Objetivo: profundizar y actualizar los conocimientos sobre pacientes que sufren de fractura del extremo distal del radio, debido a su alta incidencia en los servicios de urgencia. Métodos: la búsqueda de la información se realizó en el periodo de un mes (octubre) y se emplearon las siguientes palabras: fractures of the distal radius y wrist fracture, a partir de la información obtenida se realizó una revisión bibliográfica de un total de 1507 artículos publicados en las bases de datos PubMed, Hinari, SciELO y Medline mediante el gestor de búsqueda y administrador de referencias EndNote, de ellos se utilizaron 55 citas seleccionadas para realizar la revisión, 53 de ellas de los últimos cinco años donde se incluyeron siete libros. Desarrollo: se describen los criterios radiográficos de reducción aceptable, así como las indicaciones quirúrgicas generales y en específico de la fijación externa. Se hace referencia a los factores que afectan el tratamiento. Se describen los tipos de fijación externa e indicaciones específicas de cada una. Se plasman los factores predictivos de inestabilidad, las complicaciones y las principales ventajas y desventajas de la fijación externa al compararla con la fijación interna mediante láminas volares. Conclusiones: la fijación externa es un método efectivo para el tratamiento de pacientes con fractura del extremo distal del radio, ofrece ventajas como su fácil aplicación, logra una buena reducción y estabilidad de manera independiente de los dos tipos de fijación, que tienen indicaciones muy específicas, su uso implica un bajo índice de complicaciones entre las que se destaca la infección en el trayecto de los pines alambres. Por otra parte, los resultados funcionales a largo plazo son muy similares a los logrados por la fijación interna con placas volares.]]></p></abstract>

Treatment may be conservative or surgical, and one of the surgical modalities is external fixation based on its advantages. Objective: to deep and update about the knowledge of patients with fractures of the distal radius. Methods: a search in the databases PubMed, Hinari, SciELO and Medline was done through the information locator EndNote by using the words fractures of the distal radius and wrist fracture resulting in a total of 1507 articles which 55 of them selected for review, 53 of them in the last five years, including seven books. Development: radiographic criteria in regards to acceptable reduction were described, also as general surgical indications and mainly those related to external fixation. The two types of external fixation were pointed out, as well as their mains indications. Factor related to further instability were described, also as complications, advantages and disadvantages of external fixation in relation to internal fixation by volar plates. Conclusions: external fixation is an effective method for patients with fractures of the distal radius, because of its easy application; good reduction and alignment are obtained despite either of the two types of external fixators. Complications are few, among them pin tract infections are the most common. On the other hand, functional results are similar between external fixation and internal fixation by volar plates.]]></p></abstract>

<body><![CDATA[<p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">El  sexo femenino es el m&aacute;s afectado debido a la presencia de mayor osteoporosis de  la zona fracturada, por debajo de los 40 a&ntilde;os tiene una incidencia de 368 por  100 000 y por encima de 40 a&ntilde;os la incidencia es de 1 150 por 100 000  habitantes. <sup>2-4</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Las  FEDR son causadas por traumas de alta energ&iacute;a en el paciente joven y de baja  energ&iacute;a en el anciano provocado por compresi&oacute;n axial de la extremidad con la  mu&ntilde;eca en extensi&oacute;n de 40 a 90 grados, ya que en este &uacute;ltimo caso son consideradas  fracturas por fragilidad &oacute;sea. <sup>5-7</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">El  cuadro cl&iacute;nico es caracterizado por dolor, p&eacute;rdida de la movilidad articular,  edema y deformidad en dorso de tenedor y bayoneta. El paciente sostiene la  extremidad afectada por la sana y se dirige de forma inmediata a los servicios  de urgencias. <sup>8-10</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Aunque  el tratamiento de esta afecci&oacute;n traum&aacute;tica es por lo general conservador,  existen condiciones en que este no puede ser aplicado debido al desplazamiento,  lesiones asociadas, la edad del enfermo, su nivel de actividad f&iacute;sica entre  otros. <sup>11-13</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">El  tratamiento quir&uacute;rgico tiene varias modalidades como el uso de agujas de  Kirschner, l&aacute;minas volares entre las que se destacan las autobloquedas. <sup>14-16</sup>  Por otra parte, la fijaci&oacute;n externa (FE) que puede ser empleada basada en el  principio de la ligamentotaxia y tiene m&uacute;ltiples ventajas entre las que se  destacan su f&aacute;cil aplicaci&oacute;n, permite restaurar la longitud del radio, facilita  la cura de las partes blandas en caso de fracturas abiertas y tiene un bajo  &iacute;ndice de complicaciones. <sup>17-19</sup> Una de las primeras descripciones en  el uso de la FE fue realizada por Anderson R y O'Neil G en el a&ntilde;o 1944 citado  por Simic PM, et al, <sup>20</sup> donde emplearon la tracci&oacute;n esquel&eacute;tica en  una barra simple.</font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Debido  a la importancia de este tema, la gran cantidad de pacientes atendidos por esta  afecci&oacute;n traum&aacute;tica, se traz&oacute; como objetivo profundizar y actualizar sobre el  uso de la FE en pacientes con FEDR.</font></p>     <p align="justify">&nbsp;</p>     <p align="justify"><font size="3" face="Verdana, Arial, Helvetica, sans-serif"><b>M&Eacute;TODOS</b></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">La b&uacute;squeda de la informaci&oacute;n se realiz&oacute; en el periodo de un mes  (octubre del 2016) y se emplearon las siguientes palabras: <i>fractures of the distal radius y wrist fracture</i>, a partir de la informaci&oacute;n obtenida se realiz&oacute; una  revisi&oacute;n bibliogr&aacute;fica de un total de 1507 art&iacute;culos publicados en las bases de  datos PubMed, Hinari, SciELO y Medline mediante el gestor de b&uacute;squeda y  administrador de referencias EndNote, de ellos se utilizaron 55 citas  seleccionadas para realizar la revisi&oacute;n, 53 de ellas de los &uacute;ltimos cinco a&ntilde;os,  donde se incluyeron siete libros.</font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Se consideraron  estudios de pacientes con fractura del extremo distal del radio, diagn&oacute;stico  cl&iacute;nico e imaginol&oacute;gico y tratamiento quir&uacute;rgico en especial la fijaci&oacute;n  externa y la interna mediante placas volares. Se excluyeron las  investigaciones de pacientes que abordaban la fractura del extremo distal del  radio tratadas por modalidades conservadoras. Este estudio tiene un nivel de evidencia III  recomendaci&oacute;n B.</font></p>     ]]></body>
<body><![CDATA[<p align="justify">&nbsp;</p>     <p align="justify"><font size="3" face="Verdana, Arial, Helvetica, sans-serif"><b>DESARROLLO</b></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Para  lograr una reducci&oacute;n aceptable, se necesita considerar algunos par&aacute;metros  radiogr&aacute;ficos, aparte del m&eacute;todo empleado, tanto conservador como quir&uacute;rgico  (<a href="#tabla1">tabla 1</a>). <sup>21-23</sup></font></p>     <p align="center"><font size="2" face="Verdana, Arial, Helvetica, sans-serif"><img src="/img/revistas/amc/v21n4/t01140417.jpg" alt="tabla 1" width="565" height="187" longdesc="img/t01140417.jpg"></font><a name="tabla1"></a></p>     
<p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Las  indicaciones generales del tratamiento quir&uacute;rgico en pacientes con FEDR son: traumas de alta energ&iacute;a, p&eacute;rdida secundaria de la reducci&oacute;n, conminuci&oacute;n  articular con escal&oacute;n o defecto, conminuci&oacute;n metafisaria o p&eacute;rdida de hueso, incongruencia  de la articulaci&oacute;n radiocubital distal, fracturas abiertas, fracturas asociadas  del carpo, da&ntilde;o asociado de tendones y vasculo-nerviosos, fracturas bilaterales  y extremidad contralateral afectada. <sup>24-26</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Por otra parte, las indicaciones espec&iacute;ficas de la FE en la mu&ntilde;eca son:  fracturas inestables, pacientes con traumas agudos de la mu&ntilde;eca acompa&ntilde;ados de  luxaci&oacute;n de los huesos del carpo y de la articulaci&oacute;n radiocarpiana, para el  control de da&ntilde;os, en fracturas muy severas conminutas con p&eacute;rdida de sustancia  &oacute;sea en que se necesita mantener la longitud del radio, fracturas abiertas con  p&eacute;rdida de partes blandas y gran contaminaci&oacute;n, adem&aacute;s de la estabilizaci&oacute;n de  la mu&ntilde;eca por infecciones. <sup>27-29</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Los factores que afectan el tratamiento pueden  ser locales: calidad &oacute;sea,  da&ntilde;o de partes blandas, conminuci&oacute;n, desplazamiento inicial de la fractura e  intensidad del trauma. Del paciente: edad  biol&oacute;gica, estilo de vida, ocupaci&oacute;n, mano dominante, condiciones m&eacute;dicas y  da&ntilde;os asociados. As&iacute; como la presencia de par&aacute;metros radiogr&aacute;ficos inaceptables  y mala alineaci&oacute;n de los huesos del carpo con el radio distal. <sup>30, 31</sup>     <br>   Existen  dos tipos de fijaci&oacute;n externa las que utilizan la mu&ntilde;eca como puente y las  otras que no, en el primer caso est&aacute; basada en el efecto de ligamentotaxia  (tracci&oacute;n de los fragmentos &oacute;seos a trav&eacute;s de las inserciones de los  ligamentos). <sup>32-34</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">La  FE tipo puente est&aacute; indicada en pacientes con fractura inestable por  conminuci&oacute;n dorsal extrarticular o articular con desplazamiento m&iacute;nimo y cuando  el fragmento distal es tan peque&ntilde;o, que no permite la colocaci&oacute;n de pines alambres,  este tipo de FE puede ser est&aacute;tica o din&aacute;mica (<a href="#fighura1">figuras 1</a> y <a href="#figura2">figura 2</a>). <sup>35, 36</sup></font></p>     <p align="center"><font size="2" face="Verdana, Arial, Helvetica, sans-serif"><img src="/img/revistas/amc/v21n4/f01140417.jpg" alt="figura 1" width="385" height="342" longdesc="img/f01140417.jpg"></font><a name="fighura1"></a></p>

<p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Sin  embargo, las indicaciones de la FE tipo no puente son: fracturas  inestables extrarticulares o con extensi&oacute;n articular reducible por m&eacute;todos  cerrados y se requiere de al menos un cent&iacute;metro de cortical volar intacta,  para la colocaci&oacute;n de los pines, alambres este requisito es tambi&eacute;n  indispensable para el uso de placas volares y tornillos, adem&aacute;s del uso en las  osteotom&iacute;as correctoras por consolidaci&oacute;n viciosa. <sup>33, 36 </sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Para  aumentar la estabilidad al emplear la FE, se pueden utilizar algunos m&eacute;todos  alternativos como la colocaci&oacute;n de agujas de Kirchner Kirschners por las zonas  radial y cubital del extremo distal del radio, las que son cortadas y dobladas alrededor  de un cent&iacute;metro de la piel del paciente. En presencia de fracturas impactadas  en las que no se logra la reducci&oacute;n por la FE, est&aacute; justificada la reducci&oacute;n  abierta por m&iacute;nima incisi&oacute;n, reducci&oacute;n y combinaci&oacute;n de alg&uacute;n m&eacute;todo de  fijaci&oacute;n interna. Adem&aacute;s de la combinaci&oacute;n con la reducci&oacute;n abierta y  osteos&iacute;ntesis, tanto por la regi&oacute;n dorsal como volar seg&uacute;n sea necesaria, para  lograr la reducci&oacute;n anat&oacute;mica de los fragmentos. <sup>37, 38 </sup>    <br>   Los  factores predictivos de inestabilidad son: edad del enfermo (los pacientes por encima de 80 a&ntilde;os tienen tres  veces m&aacute;s posibilidades de redesplazamiento o inestabilidad), grado de desplazamiento inicial (en  especial del radio), extensi&oacute;n de la  conminuci&oacute;n metafisaria (mayor a un cent&iacute;metro) y desplazamiento durante el  tratamiento. <sup>39, 40</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Las  complicaciones de la FE en pacientes con FEDR pueden ser divididas en tres  grupos, el primero atribuible a la dificultad para lograr y mantener la  reducci&oacute;n de la fractura, el segundo responde a problemas con los pines  alambres y por &uacute;ltimo los problemas propios de la fractura aparte del tipo de  tratamiento como complicaciones nerviosas, distrofia simp&aacute;tico refleja y  rigidez de los dedos. <sup>41-43</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Los  problemas con los pines alambres son las complicaciones m&aacute;s frecuentes y entre  ellas la infecci&oacute;n en su trayecto, su incidencia puede ser de hasta un 39 % y  por lo general no interfiere en el resultado final del tratamiento. Otras  complicaciones de los alambres pines son la ruptura y la adherencia a la piel,  pero ocurren en menor cuant&iacute;a, solo del 0,5 % al 1 %. <sup>44, 45</sup></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">En  la actualidad existen pol&eacute;micas en relaci&oacute;n a la superioridad de la fijaci&oacute;n  interna con placas volares sobre la FE. <sup>46-48</sup> Sin embargo, al  analizar las conclusiones de un grupo de investigaciones, no se encontraron  criterios que afirmen este planteamiento de forma absoluta en todos los casos,  pero existe concordancia en que el uso de placas volares permiten una  rehabilitaci&oacute;n m&aacute;s precoz y tiene un mayor &iacute;ndice de re-intervenciones  quir&uacute;rgicas. <sup>49-52</sup> Los resultados funcionales a largo plazo entre la  FE y las placas volares son muy similares seg&uacute;n algunos autores consultados (tabla  2). <sup>53-55</sup></font></p>     <p align="justify">&nbsp;</p>     <p align="justify"><font size="3" face="Verdana, Arial, Helvetica, sans-serif"><b>CONCLUSIONES</b></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">La  FE es un m&eacute;todo efectivo para el tratamiento de pacientes con FEDR, ofrece  ventajas como: f&aacute;cil aplicaci&oacute;n, logra una buena reducci&oacute;n y estabilidad aparte  de los dos tipos de fijaci&oacute;n que tienen indicaciones muy espec&iacute;ficas, su uso  implica un bajo &iacute;ndice de complicaciones entre las que se destaca la infecci&oacute;n  en el trayecto de los pines alambres Por otra parte, los resultados funcionales  a largo plazo son muy similares a los logrados por la fijaci&oacute;n interna con  placas volares.</font></p>     ]]></body>
