario  descartar lesiones asociadas, que por lo general son muy frecuentes; las FPT son  m&aacute;s frecuentes en el sexo masculino de 30 a 40 a&ntilde;os de edad.</font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">El cuadro cl&iacute;nico de los pacientes que  sufren de FPT es variado y depende de la intensidad del trauma. Un gran  porcentaje de pacientes presentan fracturas abiertas, que complejizan a&uacute;n m&aacute;s  el tratamiento. Por otra parte, las lesiones de partes blandas son muy  frecuentes y es com&uacute;n observar afecciones de la piel y del paquete  vasculo-nervioso. <sup>5-7</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Las modalidades de tratamiento son m&uacute;ltiples  y van desde la conservadora a la quir&uacute;rgica, esta &uacute;ltima aporta los mejores  resultados y pueden ser mediante el uso de placas AO, alambres de Kirschner,  fijaci&oacute;n externa (FE) o la combinaci&oacute;n de todas estas variantes, adem&aacute;s de la  artrodesis del tobillo de forma primaria o diferida. <sup>8-10</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">La   FE puede ser empleada de forma definitiva o  transitoria y ofrece excelentes resultados para pacientes que sufren de FPT. <sup>11-13</sup>    <br>   El objetivo de este trabajo es profundizar  sobre la importancia de la FE  combinada con la reducci&oacute;n y fijaci&oacute;n del peron&eacute; en el tratamiento de pacientes  con FPT.</font></p>     <p align="justify">&nbsp;</p>     <p align="justify"><font size="3" face="Verdana, Arial, Helvetica, sans-serif"><b>M&Eacute;TODOS</b></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">La b&uacute;squeda de la informaci&oacute;n se realiz&oacute; en  un periodo de tres meses (1ro de noviembre de 2015 al 31 de enero de 2016) y se  emplearon las siguientes palabras: plafound fractures, pilon fractures, a partir de la informaci&oacute;n obtenida se realiz&oacute; una  revisi&oacute;n bibliogr&aacute;fica de un total de 326 art&iacute;culos publicados en las bases de  datos Pubmed, Hinari, Scielo y Medline mediante el gestor de b&uacute;squeda y  administrador de referencias EndNote, de ellos se utilizaron 51 citas seleccionadas  para realizar la revisi&oacute;n, 49 de ellas de los &uacute;ltimos cinco a&ntilde;os donde se  incluyeron seis libros y una cita del propio autor.</font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Se consideraron estudios de pacientes con FPT  tratados por FE de forma aislada o asociada a fijaci&oacute;n interna del peron&eacute;, as&iacute;  como el uso de la FE  de manera transitoria o definitiva. Se excluyeron las investigaciones de  pacientes tratados mediante reducci&oacute;n interna con placas y tornillos, as&iacute; como la  fijaci&oacute;n intramedular.</font></p>     <p align="justify">&nbsp;</p>     ]]></body>
<body><![CDATA[<p align="justify"><font size="3" face="Verdana, Arial, Helvetica, sans-serif"><b>DESARROLLO</b></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Los mecanismos de producci&oacute;n para las FPT son  de tres tipos fundamentales, el primero por fuerzas de compresi&oacute;n axial (alta  energ&iacute;a) en el cual el astr&aacute;galo impacta en la superficie articular de la  tibia, se asocia por lo general a conminaci&oacute;n significativa. El segundo  mecanismo es provocado por un mecanismo de rotaci&oacute;n (baja energ&iacute;a) la torsi&oacute;n  se acompa&ntilde;a de fuerzas de estr&eacute;s en varo o valgo, lo que provoca la presencia  de dos o m&aacute;s fragmentos con m&iacute;nima conminaci&oacute;n articular, la fractura del  peron&eacute; es por lo general de tipo transversal u oblicua corta. El tercer  mecanismo es debido a la combinaci&oacute;n de los dos primeros. <sup>14-16</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">En pacientes con trauma de alta energ&iacute;a se  deben descartar otras lesiones como: fracturas del calc&aacute;neo, platillo tibial,  pelvis y vertebrales. <sup>17-19</sup> </font><font size="2" face="Verdana, Arial, Helvetica, sans-serif">El cuadro cl&iacute;nico est&aacute; en dependencia de la  intensidad del traumatismo. A la inspecci&oacute;n se pueden detectar, deformidades  rotacionales y angulaciones acompa&ntilde;adas en ocasiones por heridas que comunican  con el foco de fractura. Tambi&eacute;n es frecuente observar flictenas de contenido  claro o sanguinolento, estas &uacute;ltimas altamente sugestiva de trauma de gran  energ&iacute;a. <sup>20-22</sup>    <br> Mediante la palpaci&oacute;n se detecta crepitaci&oacute;n  y movilidad anormal. Es de vital importancia el examen neuro-vascular, para  descartar este tipo de lesiones asociadas. <sup>23-25</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">El examen imaginol&oacute;gico incluye: la radiograf&iacute;a  simple en vistas anteroposterior, lateral y de mortaja tibio-peronea, adem&aacute;s de  la tomograf&iacute;a axial computarizada &uacute;til en la evaluaci&oacute;n de la integridad de la  superficie articular y determinar el tipo de fractura. <sup>26-28</sup>    <br>   La clasificaci&oacute;n propuesta por R&uuml;edi TP y  Allgower M citado por Barei DP <sup>29</sup> est&aacute;  basada en el grado de conminuci&oacute;n y desplazamiento de la superficie articular,  esta clasificaci&oacute;n es &uacute;til para determinar el pron&oacute;stico, el que empeora a  medida que aumenta el grado de la clasificaci&oacute;n. Se divide en tres tipos, tipo  1- fracturas no desplazadas, tipo 2- fracturas desplazadas con impactaci&oacute;n y  conminuci&oacute;n m&iacute;nima, tipo 3- fracturas desplazadas con impactaci&oacute;n metafisaria y  conminuci&oacute;n articular significativas.</font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Por otra parte la clasificaci&oacute;n del grupo AO  es una de las m&aacute;s empleadas en la actualidad, de manera general esta subdividida  en tres grandes grupos, primero extrarticulares, parcialmente articulares y  articulares, a la vez estos grupos se dividen en otros subgrupos seg&uacute;n el grado  de complejidad de la fractura. <sup>30-32</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">El uso de la FE en pacientes con FPT tiene diferentes ventajas  como: estabilizaci&oacute;n del foco de fractura, control rotacional, aplicaci&oacute;n en  pacientes con fracturas abiertas, lo que permite el lavado y desbridamiento  continuo de la zona afectada; empleo en pacientes que sufren trauma de alta  energ&iacute;a con lesiones de partes blandas, lo que ayuda a la cicatrizaci&oacute;n; <sup>33-35</sup>  la colocaci&oacute;n de los pines respetan el foco de fractura y por ende no  intervienen en el proceso de consolidaci&oacute;n de manera negativa; permite el uso  de otros m&eacute;todos de osteos&iacute;ntesis tanto del peron&eacute; como la reducci&oacute;n de la  superficie articular de la tibia en caso de conminuci&oacute;n y desplazamiento;  adem&aacute;s una vez colocada la FE  se puede utilizar como estabilizador y distractor para colocar otros medios de  fijaci&oacute;n definitivos como placas y tornillos, adem&aacute;s permite el uso de la  artroscopia para detectar lesiones osteocondrales y visualizar la reducci&oacute;n de  las superficies intrarticulares. <sup>36-38</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">En relaci&oacute;n a si usar la FE de forma transitoria o  definitiva depende del grado de reducci&oacute;n logrado, ya que si se obtiene una  alineaci&oacute;n adecuada este m&eacute;todo puede ser empleado de manera definitiva seg&uacute;n  plantea Egol KA, et al. <sup>4</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">La FE empleada para pacientes  con FPT puede ser dividida en articulada y no articulada, para lo cual se  emplean un gran n&uacute;mero de dise&ntilde;os de fijadores. La FE no articulada ha sido la m&aacute;s  empleada y es la que mas se usa en nuestro medio en conjunto con la estabilizaci&oacute;n  del peron&eacute; ya sea con alambre de Rush o l&aacute;mina AO de 3, 5 o 4, 5 mil&iacute;metros.  Por su parte el fijador articulado permite el movimiento de la articulaci&oacute;n en  el plano sagital, lo que de forma te&oacute;rica favorece la lubricaci&oacute;n condral. El porcentaje  de consolidaci&oacute;n &oacute;sea es alto y tiene un rango del 75 % al 81 %(<a href="#figura1">figura 1</a>). <sup>39-41</sup></font></p>     ]]></body>
<body><![CDATA[<p align="center"><img src="http://revistaamc.sld.cu/index.php/amc/manager/files/v20n3img/f01150306.jpg" alt="f01150306" width="506" height="336" longdesc="v20n3 img/f01150306.jpg"><a name="figura1"></a></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">En relaci&oacute;n al uso de la FE y reducci&oacute;n del peron&eacute; se  deben seguir una serie de pasos, para logra la reducci&oacute;n correcta de la  fractura: <sup>42-44</sup>    <br>   Reducci&oacute;n y fijaci&oacute;n del peron&eacute;.    <br>   Restaurar la superficie articular en caso  de da&ntilde;o.    <br>   Mantener horizontal la l&iacute;nea articular.    <br>   Reducir y mantener alineados los  fragmentos.    <br>   Relleno del defecto &oacute;seo.    <br>   Control de la infecci&oacute;n si fractura  abierta.</font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Existen algunos autores que ponen en duda  la fijaci&oacute;n simult&aacute;nea del peron&eacute; cuando se emplea la FE, sin embargo la fijaci&oacute;n del  peron&eacute; tiene diversas ventajas como: <sup>45-47</sup>    <br>   Restaurar la longitud, alienaci&oacute;n y  rotaci&oacute;n, de forma indirecta reduce deformidad tibial debido a las uniones de  partes blandas entre los dos huesos (peron&eacute; y tibia).</font></p>     ]]></body>
<body><![CDATA[<p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Despu&eacute;s de la reducci&oacute;n y fijaci&oacute;n del  peron&eacute;, el desplazamiento residual de la tibia puede ser tratado mediante el  uso de fijaci&oacute;n externa, ya que el peron&eacute; sirve de fulcro.    <br>   La reducci&oacute;n del peron&eacute; neutraliza la  tendencia al valgo y traslaci&oacute;n lateral del astr&aacute;galo.    <br>   La reducci&oacute;n anat&oacute;mica del peron&eacute; permite  de forma indirecta la reducci&oacute;n de otras zonas fracturadas del pil&oacute;n tibial  como la anterolateral (Chaput) y la posterior (Volkmann).</font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">El tiempo de duraci&oacute;n de la FE es por lo general de ocho a  12 semanas, previo control radiogr&aacute;fico seriado, para detectar desviaciones  angulares las que pueden ser corregidas mediante el uso de distractores. <sup>48, 49 </sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Las complicaciones de la FE pueden ser clasificadas en  aquellas relacionadas propiamente con la   FE y las propias de la fractura. En el primer grupo se  incluyen la infecci&oacute;n en el trayecto de los pines, aflojamiento, ruptura y  p&eacute;rdida de la fijaci&oacute;n con la resultante desviaci&oacute;n de la fractura, ya en el segundo  grupo se pudo encontrar el retardo de consolidaci&oacute;n, seudoartrosis, artritis  postraum&aacute;tica y limitaci&oacute;n del rango de movimiento de la articulaci&oacute;n. <sup>50, 51 </sup></font></p>     <p align="justify">&nbsp;</p>     <p align="justify"><font size="3" face="Verdana, Arial, Helvetica, sans-serif"><b>CONCLUSIONES</b></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">El empleo de la FE asociada a la osteos&iacute;ntesis  del peron&eacute; constituye un m&eacute;todo aplicable y reproducible, que ofrece resultados  satisfactorios a pacientes que sufren de FPT.
