Objetivo: profundizar en la compresión subacromial y la importancia de la descompresión de este compartimiento mediante la vía artroscópica. Métodos: se realizó una revisión bibliográfica acerca del tema de un total de 570 artículos publicados en las bases de datos Pubmed, Hinari, SciELO y Medline mediante el gestor de búsqueda y administrador de referencias EndNote, de ellos se utilizaron 56 citas seleccionadas para realizar la revisión, 53 de ellas de los últimos cinco años donde se incluyeron tres libros. La búsqueda de la información se realizó en un período de dos meses del 1ro de febrero al 31 de marzo de 2016. Se emplearon las siguientes palabras: subacromial compression y arthroscopic subacromial decompression. Desarrollo: se abordan los factores participantes en la compresión subacromial divididos en intrínsecos, extrínsecos y secundarios. Se hace referencia al cuadro clínico de estos enfermos con especial énfasis en las maniobras más empleadas. En relación al tratamiento quirúrgico se describen las dos variedades de tratamiento por la vía artroscópica, así como las complicaciones más frecuentes. Conclusiones: la descompresión subacromial por la vía artroscópica es efectiva en pacientes con compresión subacromial, es un método factible con un mínimo de complicaciones.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[Background: subacromial compression is the most common cause of shoulder pain. Its treatment varies from conservative to surgical. However, surgical treatment by arthroscopy is better in regards to functional and cosmetic points of view. Objective: to deepen into the importance of subacromial decompression by arthroscopy in patients with subacromial compression. Methods: a search in the databases Pubmed, Hinari, SciELO and Medline was carried out through the management information software EndNote by using the words subacromial compression and arthroscopic subacromial decompression, resulting in a total of 570 articles, in which 56 of them were selected for review, 53 of them in the last five years, including three books. Development: important aspects related to subacromial compression causes were described and divided in intrinsic, extrinsic, and secondary. Specific maneuvers and clinical presentation were pointed. In regards to surgical treatment, a couple of arthroscopic surgical modalities were described as well as complications. Conclusions: arthroscopic subacromial decompression is an effective surgical modality in patients suffering from subacromial compression; it’s a safe procedure with few complications.]]></p></abstract>

<body><![CDATA[<p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">La compresi&oacute;n  subacromial (CS) es la causa m&aacute;s importante de dolor en el hombro con un  porcentaje que var&iacute;a del 44 % al 65 % de todas las afecciones del hombro. En un  estudio realizado en diferentes pa&iacute;ses como Estados Unidos de Norteam&eacute;rica,  Reino Unido, pa&iacute;ses escandinavos, Cuba, Sud&aacute;frica, Espa&ntilde;a y Nigeria la  incidencia de esta enfermedad es del 4,7 % al 46,7 %. <sup>4</sup> Adem&aacute;s de  dolor se presenta limitaci&oacute;n del movimiento articular acompa&ntilde;ado de atrofia  muscular en diferentes estadios. Existen maniobras que de ser positivas, son  sugestivas de la enfermedad, de all&iacute; que el m&eacute;todo cl&iacute;nico es el m&aacute;s  importante.</font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Los medios  complementarios en especial la imaginolog&iacute;a confirma el diagn&oacute;stico, basados en  el ultrasonido y la imagen de resonancia magn&eacute;tica IRM. <sup>5-7</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Existen un gran  n&uacute;mero de enfermedades relacionadas con la CS divididas en intr&iacute;nsecas, extr&iacute;nsecas y  secundarias, su conocimiento es indispensable, para enfocar de forma adecuada  el tratamiento, tanto conservador como quir&uacute;rgico. <sup>8-10</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">La modalidad de  tratamiento quir&uacute;rgica est&aacute; justificada en casos de fallo del tratamiento  conservador por m&aacute;s de ocho semanas y presencia de anormalidades del acromion.  El uso de la v&iacute;a artrosc&oacute;pica ofrece ventajas sobre la abierta, de all&iacute; que es  la m&aacute;s empleada en la actualidad, sus resultados son favorables con un m&iacute;nimo  de complicaciones. <sup>11-13</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">El objetivo del trabajo es profundizar en la CS y la importancia de la  descompresi&oacute;n de este compartimiento mediante la v&iacute;a artrosc&oacute;pica.</font></p>     <p align="justify">&nbsp;</p>     <p align="justify"><font size="3" face="Verdana, Arial, Helvetica, sans-serif"><b>M&Eacute;TODOS</b></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Se realiz&oacute; una revisi&oacute;n bibliogr&aacute;fica acerca  del tema de un total de 570 art&iacute;culos publicados en las bases de datos Pubmed,  Hinari, SciELO y Medline mediante el gestor de b&uacute;squeda y administrador de  referencias EndNote, de ellos se utilizaron 56 citas seleccionadas para  realizar la revisi&oacute;n, 53 de ellas de los &uacute;ltimos cinco a&ntilde;os donde se incluyeron  tres libros. Se excluyeron las investigaciones de pacientes que abordaban la CS asociadas a inestabilidad  del hombro.</font></p>     <p align="justify">&nbsp;</p>     <p align="justify"><font size="3" face="Verdana, Arial, Helvetica, sans-serif"><b>DESARROLLO</b></font></p>     ]]></body>
<body><![CDATA[<p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Las causas de CS  son clasificadas seg&uacute;n Ellman H y Gartsman GM <sup>14</sup> en factores  intr&iacute;nsecos, extr&iacute;nsecos y secundarios.    <br>   Los factores  intr&iacute;nsecos est&aacute;n en dependencia de la morfolog&iacute;a y estado del tend&oacute;n, los  cuales incrementan el grosor y al mismo tiempo causan la p&eacute;rdida del balance  funcional entre el deltoides y el supra-espinoso. Estos factores atribuibles  son: el proceso degenerativo de los tendones del manguito rotador por la edad,  pobre vascularidad y disminuci&oacute;n de la actividad bil&oacute;gica y propiedades  mec&aacute;nicas que afectan las cargas t&eacute;nsiles y de cizallamiento del tend&oacute;n. <sup>15-17</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">En relaci&oacute;n a  los factores extr&iacute;nsecos, que provocan la disminuci&oacute;n del espacio subacromial se  encontraron algunos elementos como: <sup>18-20</sup>    <br>   Morfolog&iacute;a  acromial: est&aacute; en estrecha relaci&oacute;n con los tipos de acromion seg&uacute;n Bigliani  LU, <sup>21</sup> tipo I- plano, tipo II- curvo y tipo III en forma de gancho  (<a href="#figura1">figura 1</a>).</font></p>     <p align="justify">&nbsp;</p>     <p align="center"><img src="/img/revistas/amc/v20n5/f01150516.jpg" alt="figura 1" width="609" height="403" longdesc="img no.5/f01150516.jpg"><a name="figura1"></a></p>     
<p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">&Aacute;ngulo o  pendiente acromial: una pendiente  acromial plana o muy horizontal del acromion se asocia a CS.     <br>   Inclinaci&oacute;n  acromial: la disminuci&oacute;n de la inclinaci&oacute;n del acromion en relaci&oacute;n a la  esc&aacute;pula se asocia a CS.    <br>   Osteofitosis: en  la superficie inferior de la articulaci&oacute;n acromioclavicular y borde  anteroinferior del acromion.    <br>   Cambios  postraum&aacute;ticos: provocados por fracturas del troquiter, coracoides y acromion. <sup>22-24</sup>    ]]></body>
<body><![CDATA[<br>   Los factores  secundarios est&aacute;n en relaci&oacute;n con la biomec&aacute;nica de la articulaci&oacute;n como:  alteraciones en la cinem&aacute;tica escapulo-humeral, d&eacute;ficit muscular del manguito  rotador y musculatura periarticular, alteraciones de la postura; y la  disminuci&oacute;n de la extensibilidad de la c&aacute;psula posterior y pectoral menor. <sup>25-27</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">La enfermedad  transita por diferentes estadios de forma unidireccional en dependencia de su  causa, la primera descripci&oacute;n de la enfermedad en relaciona este aspecto fue  realiza por Neer CS, <sup>28</sup> y consiste en tres etapas o estadios:    <br>   Estadio I:  bursitis con edema y hemorragia subacromial.    <br>   Estadio II:  comienzo de la tendinitis y presencia de desgarro parcial del manguito rotador.    <br>   Estadio III:  progresi&oacute;n de la ruptura parcial a completa.</font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">El s&iacute;ntoma m&aacute;s  importante en pacientes con CS es el dolor el cual varia en intensidad, es en  forma de cuchillada y profundo, localizado en la zona anterolateral del hombro.  Se incrementa con los movimientos de la articulaci&oacute;n en flexi&oacute;n y abducci&oacute;n por  encima de 90 grados. El dolor en ocasiones levanta el paciente en horas de la  noche, lo que provoca trastornos del sue&ntilde;o. Por lo general, se asocia a  limitaci&oacute;n del movimiento articular y debilidad muscular. <sup>29-31</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">La exploraci&oacute;n  f&iacute;sica del paciente constituye la piedra angular para el diagn&oacute;stico, comienza  con la exploraci&oacute;n del cuello y el hombro. La disminuci&oacute;n en los movimientos  del cuello y las maniobras provocativas de dolor de esta zona son sugestivas de  lesi&oacute;n a este nivel y no deben confundirse con la CS. Se debe observar el  contorno del hombro y compararlo con el otro lado, para detectar atrofia  muscular. <sup>32-34</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Seg&uacute;n Plancher  KD, et al, <sup>35</sup> la presencia de positividad de la maniobra de Hawkins,  el signo del arco doloroso y las pruebas para el infraespinoso se relaciona con  el diagn&oacute;stico positivo de CS en m&aacute;s del 95 %.</font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Por otra parte,  el diagn&oacute;stico tambi&eacute;n puede ser corroborado mediante la administraci&oacute;n de 10  ml de lidoca&iacute;na en el espacio subacromial y el dolor disminuye su intensidad  aun con las maniobras, que resultaron dolorosas con anterioridad. <sup>36-38</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Para confirmar  el diagn&oacute;stico se realizan vistas radiogr&aacute;ficas en proyecciones espec&iacute;ficas. El  ultrasonido y la IRM  son de gran utilidad para definir los factores que participan en esta enfermedad.  <sup>39-41</sup></font></p>     ]]></body>
<body><![CDATA[<p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">El tratamiento  de esta afecci&oacute;n puede ser conservador o quir&uacute;rgico, este &uacute;ltimo puede ser de  manera abierta o mediante el uso de la v&iacute;a artrosc&oacute;pica. Las ventajas de la v&iacute;a  artrosc&oacute;pica son varias: el acceso m&iacute;nimo de la articulaci&oacute;n, evita la  desinserci&oacute;n de fibras del deltoides, se observa toda articulaci&oacute;n en busca de  otras enfermedades intrarticulares, permite una r&aacute;pida incorporaci&oacute;n a la  rehabilitaci&oacute;n, menor dolor posoperatorio, no afecta la est&eacute;tica y permite que el  enfermo retorne a sus actividades deportivas con mayor rapidez. <sup>42-44</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Las t&eacute;cnicas  artrosc&oacute;picas para la descompresi&oacute;n subacromial (DS) en la regi&oacute;n  anteroinferior son de dos tipos: la lateral descrita por Ellman H, <sup>14</sup>  y la del corte en bloque propuesta por Sampson TG ambos citados por Plancher KD,  et al. <sup>35</sup> No existen diferencias en los resultados de las dos  t&eacute;cnicas, pero la m&aacute;s empleada es la lateral.</font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Para realizar  las t&eacute;cnicas artrosc&oacute;picas se emplea la anestesia general endotraqueal  combinada con bloqueo inter-escal&eacute;nico, este m&eacute;todo anest&eacute;sico permite la  aplicaci&oacute;n de la hipotensi&oacute;n controlada, la que debe estar en una presi&oacute;n  sist&oacute;lica que var&iacute;a de 95 mm  de Hg a 105 mm  de Hg. La presi&oacute;n sist&oacute;lica por debajo de 100 permite controlar el sangramiento  articular y disminuye la necesidad de utilizar bombas de agua y de esta manera  se evita la extravasaci&oacute;n de l&iacute;quido. <sup>45-47</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Una vez  anestesiado el paciente se coloca en posici&oacute;n de silla de playa o en dec&uacute;bito  lateral, la m&aacute;s empleada es la primera. Ya al estar el paciente en la posici&oacute;n  quir&uacute;rgica descrita con anterioridad se dibujan con un marcador las eminencias  &oacute;seas del hombro, para determinar los portales, ya que una vez que se comienza  la administraci&oacute;n de l&iacute;quidos pueden distorsionarse los puntos anat&oacute;micos. <sup>48-50</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">El procedimiento  quir&uacute;rgico comienza con la exploraci&oacute;n de la articulaci&oacute;n gleno-humeral  mediante el portal posterior est&aacute;ndar, en caso de detectarse alguna afecci&oacute;n se  proceder&aacute; a realizar tantos portales como sean necesarios. Para la DS se emplea un portal situado cerca  de un cent&iacute;metro superior y lateral del portal posterior est&aacute;ndar. Los portales  laterales se realizan a dos cent&iacute;metros del borde lateral del acromion. Una vez  dentro de espacio subacromial se procede al desbridamiento con el uso de  rasuradores y electrocoaguladores. El tejido bursal debe ser extra&iacute;do hasta  observar tejido sangrante y el techo del manguito rotador. <sup>51-53</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">La secci&oacute;n del  ligamento corarco-acromial hipertr&oacute;fico debe ser llevada a cabo en todos los  casos, excepto en presencia de ruptura masiva e irreparable del manguito  rotador. El uso de una aguja espinal para marcar el borde anterolateral del  acromion ayuda a identificar este ligamento. Durante este procedimiento se puede  producir sangramiento por lesi&oacute;n de la arteria acromial que es una rama proveniente  de la arteria coraco-acromial, pero puede ser controlado por el uso de electrocoagulador  intrarticular. <sup>49, 50 </sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Terminado el  procedimiento artrosc&oacute;pico, se dan puntos de piel y se coloca la extremidad en  un Velpeau de tela. <sup>5, 49 </sup>    <br>   Las  complicaciones m&aacute;s reportadas por la   DS son: desinserci&oacute;n de fibras del deltoides debido a resecci&oacute;n  lateral excesiva, infecci&oacute;n posquir&uacute;rgica, rigidez posoperatoria y fallo para  identificar hueso acromial sintom&aacute;tico. <sup>45-56</sup></font></p>     <p align="justify">&nbsp;</p>     <p align="justify"><font size="3" face="Verdana, Arial, Helvetica, sans-serif"><b>CONCLUSIONES</b></font></p>     ]]></body>
