Objetivo: profundizar en aspectos como indicaciones quirúrgicas y las técnicas más empleadas por la vía artroscópica. Métodos: la búsqueda de la información se realizó desde el primero de mayo de 2016 hasta el 30 de junio de 2016. Se realizó una revisión bibliográfica de un total de 539 artículos publicados en las bases de datos PubMed, Hinari, SciELO y Medline, mediante el gestor de búsqueda y administrador de referencias EndNote, de ellos se utilizaron 54 citas seleccionadas para realizar la revisión, todas de los últimos cinco años donde se incluyeron cuatro libros. Desarrollo: son descritos los estabilizadores del hombro divididos en estáticos y dinámicos. Se plantean las indicaciones quirúrgicas generales y las ventajas del tratamiento artroscópico, así como los métodos anestésicos más empleados y posiciones quirúrgicas. Se describen las técnicas de Bankart, Lartajet, reconstrucción de ligamentos glenohumerales y de remplissage. Conclusiones: la vía artroscópica ofrece múltiples ventajas en relación a la vía abierta. Las técnicas más empleadas son las de Bankart, plicatura de la cápsula, Latarjet y la de remplissage.]]></p></abstract>

A bibliographical review of 539 published articles in Pubmed, Hinari, SciELO and Medline databases, through the information locator EndNote was conducted. Among all, 54 quotes were selected to conduct the revision; all of them were from five years including four books. Development: stabilizers of the shoulder divided into static and dynamic are described. General surgical indications and advantages of arthroscopic treatment, as well as the most used anesthetic methods are stated. The techniques of Bankart, Lartajet, repair of glenohumeral ligaments, and remplissage were described. Conclusions: arthroscopic treatment has several advantages over open ones. <body><![CDATA[<p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">El diagn&oacute;stico positivo de esta  afecci&oacute;n es realizado por los antecedentes de trauma, despu&eacute;s del cual se han  repetido varios episodios de luxaci&oacute;n de la articulaci&oacute;n, el paciente por lo  general refiere dolor y aprehensi&oacute;n a los movimientos de abducci&oacute;n y rotaci&oacute;n  externa del hombro. Las maniobras m&aacute;s empleadas son las de caj&oacute;n anterior,  posterior, inferior y el signo del surco. <sup>5-7</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Aunque el diagn&oacute;stico es cl&iacute;nico, los  ex&aacute;menes imaginol&oacute;gicos como la radiograf&iacute;a simple, ultrasonograf&iacute;a y la imagen  de resonancia magn&eacute;tica nuclear, confirman el diagn&oacute;stico y son decisivas para  la detecci&oacute;n de otras lesiones asociadas. <sup>8-10</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">El tratamiento de la ICH es quir&uacute;rgico, ya sea por  v&iacute;a abierta o artrosc&oacute;pica, pero con la introducci&oacute;n de la artroscopia, esta  modalidad se ha convertido en el est&aacute;ndar de oro para su tratamiento a trav&eacute;s  de la cual se pueden realizar numerosas t&eacute;cnicas quir&uacute;rgicas con diferentes  grados de complejidad. <sup>11-13</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">El tratamiento conservador en  pacientes menores de 30 a&ntilde;os se acompa&ntilde;a de un 17 % al 96 % de recurrencia de  la luxaci&oacute;n, mientras que con el tratamiento artrosc&oacute;pico es del 4 % al 22 %. <sup>14</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Debido a la importancia del tema, la  gran cantidad de pacientes que padecen de ICH y las ventajas de la v&iacute;a artrosc&oacute;pica,  se propuso profundizar en aspectos como indicaciones quir&uacute;rgicas y las t&eacute;cnicas  m&aacute;s empleadas por la v&iacute;a artrosc&oacute;pica.</font></p>     <p align="justify">&nbsp;</p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif"><b>M&Eacute;TODOS</b></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">La b&uacute;squeda de la  informaci&oacute;n se realiz&oacute; desde el primero de mayo de 2016 hasta el 30 de junio de  2016, a partir de la informaci&oacute;n obtenida se  realiz&oacute; una revisi&oacute;n bibliogr&aacute;fica de un total de 539 art&iacute;culos publicados en  las bases de datos PubMed, Hinari, SciELO y Medline, mediante el gestor de  b&uacute;squeda y administrador de referencias EndNote, de ellos se utilizaron 54 citas  seleccionadas para realizar la revisi&oacute;n, todas de los &uacute;ltimos cinco a&ntilde;os donde  se incluyeron cuatro libros.</font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Se  consideraron estudios de pacientes con inestabilidad cr&oacute;nica del hombro en  cuanto a diagn&oacute;stico y diferentes modalidades de tratamiento quir&uacute;rgico por la  v&iacute;a artrosc&oacute;pica. Se  excluyeron las investigaciones de pacientes que abordaban la inestabilidad del  hombro asociadas a ruptura del manguito rotador y por la v&iacute;a abierta.</font></p>     <p align="justify">&nbsp;</p>     ]]></body>
<body><![CDATA[<p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif"><b>DESARROLLO</b></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Los estabilizadores del hombro pueden  ser pasivos o est&aacute;ticos y activos o din&aacute;micos, en los primeros se encontr&oacute; la  forma de la articulaci&oacute;n glenohumeral, del labrum, el efecto de vac&iacute;o y  el complejo capsuloligamentoso en especial el ligamento glenohumeral inferior.  Los estabilizadores din&aacute;micos son: el m&uacute;sculo subescapular, deltoides y los que  conforman el manguito de los rotadores. 15-16 Es en estas  estructuras donde se presentan las lesiones m&aacute;s importantes que producen la ICH (<a href="#tabla1">tabla 1</a>).</font></p>     <p align="justify">&nbsp;</p>     <p align="center"><img src="/img/revistas/amc/v20n6/t01140616.jpg" alt="tabla 1" width="587" height="259" longdesc="img n6/t01140616.jpg"><a name="tabla1"></a></p>     
<p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">La interacci&oacute;n entre estabilizadores  est&aacute;ticos y din&aacute;micos ayuda a mantener la estabilidad de la articulaci&oacute;n glenohumeral  y su rango de movimiento. <sup>17-19</sup>    <br>   Para determinar la indicaci&oacute;n  quir&uacute;rgica de pacientes con ICH, se deben tener en cuenta varios factores como:  edad, frecuencia y cantidad de los episodios de inestabilidad, nivel de  actividad f&iacute;sica del enfermo y presencia de lesiones asociadas entre las que se  encuentran p&eacute;rdida &oacute;sea de la cabeza humeral o glenoides, lesiones del manguito  rotador y lesiones del borde superior del labrum glenoideo de anterior a posterior. <sup>20-22</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">En relaci&oacute;n a las ventajas del  tratamiento artrosc&oacute;pico sobre el abierto se sabe que existe disminuci&oacute;n de la morbilidad posoperatoria y el  dolor, per&iacute;odo de hospitalizaci&oacute;n corto, mejor funci&oacute;n articular, permite una  mejor exploraci&oacute;n de la articulaci&oacute;n glenohumeral y menor da&ntilde;o a los tejidos  blandos circundantes en especial el tend&oacute;n del subescapular. <sup>23-25</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">La  anestesia empleada para las t&eacute;cnicas por la v&iacute;a artrosc&oacute;pica es la general  endotraqueal combinada con bloqueo interescal&eacute;nico y se aplica hipotensi&oacute;n  controlada para mantener la visualizaci&oacute;n del campo quir&uacute;rgico. Una vez  anestesiado el enfermo se procede a colocarlo en posici&oacute;n quir&uacute;rgica, que puede  ser en dec&uacute;bito lateral o en silla de playa, esta &uacute;ltima la empleada por los  autores del trabajo. <sup>26-28</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">La lesi&oacute;n de Bankart es tratada  mediante la v&iacute;a artrosc&oacute;pica con el empleo de anclas, para lo cual debe existir  buena calidad del tejido del labrum, y la ausencia de lesi&oacute;n &oacute;sea. Las  contraindicaciones para la t&eacute;cnica de Bankart incluyen: p&eacute;rdida &oacute;sea  significativa de la glenoides mayor al 25 %, mala calidad del tejido del labrum,  as&iacute; como las lesiones de Hill Sachs mayores a un 35 % de la superficie  articular. Las anclas son colocadas de inferior a superior en el hombro derecho  a las horas 5:30, cuatro y tres; y en el izquierdo a las 6:30, ocho o nueve.  Aunque la posici&oacute;n ideal son las seis, esta resulta muy dif&iacute;cil debido al  &aacute;ngulo requerido para colocar el ancla. <sup>29-31</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Aunque la mayor&iacute;a de las t&eacute;cnicas en  la inestabilidad anterior con lesi&oacute;n de Bankart emplean anclas y plicatura de  la c&aacute;psula, en ocasiones cuando existe buena calidad de los tejidos de la  disrupci&oacute;n entre el labrum y la c&aacute;psula se puede emplear la plicatura  como &uacute;nica forma de tratamiento. La plicatura puede ser realizada en los cuadrantes  anteroinferior y posteroinferior. La cantidad de tejido por la plicatura es del  16,2 % cuando se plican cinco mil&iacute;metros de la c&aacute;psula, y del 33,7 % cuando se  plican 10 mil&iacute;metros. Por lo que, esta t&eacute;cnica se puede acompa&ntilde;ar de limitaci&oacute;n  de la rotaci&oacute;n externa. En caso de estar asociada laxitud de la c&aacute;psula, se  puede realizar el cierre de los espacios entre los ligamentos glenohumeral  superior y medio. <sup>32-34</sup></font></p>     ]]></body>
<body><![CDATA[<p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">La t&eacute;cnica de Latarjet est&aacute;  justificada en pacientes que presentan: p&eacute;rdida &oacute;sea de la porci&oacute;n anterior de  la glenoides (deficiencia de un 25 % o m&aacute;s del di&aacute;metro anteroposterior), mala  calidad de los ligamentos, en la cirug&iacute;a de revisi&oacute;n y en pacientes que  participan en deportes extremos. <sup>35-37</sup> Esta modalidad artrosc&oacute;pica  se puede llevar a cabo mediante la transferencia de la punta de la coracoides,  al borde anterior de la glenoides o con otra variante que consiste en peque&ntilde;o  injerto &oacute;seo proveniente de la creta iliaca. <sup>38-40</sup> Ambas variantes  de la t&eacute;cnica tienen como objetivo aumentar el stock &oacute;seo del borde anterior de  la glenoides y de esta manera evitar los continuados episodios de luxaci&oacute;n. <sup>41, 42 </sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">En relaci&oacute;n a los ligamentos  glenohumeral, el inferior es el m&aacute;s importante en cuanto a su funci&oacute;n  estabilizadora en las direcciones anterior, inferior y posterior, ya que es el  principal restrictor primario a los 45 grados y 90 grados de abducci&oacute;n del  hombro. <sup>43-45</sup> En el hombro derecho su banda anterior se origina desde  la hora dos a la cuatro y la banda posterior de la siete a la nueve, en la cabeza  humeral se inserta cerca de la superficie articular, esta inserci&oacute;n tiene forma  de collar o en V. La disrupci&oacute;n del ligamento glenohumeral inferior ocurre en  un 40 % en su inserci&oacute;n en la glenoides, 35 % en la sustancia media y un 25 %  en su inserci&oacute;n en la cabeza humeral. Antes de realizar la reparaci&oacute;n se debe  proceder al desbridamiento de la zona &oacute;sea donde se reinsertar&aacute; el ligamento,  como por ejemplo en el h&uacute;mero, la reparaci&oacute;n se comienza de inferior a  superior. <sup>46-48</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">La lesi&oacute;n de Hill Sachs puede ser  detectada del 94 % al 100 % de los pacientes con ICH, de no ser tratada de  forma correcta se compromete el &eacute;xito de otros procedimientos quir&uacute;rgicos como  por ejemplo la operaci&oacute;n de Bankart. <sup>49-51</sup> Cuando la lesi&oacute;n ocupa  menos de un 20 % de la superficie articular es tratada de forma conservadora, y  si es mayor al 40 % es quir&uacute;rgica. Sin embargo, cuando la magnitud tiene un  rango del 20 % al 40 % existen contradicciones en su conducta. La t&eacute;cnica de remplissage consiste en fijar con anclas el tend&oacute;n del infraespinoso en el defecto de la  cabeza humeral. Las indicaciones de esta modalidad de tratamiento son lesi&oacute;n de  Hill Sach mayor de un 25 % y p&eacute;rdida &oacute;sea de la glenoides menor al 20 %. Aunque  existen otras t&eacute;cnicas para el tratamiento de la lesi&oacute;n de Hill Sachs, la de remplissage es la &uacute;nica realizada por la v&iacute;a artrosc&oacute;pica y est&aacute; encaminada a disminuir la  incidencia de fallos de otras t&eacute;cnicas. <sup>52-54</sup></font></p>     <p align="justify">&nbsp;</p>     <p align="justify"><font size="3" face="Verdana, Arial, Helvetica, sans-serif"><b>CONCLUSIONES</b></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">La   ICH es una enfermedad frecuente en  especial en pacientes j&oacute;venes, existen varias t&eacute;cnicas quir&uacute;rgicas para esta enfermedad  y dependen del tipo de lesi&oacute;n que presente el enfermo. La v&iacute;a artrosc&oacute;pica  ofrece m&uacute;ltiples ventajas en relaci&oacute;n a la v&iacute;a abierta.
