<body><![CDATA[<p>&nbsp;</p>    <p>&nbsp;</p> <hr noshade size=1 />     <p align="justify"><font size="2" face="Verdana, sans-serif"><b>Se&ntilde;or editor:</b></font></p>      <p align="justify"><font size="2" face="Verdana, sans-serif">Hemos le&iacute;do un interesante art&iacute;culo de Medina Mart&iacute;n et al, que encontr&oacute; una elevada frecuencia de antecedentes patol&oacute;gicos de hipertensi&oacute;n arterial, obesidad y diabetes entre familiares de adolescentes<sup>1</sup>, lo que sugiere un v&iacute;nculo entre los genes y estos factores de riesgo. Un estudio similar hall&oacute; menores porcentajes de antecedentes familiares en estos trastornos <sup>2</sup>.</font></p>      <p align="justify"><font size="2" face="Verdana, sans-serif">En este comentario nos referiremos a los factores gen&eacute;ticos implicados en la hipertensi&oacute;n esencial, un aspecto menos conocido que los factores ambientales con controversias, que amerita un estudio m&aacute;s profundo. Se considera que alrededor del 40 % de los cambios en la presi&oacute;n arterial se deben a factores heredables y un 50 % a factores ambientales, aunque se requieren m&aacute;s estudios que esclarezcan las cuestiones pol&eacute;micas <sup>3,4</sup>.</font></p>      <p align="justify"><font size="2" face="Verdana, sans-serif">Nos concentraremos en tres grupos de genes: de la enzima convertidora de angiotensina (ACE), &nbsp;de los receptores <b>&alpha;</b> y <b>&beta;</b> adren&eacute;rgicos y del &oacute;xido n&iacute;trico sintasa endotelial.</font></p>      <p align="justify"><font size="2" face="Verdana, sans-serif">La ACE es la enzima reguladora del sistema renina-angiotensina-aldosterona (RAS), una cascada proteol&iacute;tica que juega un rol cr&iacute;tico en la homeostasia de la presi&oacute;n arterial, cuyas principales acciones son convertir la angiotensina <b>I</b> en su vasoconstrictor activo angiotensina <b>II </b>y la inactivaci&oacute;n de los p&eacute;ptidos vasodilatores bradicinina y kalidina <sup>5</sup>. El gen ACE se localiza en 17<b>q</b>23 y se ha demostrado que su variaci&oacute;n al&eacute;lica es responsable del 47 % de la variabilidad individual en los niveles circulantes de ACE. Otros componentes del RAS tambi&eacute;n se han involucrado en la hipertensi&oacute;n arterial <sup>6</sup>.</font></p>      <p align="justify"><font size="2" face="Verdana, sans-serif">Los receptores adren&eacute;rgicos (ARs) son receptores acoplados a prote&iacute;na G que representan los principales componentes del sistema nervioso simp&aacute;tico que juegan un papel cr&iacute;tico en la homeostasia del organismo y en regulaci&oacute;n de la presi&oacute;n arterial <sup>5</sup>. Debido al involucramiento de las catecolaminas en la regulaci&oacute;n del gasto energ&eacute;tico por estimulaci&oacute;n del metabolismo lip&iacute;dico a trav&eacute;s de estos receptores, la mayor&iacute;a de los estudios en ni&ntilde;os y adolescentes han asociado los polimorfismos de ARs con la obesidad y el s&iacute;ndrome metab&oacute;lico; otros estudios lo relacionan directamente con la hipertensi&oacute;n <sup>7</sup>.</font></p>      <p align="justify"><font size="2" face="Verdana, sans-serif">La isoforma endotelial de la &oacute;xido n&iacute;trico sintasa (eNOS) es la principal fuente de &oacute;xido n&iacute;trico en las c&eacute;lulas cardiovasculares <sup>3</sup>. El &oacute;xido n&iacute;trico es un regulador fisiol&oacute;gico de la presi&oacute;n arterial y el remodelado vascular, entre otras funciones. El gen eNOS se sit&uacute;a en 7<b>q</b>35-7<b>q</b>36 y sus variantes al&eacute;licas tambi&eacute;n se&nbsp; relacionan con la hipertensi&oacute;n arterial <sup>8</sup>.</font></p>      <p align="justify"><font size="2" face="Verdana, sans-serif"><b>Palabras clave:</b> hipertensi&oacute;n/gen&eacute;tica.</font></p>      ]]></body>
