Tras una revisión de la literatura previa publicada, se realizó la descripción de un caso clínico de mielolipoma adrenal bilateral, que coexiste con hipotiroidismo primario, relación no reportada hasta el presente. Dichas masas tumorales se diagnosticaron en una mujer de 66 años, aquejada de dolor abdominal recurrente, que además padecía hipotiroidismo e hipercolesterolemia, a la que se le practicó con éxito la exéresis de la lesión de mayor tamaño. Se propuso un tratamiento diagnóstico-terapéutico para este tipo de tumores.]]></p></abstract>

After a review of published previous literature it was possible to describe of a clinical case of bilateral adrenal myelolipoma coexisting with a primary hypothyroidism with a relation not reported until now. Such tumoral masses were diagnosed in a woman aged 66 with recurrent abdominal pain and hypothyroidism and hypercholesterolemia who undergoes successfully a exeresis of the bigger lesion. A diagnostic-therapeutic treatment was proposed for this type of tumors.]]></p></abstract>

<body><![CDATA[<P><font size="2" face="Verdana"><B>RESUMEN</B> </font>     <P><font size="2" face="Verdana">Los mielolipomas adrenales son tumores benignos,    no funcionantes e infrecuentes, generalmente de diagn&oacute;stico incidental,    que eventualmente provocan s&iacute;ntomas locales. Tras una revisi&oacute;n    de la literatura previa publicada, se realiz&oacute; la descripci&oacute;n de    un caso cl&iacute;nico de mielolipoma adrenal bilateral, que coexiste con hipotiroidismo    primario, relaci&oacute;n no reportada hasta el presente. Dichas masas tumorales    se diagnosticaron en una mujer de 66 a&ntilde;os, aquejada de dolor abdominal    recurrente, que adem&aacute;s padec&iacute;a hipotiroidismo e hipercolesterolemia,    a la que se le practic&oacute; con &eacute;xito la ex&eacute;resis de la lesi&oacute;n    de mayor tama&ntilde;o. <body><![CDATA[<P>     <P><font size="2" face="Verdana"><B><font size="3">INTRODUCCI&Oacute;N</font></B></font>     <P><font size="2" face="Verdana">Los mielolipomas adrenales son tumores benignos,    no funcionantes e infrecuentes, compuestos por tejido adiposo y elementos tisulares    mieloides. Fueron descritos por primera vez en 1905 por <I>Grieke,</I> y nombrados    mielolipomas, en 1929, por <I>Oberling</I>. D&eacute;cadas atr&aacute;s solo    eran encontrados casualmente durante necropsias. Hoy, tecnolog&iacute;as como    la ecograf&iacute;a, la tomograf&iacute;a axial computarizada (TAC) y la resonancia    magn&eacute;tica nuclear (RMN) revelan su presencia sin dificultad, gracias    a las caracter&iacute;sticas de imagen que los hacen distintivos. Su infrecuente    presentaci&oacute;n motiva reportes en la literatura m&eacute;dica internacional,<SUP>1-6</SUP>    y en nuestro pa&iacute;s no ha sido expuesto a&uacute;n un consenso sobre su    tratamiento terap&eacute;utico. </font>      <P><font size="2" face="Verdana">Se presentan t&iacute;picamente como masas adrenales    incidentales (11 %),<SUP>7-9</SUP> de forma unilateral, o m&aacute;s raramente    bilateral (25 %).<SUP>10-16</SUP> Pueden aparecer tambi&eacute;n a expensas    de tejido adrenal ect&oacute;pico (hilio renal y regi&oacute;n presacra),<SUP>17-19</SUP>    o en otras localizaciones independientes del tejido adrenal (mediastino, pulm&oacute;n    e h&iacute;gado).<SUP>20-23</SUP> </font>     <P><font size="2" face="Verdana">Son tumores cuya patog&eacute;nesis no est&aacute;    bien esclarecida. En el 10 % pueden acompa&ntilde;arse de alguna endocrinopat&iacute;a,    lo cual ha sido asociado a una hiperproducci&oacute;n de hormona corticotropina    (ACTH) por coexistir con entidades como la enfermedad de Cushing,<B><SUP> </SUP></B>la    enfermedad de Addison y la hiperplasia adrenal cong&eacute;nita (HAC) por d&eacute;ficit    de 21 hidroxilasa.<SUP>24<B>-</B>27</SUP> No obstante, otras evidencias sugieren    que el rol de esta hormona es limitado por la ausencia de sobreexpresi&oacute;n    de receptores para ACTH y andr&oacute;genos en el tejido tumoral.<SUP>26</SUP>    </font>     <P><font size="2" face="Verdana">Son m&aacute;s frecuentes en el sexo masculino    entre los 40 y 60 a&ntilde;os de edad. Suelen tener di&aacute;metros menores    de 4 cm, pero se citan algunos casos con tama&ntilde;os superiores, y se han    resecado algunos hasta de 6 kg de peso.<SUP>5</SUP> El crecimiento exagerado    puede provocar s&iacute;ntomas locales secundarios a la compresi&oacute;n mec&aacute;nica,    la necrosis o la hemorragia. Cl&iacute;nicamente pueden ser asintom&aacute;ticos    (70 %) o expresarse como una masa tumoral palpable en el hemiabdomen superior,    con dolor abdominal epig&aacute;strico, lumbar o de flanco, que puede irradiarse    ocasionalmente al t&oacute;rax. Tambi&eacute;n pueden acompa&ntilde;arse de    hipertensi&oacute;n, hematuria, dispepsia, n&aacute;useas, v&oacute;mitos y    aparecer s&iacute;ntomas y signos de enfermedades con las que se encuentran    patog&eacute;nicamente vinculados, tales como, virilizaci&oacute;n o estigmas    de hipo, o hipercortisolismo.<SUP>28-32</SUP> </font>     <P><font size="2" face="Verdana">Se han descrito otras enfermedades asociadas    a los mielolipomas. Aparecen referencias de su asociaci&oacute;n con la obesidad,    el hiperaldosteronismo primario, el hipertiroidismo, la hiperplasia de m&eacute;dula    adrenal (tumor mixto), el d&eacute;ficit de 3 <font face="Symbol">b</font> hidroxiesteroide    deshidrogenasa, el lipoma col&oacute;nico y algunos tumores malignos, como el    carcinoma de c&eacute;lulas renales y el carcinoma cervicouterino.<SUP>2,11,29,33-36</SUP>    </font>      <P><font size="2" face="Verdana">Esta presentaci&oacute;n se propone reportar    un caso con el diagn&oacute;stico de mielolipoma adrenal bilateral concomitando    con hipotiroidismo primario.</font>     <P>     <P><font size="2" face="Verdana"><B><font size="3">CASO CL&Iacute;NICO</font></B></font>     ]]></body>
<body><![CDATA[<P><font size="2" face="Verdana">Mujer blanca, de 66 a&ntilde;os de edad, procedente    de &aacute;rea rural, con antecedentes de estar histerectomizada desde los 40    a&ntilde;os de edad. Padece de hipertensi&oacute;n arterial esencial de 20 a&ntilde;os    de evoluci&oacute;n, y se ha mantenido controlada con captopril (25 mg/d&iacute;a),    atenolol 50 (mg/d&iacute;a) e hidroclorotiazida (25 mg/d&iacute;a). En el chequeo    m&eacute;dico renal se detect&oacute; un aumento de volumen suprarrenal bilateral    de aspecto benigno mediante una ecograf&iacute;a, y este resultado fue confirmado    con un estudio tomogr&aacute;fico abdominal contrastado. En ese momento se decidi&oacute;    el seguimiento evolutivo por el tama&ntilde;o tumoral y la ausencia de s&iacute;ntomas    compresivos. </font>      <P><font size="2" face="Verdana">La paciente fue evaluada cada 6 meses con ultrasonograf&iacute;a    y tomograf&iacute;a durante 18 meses. En el mes de mayo de 2009 refiri&oacute;    dolor de baja intensidad en flanco izquierdo, insidioso e irradiado al hipocondrio    correspondiente. Tomogr&aacute;ficamente, la suprarrenal derecha se mantuvo    de iguales dimensiones, lo que no sucedi&oacute; contralateralmente, donde se    inform&oacute; un crecimiento por encima de los 6 cm. Se le realiz&oacute; adrenalectom&iacute;a    izquierda el 25 de mayo de 2009, con una evoluci&oacute;n posquir&uacute;rgica    favorable, y el resultado anatomopatol&oacute;gico suprarrenal confirm&oacute;    el diagn&oacute;stico de mielolipoma. </font>      <P><font size="2" face="Verdana">En noviembre de 2009 la paciente comenz&oacute;    a referir astenia e incremento en el peso corporal (sobrepeso corporal: IMC    de 27,2 kg/m<SUP>2</SUP>). Se hicieron estudios para evaluar la funci&oacute;n    corticosuprarrenal y no se encontraron alteraciones. En ese ingreso se detect&oacute;    al examen f&iacute;sico un bocio grado I (el&aacute;stico y difuso) y bradicardia    (56 latidos/min), adem&aacute;s de una hipercolesterolemia en el chequeo bioqu&iacute;mico    sin antecedentes previos. Se estudi&oacute; la funci&oacute;n tiroidea, se diagnostic&oacute;    un hipotiroidismo primario, y se prescribi&oacute; tratamiento con levotiroxina    s&oacute;dica. </font>     <P><font size="2" face="Verdana">Hasta la fecha se mantiene estable cl&iacute;nicamente    con su tratamiento sustitutivo tiroideo, y con seguimiento imagenol&oacute;gico    de la gl&aacute;ndula suprarrenal derecha sin evidencias de disfunci&oacute;n    ni progresi&oacute;n en el crecimiento.</font>     <P>    <br>     <P><font size="2" face="Verdana"><B>Estudios complementarios</B> </font>     <P><font size="2" face="Verdana"><I>Generales</I> </font>     <P><font size="2" face="Verdana">- Hemoglobina: 137 g/L    <br>   </font><font size="2" face="Verdana">- Conteo global de leucocitos: 6,0 x 10<SUP>9</SUP>/L    ]]></body>

En la zona de la    suprarrenal derecha se identific&oacute; imagen similar de 50 x 48 mm (<a href="#fig1">figura    1</a>).</font>     <P><font size="2" face="Verdana">    <BR>   </font>     <P align="center"><a name="fig1"></a><img src="/img/revistas/end/v21n3/f0106310.jpg" width="450" height="351">      
<P><font size="2" face="Verdana">TAC contrastada de abdomen: masa hipodensa de    atenuaci&oacute;n grasa bilobulada de 79 x 64 mm en territorio suprarrenal izquierdo.    Uno de los l&oacute;bulos es de contenido graso homog&eacute;neo, y el otro,    mayor, con predominio graso, presenta &aacute;reas s&oacute;lidas y gruesa calcificaci&oacute;n.    Se observa desplazamiento del ri&ntilde;&oacute;n izquierdo (<a href="#fig2">figura    2</a>). A nivel de la suprarrenal derecha se identific&oacute; una imagen hipodensa    redondeada de atenuaci&oacute;n grasa (-116 unidades Hounsfield), bien encapsulada    y homog&eacute;nea, de 45 x 50 mm.    ]]></body>

<body><![CDATA[<P>     <P>      <P><font size="3" face="Verdana"><B>DISCUSI&Oacute;N</B> </font>     <P><font size="2" face="Verdana">En la actualidad se observa un incremento de    los tumores descubiertos incidentalmente, y dentro de sus causas, el mielolipoma    ocupa un lugar no despreciable en cuanto a frecuencia.<SUP>2</SUP> </font>     <P><font size="2" face="Verdana">Despu&eacute;s de descartar la disfunci&oacute;n    hormonal, ante tumores menores de 4 cm, con caracter&iacute;sticas imagenol&oacute;gicas    sugestivas de benignidad, puede adoptarse una conducta expectante que fue precisamente    la que se sigui&oacute; en los primeros 18 meses del diagn&oacute;stico con    esta paciente. El incremento de las dimensiones del tumor, as&iacute; como la    presencia de s&iacute;ntomas de compresi&oacute;n, justificar&iacute;an la adrenalectom&iacute;a    en estos casos por el riesgo incrementado de ruptura espont&aacute;nea, hemorragia    retroperitoneal y <I>shock </I>hipovol&eacute;mico, con la consiguiente morbimortalidad    que esto representa.<SUP>37,38</SUP> En esta paciente, la dimensi&oacute;n que    alcanz&oacute; evolutivamente la suprarrenal izquierda, fue lo que motiv&oacute;    la indicaci&oacute;n de la adrenalectom&iacute;a. </font>     <P><font size="2" face="Verdana">En cuanto a la bilateralidad del caso, se se&ntilde;alan    como importantes en el diagn&oacute;stico diferencial: los procesos malignos,    primarios o secundarios; y la HAC. Desde el inicio, la primera etiolog&iacute;a    no fue la de elecci&oacute;n, a pesar de la edad de la paciente, teniendo en    cuenta las caracter&iacute;sticas imagenol&oacute;gicas, en especial, los valores    de atenuaci&oacute;n reportados por la TAC (negativos). En relaci&oacute;n con    la segunda posibilidad diagn&oacute;stica, en la literatura aparecen descritos    unos 20 casos de mielolipoma e HAC. Coincidentemente, alrededor del 50 % son    bilaterales y la gl&aacute;ndula izquierda es generalmente mayor que la derecha    en la mayor&iacute;a de los casos, probablemente por condiciones anat&oacute;micas.<SUP>26<B>    </B></SUP>En nuestro caso se descart&oacute; este diagn&oacute;stico con el    resultado de la 17 hidroxi-progesterona, y con el criterio acerca de que, por    lo general, aparece en pacientes ya diagnosticados con d&eacute;ficit enzim&aacute;tico    de 21 hidroxilasa con tratamientos irregulares, o que han sido tard&iacute;amente    diagnosticados y tratados.<SUP>39-41<B> </B></SUP> </font>     <P><font size="2" face="Verdana">En cuanto a la coexistencia del mielolipoma y    la enfermedad tiroidea, solo encontramos 4 casos asociados con el hipertiroidismo.    Los autores tratan de explicar esta relaci&oacute;n, en cierta medida, porque    se ha encontrado respuesta a hormonas tiroideas en la gl&aacute;ndula adrenal,    y en algunos de los estudios se ha identificado la presencia de receptores para    estas hormonas (a y b) en el an&aacute;lisis inmunohistoqu&iacute;mico de los    mielolipomas.<SUP>33</SUP> </font>     <P><font size="2" face="Verdana">No encontramos en la literatura revisada caso    alguno de mielolipoma asociado con hipotiroidismo primario. No aparece entre    las endocrinopat&iacute;as reportadas que concomitan con este tipo de tumor    adrenal. Desde el punto de vista patog&eacute;nico esta relaci&oacute;n podr&iacute;a    estar condicionada por el perfil lip&iacute;dico del hipotiroidismo, donde se    produce un incremento de colesterol, fosfol&iacute;pidos, triglic&eacute;ridos    y lipoprote&iacute;nas de baja densidad (LDL), todo lo cual podr&iacute;a contribuir    al dep&oacute;sito de grasa tumoral.<SUP>42<B> </B></SUP>Esta relaci&oacute;n    tambi&eacute;n podr&iacute;a ser casual porque la frecuencia del hipotiroidismo    en edades como la de nuestra paciente no es baja. Por otro lado, el probable    tiempo de evoluci&oacute;n de ambas entidades tampoco definir&iacute;a la relaci&oacute;n    de causalidad, debido a que las 2 entidades pueden tener un largo per&iacute;odo    silente. </font>     <P><font size="2" face="Verdana">En cuanto a la conducta, la adrenalectom&iacute;a    bilateral implicar&iacute;a necesariamente la sustituci&oacute;n esteroidea    permanente. Por lo que, usualmente, se recomienda la adrenalectom&iacute;a unilateral    en los casos bilaterales, con el objetivo de preservar la funci&oacute;n adrenal,    y la gl&aacute;ndula contralateral se mantiene bajo seguimiento bioqu&iacute;mico    e imagenol&oacute;gico. </font>     <P><font size="2" face="Verdana">Esta experiencia cl&iacute;nica refuerza la importancia    de una adecuada historia cl&iacute;nica. Siempre ofrece datos b&aacute;sicos    que motivan a la b&uacute;squeda de comorbilidades que tambi&eacute;n necesitan    ser tratadas, para mejorar la calidad de vida y el perfil de riesgo de estos    pacientes. Hasta el momento, estas comorbilidades no modifican la decisi&oacute;n    quir&uacute;rgica de los casos con mielolipomas, en los cuales el tama&ntilde;o    de la lesi&oacute;n y su potencial maligno y/o funcional siguen siendo los factores    decisivos. </font>     ]]></body>
