Objetivo:  Identificar la influencia del ajuste del tratamiento según estratificación de riesgo en el control de la hipertensión.  Método:  Se realizó un estudio que incluyó 118 hipertensos con riesgo cardiovascular global moderado o alto, fueron seguidos durante un año cada cuatro meses. Se estudiaron las variables, edad, sexo, color de la piel, comorbilidades, obesidad, hábito de fumar, e ingestión de alcohol. Se determinaron creatinina, glucemia, colesterol, triglicéridos, ácido úrico, parcial de orina y microalbuminuria. Se ajustó el tratamiento según la estratificación de riesgo, y se registraron los ingresos y las descompensaciones en el año previo al estudio y durante el seguimiento.  Resultados:  La edad media fue de 64 ± 13,4 años, predominaron las mujeres (63,4%) y el color de la piel blanco (85,59%). Las comorbilidades más frecuentes fueron la cardiopatía isquémica (18,4%) y la insuficiencia cardíaca (13,56), la diabetes mellitus estuvo presente en 27,97%, las dislipidemias en 20,34%), el hábito de fumar en 11,86% y la obesidad en 10,17% de los pacientes. El control de la presión arterial mejoró un 20% y un 26,2% la adherencia al tratamiento. Se redujeron significativamente las descompensaciones (82,5%) y los pacientes descompensados (25,2%), (p&lt; 0,01). La drástica reducción de las descompensaciones y un mejor complimiento del tratamiento, sugieren que el ajuste terapéutico según estratificación de riesgo, resultó efectivo.  Conclusiones:  Los antecedentes de cardiopatía isquémica, insuficiencia cardíaca y diabetes mellitus fueron las principales causas de que los hipertensos clasifiquen como de alto riesgo. El ajuste del tratamiento según los niveles de riesgo logró una reducción en el número de descompensaciones y mejoró la adherencia al tratamiento.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[ABSTRACT  Introduction:  High blood pressure is the main risk factor for the occurrence of cardiovascular diseases.  Objective:  To identify the influence of treatment adjustment according to risk stratification on high blood pressure control.  Method:  A study was carried out including 118 hypertensive patients with moderate or high global cardiovascular risk, who were followed up every four months during one year. The variables studied were age, gender, skin color, comorbidities, obesity, smoking habit, and alcohol consumption. Creatinine, glycemia, cholesterol, triglycerides, uric acid, urinalysis and microalbuminuria were determined. Treatment was adjusted according to risk stratification, and admissions and decompensations in the year prior to the study and during follow-up were recorded.  Results:  Mean age was 64±13.4 years old. Females (63.4%) and white skin color (85.59%) predominated. The most frequent comorbidities were ischemic heart disease (18.4%) and heart failure (13.56), diabetes mellitus was present in 27.97%, dyslipidemias in 20.34%, smoking habit in 11.86% and obesity in 10.17% of patients. Blood pressure control improved by 20% and adherence to treatment by 26.2%. Decompensations (82.5%) and decompensated patients (25.2%) were significantly reduced (p&lt;0.01). The drastic reduction in decompensations and better treatment compliance suggest that therapeutic adjustment according to risk stratification was effective.  Conclusions:  A history of ischemic heart disease, heart failure and diabetes mellitus were the main causes due to which hypertensive patients classified as high risk. Adjustment of treatment according to risk levels achieved a reduction in the number of decompensations and improved adherence to treatment.]]></p></abstract>
